The question of standardising bacteriological methods is at present under discussion and, although it is most undesirable that any one method should be forced on all bacteriologists, it is certainly resonable that any method which has given excellent results in the hands of a number of independent workers should have a strong recommendation.

Haemolytic uraemic syndrome (HUS) caused by infection with Shiga toxin-producing Escherichia coli (STEC) is a relatively rare but potentially fatal multisystem syndrome clinically characterised by acute kidney injury. This study aimed to provide robust estimates of paediatric HUS incidence in England, Wales, Northern Ireland, and the Republic of Ireland by using data linkage and case reconciliation with existing surveillance systems, and to describe the characteristics of the condition. Between 2011 and 2014, 288 HUS patients were included in the study, of which 256 (89.5%) were diagnosed as typical HUS. The crude incidence of paediatric typical HUS was 0.78 per 100,000 person-years, although this varied by country, age, gender, and ethnicity. The majority of typical HUS cases were 1 to 4 years old (53.7%) and female (54.0%). Clinical symptoms included diarrhoea (96.5%) and/or bloody diarrhoea (71.9%), abdominal pain (68.4%), and fever (41.4%). Where STEC was isolated (59.3%), 92.8% of strains were STEC O157 and 7.2% were STEC O26. Comparison of the HUS case ascertainment to existing STEC surveillance data indicated an additional 166 HUS cases were captured during this study, highlighting the limitations of the current surveillance system for STEC for monitoring the clinical burden of STEC and capturing HUS cases.

Young pregnant women with HIV may be at significant risk of unplanned pregnancy, lower treatment coverage, and other adverse pregnancy outcomes. In a large cohort of pregnant women with HIV in Italy, among 2979 pregnancies followed in 2001–2016, 9·0% were in women <25 years, with a significant increase over time (2001–2005: 7·0%; 2006–2010: 9·1%; 2011–2016: 12·2%, P < 0·001). Younger women had a lower rate of planned pregnancy (23·2% vs. 37·7%, odds ratio (OR) 0·50, 95% confidence interval (CI) 0·36–0·69), were more frequently diagnosed with HIV in pregnancy (46·5% vs. 20·9%, OR 3·29, 95% CI 2·54–4·25), and, if already diagnosed with HIV before pregnancy, were less frequently on antiretroviral treatment at conception (<25 years: 56·3%; ⩾25 years: 69·0%, OR 0·58, 95% CI 0·41–0·81). During pregnancy, treatment coverage was almost universal in both age groups (98·5% vs. 99·3%), with no differences in rate of HIV viral suppression at third trimester and adverse pregnancy outcomes. The data show that young women represent a growing proportion of pregnant women with HIV, and are significantly more likely to have unplanned pregnancy, undiagnosed HIV infection, and lower treatment coverage at conception. During pregnancy, antiretroviral treatment, HIV suppression, and pregnancy outcomes are similar compared with older women. Earlier intervention strategies may provide additional benefits in the quality of care for women with HIV.

We aimed to assess the burden and trend of the HIV/AIDS epidemic among older adults over the past three decades at different geographical levels, based on the data collected from the Global Burden of Diseases (GBD) study 2019. This assessment identified the average annual percentage changes (AAPCs) using Joinpoint regression analysis. Globally, the incidence of HIV/AIDS has decreased (AAPC = −3.107); however, the overall prevalence has consistently increased (AAPC = 5.557). Additionally, both mortality (AAPC = 2.166) and disability-adjusted life years (DALYs; AAPC = 2.429) have increased. The highest increasing trends in female HIV/AIDS incidence and prevalence were observed in the Central Asia region. However, for males, these trends were observed in the Oceania region and the high-income Asia Pacific region, respectively. In recent decades, females aged 70–74 years had the highest incidence and prevalence, while males aged 70–74 years had highest mortality and DALYs in low social development index (SDI) regions. Unsafe sex resulted in 15 381.16 deaths, accounting for 90.73% of all HIV/AIDS deaths, and 331 140.56 DALYs, accounting for 91.12% of all HIV/AIDS DALYs. The HIV/AIDS disease burden differs by region, age, and sex among older adults. Sexual health education and targeted screening for older adults are recommended.

This subject was first considered statistically by the late Dr Farr. It is one of the brilliant attempts to extract the real meaning of figures so frequent in his work, but though this theory has not shared in the complete neglect that has been the lot of his attempt to put a quantitative measure to the course of epidemics, it has suffered as much from the kind of patronage with which it is usually discussed. On at least one of the great medical officers of health of his time, however,— the late Dr J. B. Russell—the theory exercised a strong fascination. My own copy of Fare's Vital Statistics came from Dr Russell's library, and the whole passage referring to the law is lined with his characteristic nervous pencil marks, while in much of his work on vital statistics the influence can easily be traced.

During a survey in Jakarta, Indonesia, 158 cultures of Salmonella oranienburg, consisting of two phage types, were obtained from 150 hospitalized patients with diarrhoea. Phage type I, though found notably in young children, was found in all age groups while phage type II was found almost exclusively in young children aged 0–7 years. Phage type I may produce a more severe clinical picture affecting all age groups alike, while phage type II may result in hospitalization of only the very young, who are more susceptible to dehydration. Phage type I was significantly more resistant than phage type II to the individual antibiotics: tetracycline, chloramphenicol, kanamycin and neomycin. However, there was no difference in their respective antibiotic resistance patterns as measured by disk and MIC assay. All cultures were sensitive to gentamicin and trimethoprimsulphamethoxazole 1:19.

The construction of a Regional Burns Centre in Pinderfields General Hospital, Wakefield, presented an opportunity to study Pseudomonas aeruginosa infection in patients with extensive burns. During the first year (Barclay & Dexter, 1968) a system of disinfection and bacteriological control created conditions permitting more detailed studies to be undertaken which resulted in a significant reduction of infection and cross-infection.

In a molecular investigation into the outbreak of HIV in Glenochil during the first 6 months of 1993, we previously demonstrated that 13 out of the 14 HIV positive inmates were infected with a virtually identical strain, and discounted 2 others as potential sources. Here we investigate a further 8 potential contacts and 4 potential sources which were identified in the companion paper [1]. We were able to examine viral sequence from all but one of these 12 and results have revealed them to be distinct both from each other and the original 14. Thus, despite an intensive follow-up investigation, we have been unable to identify any further HIV infections that might have been part of the 1993 outbreak. It is possible that persons who were infected at that time remain undetected; however this and the companion report strongly suggest that if this were the case the likely numbers would be few.

Experiments begun in 1925 in Egypt led the writer quickly to realise that comparable results in nutrition studies upon pigeons could only be obtained by a careful choice of the breed. It was found that lightly coloured birds and especially white bids were extremely sensitive to climatic change or to the harmful effects on an inadequate diet.

A single-tube confirmatory test that allows a result to be obtained in 4 h has been developed from the single-tube confirmatory test recommended by the Joint Committee of the Public Health Laboratory Service and the Standing Committee of Analysts (PHLS/SCA, 1980). A variety of river, lake and reservoir samples were examined for the presence of E. coli using either most probable number (MPN) or membrane filtration (MF) technique, and the PHLS/SCA recommended confirmatory medium (LTMB) was evaluated against traditional methods. To improve the performance of LTMB, the medium was modified and this modified medium when used in 0·1 ml volumes and incubated for 4 h at 44°C provided 99% agreement with traditional methods.

Stool specimens from 631 children with acute diarrhoea and from 277 healthy controls were tested for the presence of non-enteropathogenic, non-enterotoxigenic Escherichia coli strains which mediated mannose-resistant haemagglutination of human erythrocytes (MRHA + ). Fifty-nine (34·9%) of 169 isolated MRHA + strains but none of 210 MRHA - strains exhibited diffuse adherence (DA+) to HEp–2 cells. DA+ strains were found in 37 (5·9%) children with diarrhoea and in 22 (7·9%) controls. MRHA + /DA + strains in comparison to MRHA + /DA - strains significantly less frequently expressed P fimbriae (10·7 vs. 73·6%), haemolysin production (12·5 vs. 63·2%), and MRHA of other species erythrocytes (21·4 vs. 84%).

These data demonstrate that E. coli which exhibit the diffuse pattern of adherence to HEp–2 cells also cause MRHA of human erythrocytes. Since these strains were found with similar frequencies in children with and without diarrhoea it seems that DA is not a marker of enteropathogenicity of E. coli.

In Spain, the epidemic curve caused by COVID-19 has reached its peak in the last days of March. The implementation of the blockade derived from the declaration of the state of alarm on 14th March has raised a discussion on how and when to deal with the unblocking. In this paper, we intend to add information that may help by using epidemic simulation techniques with stochastic individual contact models and several extensions.

Genetic marker tests were performed on 61 strains of poliovirus isolated during the Blackburn outbreak from cases or suspected cases and their contacts. The results were correlated with the epidemiological data and good agreement was found for the serological and dextran inhibition tests. The reproductive capacity temperature marker, however, showed inhibition at 39·8° C. of strains otherwise judged to be of naturally occurring type. These strains could be differentiated from those of vaccine origin because they grew readily at 39·2° C. This feature also distinguished the strains isolated in Blackburn from those obtained in the nearby town of Hyde in a simultaneous outbreak.

Dr J. Ardley, Medical Officer of Health, Blackburn, kindly provided a key which enabled the contacts of cases to be traced; and Dr L. Robertson, Public Health Laboratory, Preston, made available his extensive records as well as the many strains isolated in his laboratory.

The bactericidal activity of Tego 103S was compared with that of chlorhexidine in ethanol, and Tego 103G with Halamid. The activity was determined on Pseudomonas aeruginosa, Staphylococcus aureus and Achromobacter anitratus by various methods. Tego 103S in 1% solution was less active than chlorhexidine 0·5% in 70% ethanol, and Tego 103 G in 1% solution was less active than Halamid 0·3 %. The presence of serum did not noticeably influence the activity of the Tego preparations.

Cysticercosis, a leading cause of acquired epilepsy in developing countries, has been controlled or eradicated in industrialized countries. This paradigm has recently been challenged, with human neurocysticercosis (NCC) being increasingly diagnosed in these countries. In order to assess the NCC burden in Portugal, a retrospective study on NCC hospitalizations (2006–2013) was conducted based on the national database on hospital morbidity: 357 hospitalized cases were detected. NCC was most frequent in the following age groups: 20–64 years (n = 197, 55·2%) >64 years (n = 111, 31·1%), and <20 years (n = 49, 13·7%). In the Norte and Centro regions cases tended to be older than in the Lisboa and Vale do Tejo Region. The results raise concerns for imported and autochthonous disease, suggesting the Lisboa and Vale do Tejo Region, due to its higher frequency of cases at younger ages, as a priority for research and intervention, and further suggest that NCC should be under surveillance (notifiable). The National Observatory of Cysticercosis and Taeniasis has been established and will define NCC cases as well as monitoring and surveillance.

Faraday Base represents a small isolated community producing its own domestic water by desalination of sea water. During the Antarctic winter of 1986 (April to October), regular bacteriological examination of the water supply and surrounding sea took place. Samples were collected and examined every 2 weeks by the methods described in the Department of Health and Social Security Report No. 71, on the Bacteriological Examination of Drinking Water Supplies (DHSS, 1982), for membrane filtration and colony counting. The results of these examinations are presented in this paper. The results obtained suggest that water of good bacteriological quality was produced by the desalination plant, but some samples from the distribution system contained coliforms or presumptive Escherichia coli in small numbers. The possible reasons for this low-level contamination are discussed. No cases of gastroenteritis occurred on the base during this time.

A new adsorbent, microporous polyethylene chopped fibre, was produced from a high density polyethylene. This can adsorb lipopolysaccharides (LPS) linearly up to 2 h, and showed the highest capacity to adsorb LPS when compared with two other polyethylene-based adsorbents and a polystyrene-based adsorbent. More than twice as much orange II and 4-nitroquinoline N oxide were adsorbed in the new adsorbent as was LPS. The adsorption isotherm of the new adsorbent for LPS was of Ln type, the correlation between adsorption and concentration of solute was proportional; whereas orange II and 4-nitroquinoline N oxide were of L type (greater adsorption than Ln type); tetrachloroethylene adsorption was of S type, less than Ln type. Adsorption of LPS to the new adsorbent increased when temperature rose, whereas adsorption of orange II and 4-nitroquinoline N oxide decreased. These data suggest that the binding of LPS to the new adsorbent is a hydrophobic interaction, whereas the binding of both orange II and 4-nitroquinoline N oxide is not. The new adsorbent has a greater potential for the removal of endotoxin from tap water than other commercially available adsorbents such as charcoal and Amberlite XAD-2.

Helicobacter pylori eradication therapy was included with insurance coverage from 1999 onwards in Japan, with the incidence of peptic ulcer expected to decrease as a consequence. This study investigated the temporal dynamics of peptic ulcer in Japan and identified underlying contributory factors using mathematical models. We investigated the seroprevalence of H. pylori and analysed a snapshot of peptic ulcer cases. Ten statistical models that incorporated important events – H. pylori infection, the cohort effect, eradication therapy and the natural trend for reduction – were fitted to the case data. The hazard of infection with H. pylori was extracted from published estimates. Models were compared using the Akaike information criterion (AIC), and factor contributions were quantified using the coefficient of determination. The best-fit model indicated that 88.1% of the observed snapshot of cases (AIC = 289.2) included the effects of (i) H. pylori infection, (ii) the cohort effect and (iii) eradication therapy, as explanatory variables, the contributions of which were 80.8%, 4.0% and 3.2%, respectively. Among inpatients, a simpler model with (i) H. pylori infection only was favoured (AIC = 107.7). The time-dependent epidemiological dynamics of peptic ulcers were captured and H. pylori infection and eradication therapy explained ⩾84% of the dramatic decline in peptic ulcer occurrence.
