Women with reproductive difficulties often feel stigmatized and isolated. Information concerning their specific experience can help plan psychosocial interventions.

The study aims to analyze reproductive difficulties representations in women with different coping orientations.

Participants: 48 women (aged 24-43) from support groups arranged by the “You Are Not Alone” non-profit organization for women with reproductive issues. Based on the questionnaire “Types of Orientations in Difficult Situation”, participants were divided into three groups: approach coping (N=16), avoidance coping (N=9), ambivalent coping (N=23). Content analysis was conducted based on stories about reproduction difficulties experiences.

We identified three types of representation of reproductive difficulties in women: approaching the goal of having a child; avoidance (fear of own death/illness/insanity or not having a child); ambivalent coping (alternating approach/avoidance). PTSD after recovery from COVID-19 has been correlated to sleep problems, high anxiety level and depressive manifestations. These sleep problems have their drastic effect on the recovered patients’ quality of life including physical, psychological and social domains.

The clinical high-risk for psychosis (CHR) is mainly established by the presence of attenuated positive symptoms (APS), but there is evidence of the role of attenuated negative symptoms (ANS) in the development of psychotic spectrum disorders. It is important to establish a link between APS and ANS in patients at CHR in order to improve early detection of psychosis.

Establish the relationship between APS and ANS in depressive patients at CHR.

130 depressive young in-patients at CHR with APS (average age 19.5) and 71 ones with ANS (average age 19.5) were examined. The HDRS scale was used to assess depressive symptoms, the SOPS scale was used to assess APS and ANS, and the SANS scale was used to assess ANS. The results are presented in median values.

No differences were found between two groups in the severity of depressive symptoms on the HDRS scale and CHR symptoms on the SOPS scale (22 vs 23.5 and 45 vs 43 respectively). Statistically valid differences have been established between the groups in the APS severity on the sub-scale of positive symptoms SOPS: 11 and 7 (p 0.001). No differences in the ANS severity on the sub-scale of negative symptoms were detected (17 and 18.5, p=0.207). There were also no differences in the ANS severity on the SANS scale (40 and 47, p=0.163).

It has been established that patients at CHR with APS also have ANS, which may have clinical significance for early detection of psychosis.

There is a lack of instruments evaluating self-stigma among adolescents with mental health issues in the Russian language for today. The questionnaire developed by Tally Moses (Moses, 2009) is convenient to fill that lack.

The study aims to compare the main parameters of the original questionnaire to that of the version translated in Russian.

The original questionnaire was translated into Russian and administered to 40 adolescents (21 males, aged 12 to 17) with mental disorders except for severe cognitive deficits or pervasive developmental disorders. Means and Cronbach’s alpha for each of the four scales were assessed and compared to the author’s questionnaire values.

Reliability analysis revealed similar Cronbach’s alpha for 3 of 4 scales (table 1) except the Secrecy scale (1 of 6 questions showed low consistency; its exclusion increased α from 0.63 to 0.74).

The means for each scale were compared with original data using a one-sample t-test. Only the Personal Rejection scale was significantly low on average than the original data.

Preliminary results showed that Russian adolescent patients perceived the translated questionnaire much the same way as American ones. Thus, our findings provide optimistical perspectives of further adaptation of the questionnaire.

Despite the high prevalence of psychiatric disorders among children and adolescents, services are often accessed too late or not at all. Inpatient-equivalent treatment can be a good option here, as it can counteract structural barriers by enabling treatment from home. Although national and international studies highlight the benefits, this form of treatment is offered by only a few psychiatric facilities.

The aim is to provide a decision-making aid for the introduction of outreach treatments with regard to cost-effectiveness. Based on this, the question will be answered whether telemedicine can be an option for the distribution of rare (human) resources.

1) Based on a systematic review, the best available evidence will serve for deriving hypotheses and providing assumptions for the decision-making model. 2) Decision analytic modeling will be used to determine the cost-effectiveness of inpatient-equivalent treatment compared to conservative inpatient treatment. 3) An additional systematic review will provide information on the use of telemedicine in inpatient equivalent treatment.

The following questions need to be discussed: 1)Is there an indication for all psychiatric diseases and age groups? 2) Are there ethical considerations that need to be taken into account, especially in the use of telemedicine? What incentives need to be set for psychiatrists to opt for inpatient-equivalent treatment?

The results of the study may help to raise awareness of inpatient equivalent treatment among decision-makers. Furthermore, fears could be reduced, since admission to a psychiatric facility can mean a stigmatizing intervention in the lives of young patients.

The study of the premanifest stages of the first psychotic episode with religious delusion is relevant due to the lack of clarity in differentiating normal religiosity from pathological and, as a consequence, a relatively longer period of an untreated psychotic state, which negatively affects both the course of the disease and its outcomes. Another important factor is the high risk of antisocial, autoaggressive and suicidal behavior at different stages of the disease.

The aim of the study is to identify the conditions for the formation of religious delusion in adolescence and young adults, to analyze the correlations between religiosity at the pre-manifest stage and the subsequent manifest psychotic episode with religious delusions of different content.

The 57 male patients at a young age (16-25 years) with a manifest psychotic episode (F20, F25 according to ICD-10) with religious delusion (delusion of sin (21,6 %), delusion of demonic possession (29,4 %), antagonistic and messianic delusion (39,2 %), oneiroid with religious content (9,8 %)) were studied with the clinical-psychopathological, psychometric (PAS) methods. The religiosity of patients in premorbid was assessed with the Duke University Religion Index (DUREL) questionnaire.

Of greatest importance in the formation of psychotic episode with religious content is hereditary burden, premorbid personality structure, high scores on the PAS scale.

The presence or absence of religiosity in premorbid doesn’t matter to formation of psychotic episode with religious delusion.

Kratom (Mitragyna speciosa) is an herb found in South East Asia belonging to the Rubiacea family, the active constituents being Mitragynine and 7-hydroxymitragynine. Sold as a dietary supplement in the form of a leaf, tablet, and powder, it has been gaining popularity as a natural supplement to alleviate pain, anxiety, depression, and manage opioid withdrawal symptoms. Our case report centers around a patient encountered with high-dose Kratom use who presented to our Psychiatric ER with psychosis.

The objectives of this case report are to raise awareness regarding the use of a newly popular substance easily available over-the-counter and the potential impacts it has on mental health.

PubMed was searched for the criteria Kratom AND Intellectual Disability, with a secodary search for Kratom AND Psychosis.

A 29-year-old male with a past psychiatric history of Schizoaffective Disorder, Borderline Intellectual Functioning, Polysubstance Use, ADHD, and six prior suicide attempts was brought to the Psychiatric ED by ambulance activated by his mother for severe psychiatric decompensation following ingestion of 270 pills of Kratom over the course of three days.

This case report further increases awareness of the dangers of Kratom use as well as brings to light the psychoactive properties of Kratom. This case report exposes areas where research can further expand understanding regarding the impacts Kratom can have on psychiatric populations.

According to WHO, 30-50% of workers experience psychological stress, which contributes to the development of mental and psychosomatic disorders. It’s important to develop the problem of prenosological diagnosis and determination of criteria for the formation of mental maladaptation as predictors of mental disorders in relation to occupational risk factors. Cognitive Adaptation Training (CAT) is a psychosocial intervention focusing on reducing the impact of cognitive disorders on daily functioning in people with severe mental illness (SMI). Similar to many evidence based practices (EBP), implementation of CAT in routine care lags behind, despite the established effectiveness of the intervention. This so called ‘science-to-service gap’ is a widespread problem in mental health care. We developed an innovative implementation program to facilitate implementation of CAT and similar interventions in routine care.

The aim of this study is to evaluate the effectiveness of the implementation program and to determine factors that impede or facilitate the implementation process.

We conducted a multicenter cluster randomized controlled trial comparing the implementation program to a single training program in four mental health institutions (a total of 21 rehabilitation teams) in The Netherlands. Focus groups, semistructured interviews and questionnaires were used at multiple levels of service delivery (service user, professional, team, organization). Assessments took place before, during and after implementation and at follow-up, adding up to a total duration of 14 months. Data were analyzed using multilevel modeling.

Data collection is complete and analyses on the effectiveness of the implementation program are ongoing. Preliminary analyses show that team climate (p<.008) and organizational climate (p<.043) significantly predict the attitudes of mental health providers toward EBP.

This implementation research may provide important information about the implementation of psychosocial interventions in practice and may result in a program that is useful for Cognitive Adaptation Training, and possibly for psychosocial interventions in general.

In obsessive personalities and hypochondriacal neuroses, the COVID-19 pandemic has posed an increased risk of decompensation for affective disorders and even suicide attempts. Isolation, lack of treatment and prior monitoring, as well as the difficulty of identifying vital stressors, must be taken into account if an early intervention is to be carried out.

Posttraumatic nightmares are one of the most frequent symptoms in posttraumatic stress disorder. Prevalence can be up to 96%. These nightmares evoke the experienced traumatic event, causing a negative impact. Besides, they are and independent risk for suicide. There are different pharmacological and non-pharmacological options for PTN, despite is no optimal treatment.

To analyse the different treatment options for PTN.

This was a narrative literature review.

The two main treatments for PTN nowadays are the Imagery Rehearsal Therapy (IRT) and prazosin. IRT is a cognitive-behavioral intervention, that helps the patient to change the content of the nightmare to a “happier ending”. Prazosin is an alpha-adrenergic receptor antagonist that blocks the stress response in the central nervous system receptors. Although it was a promising drug, significant differences compared to placebo have not been found. There is growing data that suggests nabilone, a synthetic cannabinoid, could be helpful in PTN treatment. A clinical trial made in Canada revealed that 72% of patients experienced a complete disappearance or at least an important reduction of PTN.

PTN is a very common and distressing symptom in patients presenting PTSD. Nevertheless, there is no treatment with enough evidence for this pathology. On this account, it is fundamental to do more research in order to find and suitable treatment that can improve the quality of life of these patients.

Praecox feeling (PF) is a characteristic feeling of bizarreness or unease that a psychiatrist experiences when facing a patient with schizophrenia. This term, proposed by Rumke in 1941, was considered an important feature of a schizophrenia diagnosis. However, since the movement toward operational diagnostic methods in the late 1970s, it has fallen out of use.

This work aims to discuss the role of Praecox Feeling in the clinical approach to schizophrenia diagnosis.

PubMed database was searched using combinations of the terms “praecox”, combined with “feeling” and “schizophrenia”.

PF is sometimes experienced silently before the patient participates verbally. An experienced and attentive clinician can intuitively feels changes in the body posture, facial expression, the tone of the voice, motor behavior, and attitude that could look insignificant, but as a whole they present the patient as “definitely un-understandable.” Although there is lacking evidence to sustain the rehabilitation of the PF as a reliable and valid clinical criterion consistent with the operational approach, a broader scientific approach is called for. PF should not be trivialized, as is sometimes the case, into a quick diagnosis but could be a real determinant of medical decision.

Even though there may not be sufficient evidence to consider it valid clinical diagnostic criteria, it still appears to play an important role in the clinical decision-making process and should not be underestimated or stigmatized. This concept is not completely subjective and does rely on objective information, such as the patient’s behaviour and body language.

Intimate partner violence (IPV) and psychological distress are major public health concerns among emerging adult women.

To study the self-esteem of women victims of domestic violence and to determine its associated factors .

This was a cross-sectional descriptive study carried out at the National Health Fund of Sfax among women who consulted during the months of October and November 2019. The sociodemographic and clinical characteristics of the consultants were collected using a pre-established form. Women’s Experience with Battering Scale” (WEBS) was used to screen (IPV). The Rosenberg Self-Esteem Scale (RSE), was used for evaluating individual self-esteem. It uses a scale of 0–4 where a score less than 25 may indicate a problematic very low self esteem.

The sample comprised 110 women. More than half (66.7%) of women had a primary school level and 69% had a median socioeconomic level. (IPV) was estimated at 57.3% in our population. The mean (WEBS) score was 30.92(SD=9.8) and the mean (RSE) score was 31.26 (SD=3.5) among abused women self-esteem was very low in15.5% and low in 42.7% Abused women were more likely to have a low self-esteem (r= -0.528 ;p=0.012) The score of self-esteem decreased with age (r=-0.685 p=0.0001) and previous history of violence(p=0.04).

The findings suggests that Women who experienced (IPV) were more likely to have a low self-esteem. Therefore, the role of the physician is essential not only in the care of the victims but also in the detection of psychological repercussions.

Online sports betting (OSB) is frequently associated with gambling disorder (GD). In Tunisia, no study on this has been done so far.

To detect GD in a population of Tunisian OSB players, and to identify its risk factors.

This was a cross-sectional study of 58 male OSB players in the city of Sfax. The GD was assessed by a questionnaire relating to the DSM-V criteria. Depression, anxiety and stress were assessed using the DASS scale, gambling motivations using the GMQ-F scale, and impulsivity using the UPPS-P scale.

The mean age was 37.4 ± 8.29 years. The prevalence of JAP was 53.4%. On univariate analysis, the factors associated with GD were university level of education, the practice of other gambling, daily gambling, gambling spending > 300 Dinars / month , gambling duration > 3 years, the frequency of winning >1 win /6months , the occurrence of a Big Win, total GMQ-F score, coping motivation , and financial motivation. In the multivariate study, GD risk factors were gambling spending > 300 Dinars / month (p = 0.011; ORa = 223.16), financial motivation (p = 0.022; ORa = 3.967), pathological stress (p = 0.036; ORa = 224.388) and inversely associated with the age at onset of gambling (p = 0.026; ORa = 0.751) and the UPPS score (p = 0.011; ORa = 0.6).

Our results push us to deepen our knowledge and our studies concerning this problem in our country and to reflect on the management and prevention measures.

Residency training has been reported as being stressful which may lead to different mental disorders.

To study the prevalence and associated factors of anxiety and depression symptoms among psychiatry residents.

A cross-sectional study was conducted through an online survey among psychiatry residents. Participants completed an anonymous self-administered questionnaire and the HADS questionnaire for screening anxiety and depression.

Forty responses were collected.The average age of the sample was 28.08 ± 2.43 and the sex-ratio (F/M) was 0.875. Eleven participants were married. Eight residents were smokers. The prevalence of alcohol use and cannabis use was 22.5 % and 5% respectively. Half of participants were first year residents and near three-quarter of them (72.5%) declared working in poor conditions. A considerable proportion of participants had symptoms of anxiety and depression. The prevalence of anxiety case and depression case was 52.5% and 47.5% respectively. The prevalence of Anxiety symptoms and depression symptoms was significantly higher in female participants (p = 0.017, p=0.034 respectively). Poor conditions of the workplace were significantly associated with depression symptoms (p=0.004).

Training residents in psychiatry showed high rates of anxiety and depression symptoms. Screening and early management of these psychiatric manifestations is necessary. In addition, improving working conditions would upgrade their training and quality of life.

Research on the prevalence of and risk factors for insomnia among refugee populations is limited and tends to focus on pre-migratory trauma. Yet, post migratory stressors are just as important for mental health and may also relate to insomnia.

Objective: To determine the association between different post-migration stressors and insomnia among Syrian refugees living in Norway.

We used data from the REUFGE study, a cross-sectional survey with 902 Syrian refugees who arrived in Norway between 2015 and 2017. Insomnia was measured with the Bergen Insomnia Scale and post-migrant stress with the Refugee Post-Migration Stress Scale (RPMS). We applied logistic regression analyses to investigate the association between seven different postmigration stressors and insomnia after controlling for demographics, traumatic experiences and post traumatic stress symptoms.

Of the 873 participants who completed questions on insomnia, 515 (41%) reported insomnia. There was no significant difference between men and women. The most commonly reported postmigration stressors were Competency Strain [SML1], Family and Home Concerns, and Loss of Home Country. After controlling for demographics, traumatic experiences and post-traumatic stress symptoms, Financial Strain, Loss of Home Country, Family and Home Concerns and Social Strain were still associated with higher odds of insomnia.

Resettlement difficulties are related to poorer sleep among refugees. Measures to improve the social conditions and financial concerns of refugees in receiving countries could potentially reduce insomnia among refugees which in turn, may benefit mental and physical health.

Suicidal behavior remains an important clinical problem and a major cause of death in youth.

The purpose of this study was to describe the epidemiological and clinical profile of adolescents with suicide attempts.

This is a retrospective descriptive study that focused on a population of Tunisian adolescents aged between 10 and 19 years old and who were hospitalized after a suicide attempt between between January, 1st 2010 and November,15th 2018, in Razi Hospital.

We used a pre-established questionnaire that explored the sociodemographic and clinical data of patients.

Sixty adolescents were included in this study. The average age of the respondents was 14.3±2 years. The sex-ratio (m/f) was 0, 36. The suicidal adolescent was a female (73%), single (98%), enrolled in school (66%) with school failure history (52%). Family history of suicide was reported in 8%. Fifty adolescents (83%) lived with their parents and the relationship with them was described as disturbed in 60% of them. A history of physical and sexual abuse was reported in 25% during first adolescence and 15% during second adolescence. The most frequent diagnoses were adjustment disorder with depressed mood (45%) and depression (28%). Drug ingestion was the most common mean of suicide (63%), in an impulsive way in 82% of cases.

Development of repeated epidemiological surveys makes it possible to better understand the prevalence of suicide attempts in adolescents and to implement suicide prevention programs.

Patients with schizophrenia suffer from cognitive difficulties expressed in the form of complaints as well as poor insight. The interaction of these two factors makes the management of these patients more difficult.

To assess subjective cognitive complaints in a population of schizophrenics and study its relationship to insight.

Our study was a cross-sectional, descriptive, and analytical study of 72 stabilized schizophrenics followed up at the outpatient clinic. Subjective cognitive complaints were assessed by the SSTICS, clinical symptoms by the PANSS, and insight by the SAI-E.

The mean age of our population was 46.83± 11.6 years. The patients had a low socio-economic level in 70.1%. They were unemployed in 46.9% , consumed alcohol in 23.6%, and consumed tobacco in 58,6% of the cases. the total score on the PANSS scale was 46. They had an average score of 25 on the total SSTICS score and 20,1 on the SAI-E. Cognitive complaint scores were significantly correlated with improved insight (p=0,00),low socio-economic level (p=0.04),alcoholism (p=0.001) and smoking (p=0.01)

Cognitive complaints in schizophrenia could be influenced by the level of clinical insight and reflect a deep malaise, requiring a more targeted and optimized management

Social stigma indicates a process of negative connotation of a person which results in discrimination. The victim of stigma experiences a condition of social exclusion that negatively affects his relationships. The COVID-19 pandemic has spread the fear of being “contaminated”, which has led to the discrimination of a part of population.

The purpose of this work is to analyze which people have suffered from stigma due to the COVID-19 pandemic by examining the negative effects on their health during this period.

A literature review of peer-reviewed articles was performed on Pubmed NCBI database by inserting the keywords: stigma and COVID-19 in the period 2020-2021.

The data showed that the categories most at risk were positive patients and their families; healthcare workers in COVID-19 wards; Asian people. Discrimination has included avoidance attitudes, physical or verbal abuse, hypersurveillance in public places. Negative effects on victims included anxiety, depression, feelings of rejection and shame, self-harm and suicide. COVID-19 patients attempted to hide the disease by avoiding access to hospital; health personnel developed risk of burnout; Asian restaurants experienced a drop in reservations, even after quarantine period.

Social stigma is a public health problem and greater efforts are mandatory to reduce it including correct information, with the help of social and mass media; social interventions aimed at generating empathy; avoiding the use of negative language focused on stereotypes that could generate fear or discrimination. Such interventions are crucial to reduce discrimination in such a fragile period as COVID-19 pandemic.

Realizamos un estudio prospectivo en 15 pacientes, diagnosticados de psicosis, según el DSM V, y que son usuarios de una Unidad de Rehabilitación activa, estos pacientes utilizan las modalidades de hospitalización residencial o el programa de hospitalización parcial.

El objetivo principal fue establecer el perfil del paciente (retrato de robot), que utiliza este tipo de dispositivo.

Se lleva a cabo un protocolo de evaluación que incluye la entrevista de evaluación psicopatológica (DSMV) y las escalas: impulsividad (Barrat), agresividad (Burke), calidad de vida (Woqol bref), actitud ante la medicación (DAI), Avd ( RAI), riesgo de suicidio (Plutchick), riesgo de caídas (Downton), funcionalidad (EEAG) y hábitos de consumo (DAU).

Con todos estos elementos y teniendo en cuenta la edad y sexo de los pacientes, intentamos establecer el tipo de perfil de paciente del área de Rehabilitación activa de nuestro servicio, luego luego de mediciones periódicas de todas estas variables estableceremos la influencia de Terapia de rehabilitación en la mejora o empeoramiento de nuestros pacientes.
