Towards the close of the Great War, when economy of food consumption was an urgent necessity, considerable attention was paid to the determination of the energetic needs of various kinds of muscular work. The majority of scientific papers then published dealt with the problem.

In 1927 Hilda M. Woods and I contributed to this Journal a study of some aspects of a syndrome called “Status Thymicus,” “Status Thymico-Lymphaticus” or simply “Status Lymphaticus.” We were moved by three considerations: (1) that Prof. H. M. Turnbull had placed at our disposal a long series of weighings of the Thymus, the alleged principal villain of the piece; (2) that every year a not inconsiderable number of deaths are put to the account of this syndrome; (3) that the first volume of Prof. J. A. Hammar's monograph on the Thymus had recently been published. Since our publication, the annual quota of victims shows no signs of diminution (Table I) and the second volume of Prof. Hammar's work has appeared.

In Canada, vaccination policies against the 2009 influenza H1N1 pandemic (H1N1pdm09) were modified at different times during the autumn wave. We hypothesized that ethnicity and place of residence influenced the odds of vaccination. To test this hypothesis, we used vaccination databases for the entire province of Manitoba, and obtained the age distribution of vaccination for First Nations (FN) and non-First Nations (non-FN) populations. We used regression analysis to determine the effect of ethnicity and location of residence on odds of vaccination. We found that individuals with FN identity were over 2.8 times [95% confidence interval (CI) 2.79–2.87] more likely to receive vaccination compared to non-FN individuals. For the FN populations, on-reserve residency was associated with 5.15-fold (95% CI 5.00–5.30) higher odds of vaccination compared to off-reserve residency. Our study highlights the importance of demographic and geographical variables in developing strategies for vaccine prioritization.

Young adult mice were inoculated intravenously with strains JB or Peter C of Mycoplasma pulmonis. A few were inoculated intranasally with strain JB. This strain but not Peter C was isolated for 50 days or more from the urines of more than half of the mice. Those of strains TO, C3H and CBA, but not CFLP, were susceptible. Recovery of mycoplasmas was intermittent and sometimes the numbers isolated varied within individual mice and between mice of a particular strain, ranging from 5 × 101 to <5 × 107 colour-changing units/ml. Fifty serial passes of M. pulmonis, strain JB, in mycoplasmal medium resulted in attenuation, the organisms after inoculation of TO mice not being recovered from the urine and excretion not being stimulated by treating the mice with progesterone. At autopsy, the organisms of early passage were usually but not invariably isolated from the kidneys of mice that had been urinary excretors. About half of the latter had no renal histopathological changes. The others had usually minimal renal perivascular lymphocytic infiltrates but occasionally more widespread inflammatory changes. The findings may have relevance to the spread of mycoplasmal infection within mouse colonies and suggest that an association between such infection and nephritis in other species, including man, should be sought more closely.

In the first place cultures of the nitroso-bacterium were developed in inorganic solutions. These carried to a 5th dilution exhibited practically a pure culture of this species.

Secondly, cultures of the nitroso-bacterium were inoculated into solutions containing small quantities of organic matter. In these they were able to oxidise the ammonia present. It was found that a culture developing in the presence of small quantities of organic matter was better able to oxidise the ammonia in higher percentages of organic matter than a culture taken directly from an inorganic solution.

In growing this species on plates silica jelly has the disadvantage of being difficult to prepare. It was 18 months before I obtained a satisfactory plate. This jelly grows the nitroso-bacterium well, but other species can also develop colonies on it. Ammonia agar also grows the species, as shown by the oxidation of the ammonia, and the colonies assume a characteristic form.

As to beef broth agar and gelatine, these media grew colonies of a micro-organism similiar to the nitroso-bacterium from oxidised ammonia cultures. Pieces of these plates containing such colonies in pure culture frequently oxidised ammonia in solutions. On the other hand pieces of such plates showing no colonies never produced any oxidations of the ammonia. Furthermore the single colonies from silica and ammonia agar plates, which oxidised the ammonia in the media into which they were sub-cultured, grew well on beef broth agar and gelatine.

From the above results I have come to the following conclusions:

(1) That the nitroso-bacterium grows well on any ordinary medium.

(2) That the supposed parallel organism is no other than the nitroso-bacterium itself.

(3) That in the presence of large percentages of organic matter the nitroso-bacterium, although growing very profusely, loses for a time the power of converting ammonia into a nitrite.

Nearly 1 year into the coronavirus disease 2019 pandemic, the first severe acute respiratory syndrome coronavirus 2 vaccines received emergency use authorisation and vaccination campaigns began. A number of factors can reduce the averted burden of cases and deaths due to vaccination. Here, we use a dynamic model, parametrised with Bayesian inference methods, to assess the effects of non-pharmaceutical interventions (NPIs) (such as social distancing, mask mandates, school and workplace closure), and vaccine administration and uptake rates on infections and deaths averted in the United States. We show that scenarios depicting higher compliance with NPIs avert more than 60% of infections and 70% of deaths during the period of vaccine administration, and that increasing the vaccination rate from 5 to 11 million people per week could increase the averted burden by more than one-third. These findings underscore the importance of maintaining NPIs and increasing vaccine administration rates.

1. The growth of diphtheria bacilli in suger-free pure casein digest broth may be accelerated by the addition of dilute nitric, acetic and lactic acids, and delayed by certain monobasic fatty acids.

2. The limiting acid zone for toxic and non-toxic varieties is unaffected by initial dilute acidifications by several acids, but incresaed by the addition of citric acid.

3. The time periods and intensity of the reaction changes produced by diphtheria bacilli growing in a sugar-free casein digest medium are altered by the addition of certain acids. Toxic strains produce earlier and more progressive alkalinity when dilute acetic or nitric acid is included in the medium: and later and lessened alkalinity in the presence of butyric, formic, and propionic acids. Certain other typical acids do not exert any obvious action. Non-toxic strains are less affected by the selected acids, although butyric acid generally retards reaction changes.

4. The time required for virulence testing is decreased, and the potency of the bacillus under examination is more evident when 1/200 η acetic or nitric acid is added to the culture medium. Acidified casein digest plus peptone broth yields practically the same results as acidified fermented veal peptone infusion. The acidification does not affect the metabolism of non-toxic strains.

5. The disadvantages of unfermented meat infusions and the ascribed harmful effects of the fermentation products in fermented meat broths, may perhaps be avoided by the substitution of casein digest peptone medium. The advantages of the starting action of the glucose may be retained by adding acetic or nitric acid to the casein digest.

6. Individual strains of diphtheria bacilli differ considerably in their capacity of altering the reaction of culture media, and in their toxic potency. It does not seem permissible to apply to newly isolated organisms all the published results obtained from cultures that have been repeatedly subcultured and established as toxin producers in nutrient bouillon. As a presumptive test in the virulence testing of bacilli just transferred from human tissues to artificial culture media the tendency of non-toxic strains to produce early formation of alkali in adequately buffered and adjusted unfermented veal broth may be thought worthy of extended trial.

The main Neisseria meningitidis adhesion molecules, type IV pili (Tfp) and Neisseria adhesion A (NadA), play important roles in the pathogenesis of invasive meningococcal disease. PilE is the major Tfp subunit. In this study, the prevalence and genetic diversity of pilE and nadA were investigated in the prevalent serogroups and clonal complexes (CC) of N. meningitidis isolated in China. All serogroup A strains belonging to CC1 and CC5 and all CC11 serogroup W135 strains were clustered into class II PilE clades. All serogroup C and most of serogroup B isolates except CC8 and ST5642 were class I PilE clades. Class II pilE sequences were highly conserved. All isolates belonging to class I PilE isolates were nadA negative. However, nadA-positive strains were exclusively found in CC5 and CC11 isolates (class II PilE). This study showed that PilE and NadA may be related to epidemic or endemic meningococcal disease.

1. Various workers in different countries claim that in the sera of trachoma patients a positive Weil-Felix reaction is found and regard this as evidence that the casual agent of trachoma is a Rickettsia.

2. The examination of 200 sera from trachoma patients in the Sudan has failed to reveal any significant differences with regard to the Weil-Felix reaction between them and twenty-two sera from non-trachomatous controls, or 1000 sera taken, without reference to trachoma, from a large and representative section of the population.

3. Clinical and serological evidence suggest that typhus is absent from the Sudan. For this reason the interpretation of results is less liable to confusion in the Sudan than in countries where positive Weil-Felix reactions may occur in a varying proportion of the population due to typhus infections.

Dilution series of viruses often do not fit the exponential curve which they should fit if the usual assumptions of the theory of dilution series were true. Suppose the density of the virus suspension is λ, the average number of virus particles per unit volume of the inoculum (we suppose that unit volume is the amount actually used in inoculating the egg or other test material). Let C be the proportion of infective virus so that Cλ is the effective density of virus particles, and X is a factor depending on the dilution used such that CλX is the expected number of infective particles in the inoculum.

In a rational world, scientific effort would reflect society's needs. We tested this hypothesis using the area of infectious diseases, where the research response to emerging threats has obvious potential to save lives through informing interventions such as vaccination and prevention policies. Pathogens continue to evolve, emerge and re-emerge and infectious diseases that were once common become less so or their global distribution changes. A question remains as to whether scientific endeavours can adapt. Here, we identified papers on infectious diseases published in the four highest ranking, health-related journals over the 118 years from 1900. Focussing on outbreak-related and burden of disease-related metrics over the two time periods, 1990 to 2017 and 1900 to 2017, our analyses suggest that there is little underrepresentation of important infectious diseases among top ranked journals. Encouragingly our results suggest the scientific process is largely self-correcting.

We report a familial cluster of 24 individuals infected with severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2). The index case had a travel history and spent 24 days in the house before being tested and was asymptomatic. Physical overcrowding in the house provided a favourable environment for intra-cluster infection transmission. Restriction of movement of family members due to countrywide lockdown limited the spread in community. Among the infected, only four individuals developed symptoms. The complete genome sequences of SARS-CoV-2 was retrieved using next-generation sequencing from eight clinical samples which demonstrated a 99.99% similarity with reference to Wuhan strain and the phylogenetic analysis demonstrated a distinct cluster, lying in the B.6.6 pangolin lineage.

The AEGISS (Ascertainment and Enhancement of Disease Surveillance and Statistics) project uses spatio-temporal statistical methods to identify anomalies in the incidence of gastrointestinal infections in the UK. The focus of this paper is the modelling of temporal variation in incidence using data from the Southampton area in southern England. We identified and fitted a hierarchical stochastic model for the time series of daily incident cases to enable probabilistic prediction of temporal variation in risk, and demonstrated the resulting gains in predictive accuracy by comparison with a conventional analysis based on an over-dispersed Poisson log-linear regression model. We used Bayesian methods of inference in order to incorporate parameter uncertainty in our predictive inference of risk. Incorporation of our model in the overall spatio-temporal model, will contribute to the accurate and timely prediction of unusually high food-poisoning incidence, and thus to the identification and prevention of future outbreaks.

The extensive register infrastructure available for coronavirus disease 2019 surveillance in Scania county, Sweden, makes it possible to classify individual cases with respect to hospitalisation and disease severity, stratify on time since last dose and demographic factors, account for prior infection and extract data for population controls automatically. In the present study, we developed a case–control sampling design to surveil vaccine effectiveness (VE) in this ethnically and socioeconomically diverse population with more than 1.3 million inhabitants. The first surveillance results show that estimated VE against hospitalisation and severe disease 0–3 months after the last dose remained stable during the study period, but waned markedly 6 months after the last dose in persons aged 65 years or over.

We reviewed all diagnoses of Shigella species notified to the UK Health Security Agency from January 2016 to March 2023. An overall increase in notifications of shigellosis was seen between 2016 (n = 415/quarter) and 2023 (n = 1 029/quarter). However, notifications dramatically declined between March 2020 and September 2021 during the COVID-19 pandemic (n = 208/quarter) highlighting the impact of travel and social distancing restrictions on transmission. S. sonnei diagnoses were more affected by lockdown restrictions than S. flexneri, most likely due to a combination of species-specific characteristics and host attributes. Azithromycin resistance continued to be associated with epidemics of sexually transmissible S. flexneri (adult males = 45.6% vs. adult females = 8.7%) and S. sonnei (adult males = 59.5% vs. adult females = 14.6%). We detected resistance to ciprofloxacin in S. sonnei from adult male cases not reporting travel at a higher frequency (79.4%) than in travel-associated cases (61.7%). Extensively drug-resistant Shigella species associated with sexual transmission among men almost exclusively had ESBL encoded by blaCTX-M-27, whereas those associated with returning travellers had blaCTX-M-15. Given the increasing incidence of infections and AMR, we recommend that enhanced surveillance is used to better understand the impact of travel and sexual transmission on the acquisition and spread of MDR and XDR Shigella species.

Migration is an important risk factor for malaria transmission for malaria transmission, creating networks that connect Plasmodium between communities. This study aims to understand the timing of why people in the Peruvian Amazon migrated and how characteristics of these migrants are associated with malaria risk. A cohort of 2,202 participants was followed for three years (July 2006 - October 2009), with thrice-weekly active surveillance to record infection and recent travel, which included travel destination(s) and duration away. Migration occurred more frequently in the dry season, but the 7-day rolling mean (7DRM) streamflow was positively correlated with migration events (OR 1.25 (95% CI: 1.138, 1.368)). High-frequency and low-frequency migrant populations reported 9.7 (IRR 7.59 (95% CI:.381, 13.160)) and 4.1 (IRR 2.89 (95% CI: 1.636, 5.099)) times more P. vivax cases than those considered non-migrants and 30.7 (IRR 32.42 (95% CI: 7.977, 131.765)) and 7.4 (IRR 7.44 (95% CI: 1.783, 31.066)) times more P. falciparum cases, respectively. High-frequency migrants employed in manual labour within their community were at 2.45 (95% CI: 1.113, 5.416) times higher risk than non-employed low-frequency migrants. This study confirms the importance of migration for malaria risk as well as factors increasing risk among the migratory community, including, sex, occupation, and educational status.

Tick-borne encephalitis (TBE) is a vector-borne infection associated with a variety of potentially serious complications and sequelae. Vaccination against TBE is strongly recommended for people living in endemic areas. There are two TBE vaccination schemes – standard and rapid – which differ in the onset of protection. With vaccination in a rapid schedule, protection starts as early as 4 weeks after the first dose and is therefore especially recommended for non-immune individuals travelling to endemic areas. Both schemes work reliably in immunocompetent individuals, but only little is known about how TBE vaccination works in people with HIV infection. Our aim was to assess the immunogenicity and safety of the rapid scheme of TBE vaccination in HIV-1 infected individuals. Concentrations of TBE-specific IgG > 126 VIEU/ml were considered protective. The seroprotection rate was 35.7% on day 28 and 39.3% on day 60. There were no differences between responders and non-responders in baseline and nadir CD4 + T lymphocytes. No serious adverse events were observed after vaccination. The immunogenicity of the TBE vaccination was unsatisfactory in our study and early protection was only achieved in a small proportion of vaccinees. Therefore, TBE vaccination with the rapid scheme cannot be recommended for HIV-1 infected individuals.

1. The existence of an anaerogenic strain of Bact. paratyphosum C is recorded in Egypt.

2. It was isolated in August 1937, together with an aerogenic strain, from the urine of a patient suffering from enteric fever.

3. Biologically the anaerogenic strain was identical with the aerogenic except that it did not produce gas from any “sugar”.

4. The anaerogenic strain retained its inability to produce gas from “sugars” for the three years during which tests were carried out, subculturing on various fluid media containing different sugars failing to cause any gas production.

5. Serologically there was no indication of any antigenic difference between the anaerogenic and the aerogenic strains.

6. The anaerogenic strain contained the Vi antigen and retained it for three years when kept at room temperature.

The risk ratio quantifies the risk of disease in a study population relative to a reference population. Standard methods of estimation and testing assume a perfect diagnostic test having sensitivity and specificity of 100%. However, this assumption typically does not hold, and this may invalidate naive estimation and testing for the risk ratio. We propose procedures that control for sensitivity and specificity of the diagnostic test, given the risks are measured by proportions, as it is in cross-sectional studies or studies with fixed follow-up times. These procedures provide an exact unconditional test and confidence interval for the true risk ratio. The methods also cover the case when sensitivity and specificity differ in the two groups (differential misclassification). The resulting test and confidence interval may be useful in epidemiological studies as well as in clinical and vaccine trials. We illustrate the method with real-life examples which demonstrate that ignoring sensitivity and specificity of the diagnostic test may lead to considerable bias in the estimated risk ratio.
