  • Depot neuroleptics are now commonly used for maintenance therapy of schizophrenia. Their physicochemical composition makes them ideal for such use because they are long-acting and show stable blood levels for prolonged periods. Their absorption from the site of injection may, however, be affected by local muscle factors. This paper describes three such factors in relation to fluphenazine decanoate: varying the site of injection; massage of the injection site; and effects of muscular exercise. Our results suggest that fluphenazine pharmacokinetics are not significantly affected by any of these factors.

  • Accidents and Drug Treatment in a Psychiatric Hospital

  • A survey of 351 accidents occurring in a two-year period in a psychiatric hospital showed that 77 per cent of the accidents involved female patients and 48 per cent involved patients with organic psychoses; 236 accidents were falls and 280 occurred in the ward setting.

    In 277 instances adequate controls were available. Seventy-five per cent of the accident patients had received a psychotropic drug on the day of the accident as opposed to 61 per cent of the controls. The possibility that the side-effects of psychotropic drugs may have contributed to some of these accidents is discussed.

  • Social Deprivation Amongst Short Stay Psychiatric Patients

  • Two samples of newly admitted psychiatric patients were examined. Of 558 patients admitted during one year, 76 (13.7 per cent) came in from transitory accommodation or no fixed abode; of 456 patients discharged 131 (28.7 per cent) either changed address during their stay in hospital or left without known accommodation. Of 102 patients in the wards and day hospital on one day, 29 (28.4 per cent) came in from transitory accommodation or no fixed abode, 66 (64.7 per cent) were unemployed, 51 (50 per cent) were living alone. Of the 74 inpatients 30 (40.5 per cent) were homeless and 27 (36.5 per cent) had no visitors. These results indicate that many patients have lost their community supports by the time they reach hospital.

  • The theory underlying medical classification is not always given sufficient regard. Here systematic methods developed in biology are summarised and compared with parallel developments in nosology. The classification of the functional psychoses is discussed as an example of a methodological problem in medicine. We consider that cladism could have a useful application in nosology, and could lead to classifications based on the concept of interrelationships of disease.

    Cognitive–behavioural therapy is efficacious in the treatment of major depressive disorder but response rates are still far from satisfactory.

    Aims

    To better understand brain responses to individualised emotional stimuli and their association with outcome, to enhance treatment.

    Method

    Functional magnetic resonance imaging data were collected prior to individual psychotherapy. Differences in brain activity during passive viewing of individualised self-critical material in 23 unmedicated out-patients with depression and 28 healthy controls were assessed. The associations between brain activity, cognitive and emotional change, and outcome were analysed in 21 patients.

    Results

    Patients showed enhanced activity in the amygdala and ventral striatum compared with the control group. Non-response to therapy was associated with enhanced activity in the right amygdala compared with those who responded, and activity in this region was negatively associated with outcome. Emotional but not cognitive changes mediated this association.

    Conclusions

    Amygdala hyperactivity may lessen symptom improvement in psychotherapy for depression through attenuating emotional skill acquisition.

  • Psychiatric treatment can enhance human morality. It can promote the emergence of moral motives and intentions, aid in the acquisition of skills essential to moral action, and help to develop the ability to apply moral understanding and skills in particular circumstances. Good psychiatric practice demands an honest appraisal of its moral dimension.

    People with neurodegenerative disease and mild cognitive impairment (MCI) may have an elevated risk of acquiring severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and may be disproportionally affected by coronavirus disease 2019 (COVID-19) once infected.

    Aims

    To review all eligible studies and quantify the strength of associations between various pre-existing neurodegenerative disorders and both SARS-CoV-2 susceptibility and COVID-19 illness course and outcome.

    Method

    Pre-registered systematic review with frequentist and Bayesian meta-analyses. Systematic searches were executed in PubMed, Web of Science and preprint servers. The final search date was 9 January 2023. Odds ratios (ORs) were used as measures of effect.

    Results

    In total, 136 primary studies (total sample size n = 97 643 494), reporting on 268 effect-size estimates, met the inclusion criteria. The odds for a positive SARS-CoV-2 test result were increased for people with pre-existing dementia (OR = 1.83, 95% CI 1.16–2.87), Alzheimer's disease (OR = 2.86, 95% CI 1.44–5.66) and Parkinson's disease (OR = 1.65, 95% CI 1.34–2.04). People with pre-existing dementia were more likely to experience a relatively severe COVID-19 course, once infected (OR = 1.43, 95% CI 1.00–2.03). People with pre-existing dementia or Alzheimer's disease were at increased risk for COVID-19-related hospital admission (pooled OR range: 1.60–3.72). Intensive care unit admission rates were relatively low for people with dementia (OR = 0.54, 95% CI 0.40–0.74). All neurodegenerative disorders, including MCI, were at higher risk for COVID-19-related mortality (pooled OR range: 1.56–2.27).

    Conclusions

    Our findings confirm that, in general, people with neurodegenerative disease and MCI are at a disproportionally high risk of contracting COVID-19 and have a poor outcome once infected.

    It has been suggested that psychosis genes might be associated with beneficial effects, explaining their high frequency in all human populations.

    Aims

    To test the relationship between academic success and the incidence of psychotic disorders.

    Method

    The unusually complete demographic and scholastic records available in Iceland were used to locate academically accomplished individuals and assess the probability of previously identified patients with mental disorders and their relatives being among such groups.

    Results

    Close relatives of successful students showed increased risks of psychosis. Individuals who subsequently developed psychosis and relatives of people with psychosis excelled in school performance, particularly in mathematics.

    Conclusions

    The study supports the hypothesis that stimulation associated with psychotic tendencies enhances performance in academic settings.

  • Despite extensive evidence for its effectiveness, electroconvulsive therapy remains the subject of fierce opposition from those contesting its benefits and claiming extreme harms. Alongside some reflections on my experiences of this treatment, I examine the case against electroconvulsive therapy and find that it appears to rest primarily on unsubstantiated claims about major ethical violations, rather than clinical factors such as effectiveness and risk.

  • Fifty years ago pharmacological discoveries transformed psychiatry but progress since then has been relatively slow and there is unease about the role of industry. Despite this, the possibilities of pharmacological treatment have improved in recent years but exploiting developments for the benefit of patients requires psychotherapeutic skill as well as a high level of scientific knowledge.

    There is highly replicated positive correlation between longer duration of untreated psychosis and poorer outcome

    Aims

    To study the effect of early intervention in first psychosis on one-year outcome using an historical quasi-experimental design

    Method

    We compare the outcome of two samples of first-episode psychosis from the same healthcare district at different time periods. The historical control sample was assessed during 19931994, before the establishment of a system for early detection of psychosis. The experimental sample is the early detection sample in the Early Treatment and Intervention in Psychosis study assessed during 1997–2000

    Results

    At 1-year follow-up, the early detection group was younger, had a smaller fraction of individuals with schizophrenia, had less severe negative and general symptoms and had more friends in the past year than the historical control group. No differences were found in clinical course (remission, relapse, continuously psychotic) or positive symptoms, but more patients in the early detection sample were treated as outpatients without hospitalisation

    Conclusions

    Early detection of schizophrenia in one healthcare sector is associated with less severe deterioration at 1 year

  • Pythagoras appears to have been the first to recognize “the profound effect of music upon the senses and emotions.” He tried to influence the mind and body with what he termed “musical medicine” (1).

    He declared that the soul could be purified from its irrational influences, and he made detailed investigations into the therapeutic value of the seven different Greek keys. Pythagoras is said to have cured many mental illnesses “by having certain specially prepared musical compositions played in the presence of the sufferer.” For example, Iamblichus, in his life of Pythagoras, relates how he devised certain melodies against despondency, others against anger and still others against desires.

  • The problem of differentiating impending hepatic coma from delirium tremens is one which arises less commonly in Great Britain than in the United States. It is a problem which occurs, of course, chiefly in connection with alcoholic patients and while alcoholism has declined in Great Britain it is becoming an increasing medical and social problem in certain sections of the United States (1). Thus, at the Boston City Hospital alcoholism was a factor in over 45 per cent. of patients with cirrhosis of the liver at post-mortem (2). At the Hammersmith Hospital in London alcoholism was an aetiological factor in 5 per cent. of patients with the clinical diagnosis of cirrhosis (3). There is no doubt that the problem of differentiating impending hepatic coma from delirium tremens is a common and relatively easy one in Boston, whereas in London it is a rare and therefore more difficult one.

  • The therapeutic effect of lithium in mania was first described by Cade (1949), but the possibility that lithium could have a prophylactic action in recurrent affective disorders was not suggested until some years later (Hartigan, 1963; Baastrup, 1964). The early studies of the prophylactic action of lithium were open trials, in which both investigator and patient knew that the patient was receiving the drug. More recently, controlled double-blind studies have confirmed these early reports (Melia, 1970; Coppen et al., 1971; Cundall et al., 19712).
