Data from 157 men in Nepal who had vasectomy reversal are analysed. Most sought reversal within 5 years of vasectomy. Half of the men sought reversal because of the death of a male child, and about one-fourth because of the loss of a female child. Re-marriage was the primary reason for only 10% of the men. Those having reversal because of the loss of a male child were generally younger, and for almost half of this group, the age of their last child at the time of their vasectomy was under 2 years. The results suggest that the demand for reversal could be considerably reduced by more careful screening of the potential vasectomy acceptors.

Indirect techniques are applied to estimate the current level of fertility and mortality among Kuwaiti nationals in Kuwait during 1980–85. The various methods discussed provide estimates in the vicinity of 45 per thousand for birth rate and 8 per thousand for death rate. While child spacing by birth order in Kuwait is about 2 years, the proportion of childlessness is very low (1%). As a consequence of lower mortality and stable high fertility, the Kuwaiti population remains young.

This paper, based on data from two independent but concurrent studies, seeks to show that the explanation for the inconclusive results concerning fertility and family type in India lies in the failure of previous researchers to take account of an important intervening variable, the family life cycle. Fertility levels are examined in nuclear and joint families in a village in Western India and changes in relationships at different stages in the family cycle are considered. The most important conclusion is that aggregate level observations which indicate that fertility varies according to family type are misleading. When other relevant factors such as age or stage in the family life cycle are controlled, there is little evidence of any intrinsic connection between fertility and family type.

Unintended pregnancy and sexually transmitted infections (STIs) pose a huge public health problem in the United States. Efforts towards reducing unintended pregnancies have previously focused on women, but the role of men in family planning and preventing unwanted pregnancy is becoming clearer. The primary objective of the study was to fully examine the utilization of family planning services by men in the US, and to determine whether factors such as race, health insurance type and number of sexual partners influenced their utilization and receipt of family planning services and STI-related health services. Data were from the 2006–2010 National Survey on Family Growth (NSFG) study conducted in the US. The study sample comprised 7686 men aged 14–44 who ever had sex with women, and who had had at least one sexual partner in the 12 months before the survey. The receipt of family planning and STI-related health services by this group of men was estimated. The results showed that non-Hispanic Black men were more likely to receive family planning and STI-related services than Hispanic and non-Hispanic White males. Given that non-Hispanic Black men are disproportionately affected by STIs and are a high-risk group, the finding that this group received more family planning and STI services is a positive step towards reducing the disproportionately high prevalence of STIs in men in this under-privileged population.

The inbreeding coefficient of a population, estimated from ecclesiastical Roman Catholic dispensations, results from the relative contribution of different degrees of relationships (uncle–niece, first cousin, etc.). The interpopulation comparisons of consanguinity patterns may be obscured by the fact that in 1918 the Roman Catholic Church norm regulating the closest marriageable kinship was modified, limiting the application for an ecclesiastical dispensation to relatives of third degree (second cousins) or closer. Depending on the length of the period before or after the change of regulation, coefficients and rates may differ. Deviation of frequencies for multiple marriages may also occur. The aim of the present paper is to determine how the chosen procedure based on ecclesiastical dispensations may affect results, regarding the inbreeding coefficient, the consanguinity rate, the structure of consanguinity and the close/remote kinship ratio. As a sample case, information from the Gredos mountain range (central Spain) has been used.

Data from a survey of 349 fertile married women are used to examine the significance of cultural context for contraceptive behaviour in a traditional Indian village. While sterilization was widely sanctioned, other methods were little known, favoured or used. Relationship with modernization indicators also varied for different contraceptive techniques.

Traditional respondents were as favourable towards sterilization as modern women, but modern women were significantly more likely to accept other methods. These results are attributed to differential government efforts which have placed almost exclusive emphasis upon sterilization. It is argued that an expanded family planning programme would overcome the inertia of tradition concerning non-terminal contraception.

From 1966 to 1990 there was a marked rise in the use of voluntary sterilization in Flanders, followed by a fall in women under the age of 40. In the last three decades a remarkable change has occurred in the choice between male and female sterilization. Compared with many other European countries, sterilization of men and women is widely practised in Flanders. In 1996 40% of 40- to 44-year-old women underwent voluntarily sterilization or had voluntarily sterilized partners. Additionally, another 9% of these women were sterilized for medical reasons. Voluntarily sterilized couples are on average older than non-sterilized couples, and, obviously, consider their families to be complete. For couples with a complete family, parity is the most important predictor in the choice between sterilization and non-sterilization. Regularly practising Catholics undergo sterilization least. Also, highly educated couples are less likely to have a sterilization. Couples who ever experienced a contraceptive failure choose sterilization more. Voluntary sterilization has no substantial effect on the fertility of the population since the effect on the prevented numbers of both wanted and unwanted births appears to be small. However, if voluntary sterilization did not occur, differences in fertility within the population would probably increase. It is presumed that the popularity of voluntary sterilization in Flanders has passed its peak and that its use will decline in the near future.

With the increasing interest in population dynamics, research workers with differing backgrounds, such as demographers, vital statisticians and members of the medical profession, find themselves on common ground. However, this interdisciplinary contact reveals that we sometimes use technical words in different ways, depending on the terminology of our profession. When a doctor speaks about fertility, it is synonymous with the Scandinavian word frugtbarhed whereas fertilitet for the demographer only refers to live-born. Medical research within the field of fertility usually deals with conceptions, i.e. live-births, still-births, legal abortions and other hospitalized abortions, including illegal and spontaneous abortions. There are also an unknown number of conceptions, such as non-hospitalized abortions, but it is now generally believed that, in Denmark, this figure is negligible (Hoffmeyer & Norgaard, 1964; Sele & Norgaard, 1972). Moreover, during the last three decades, contraception has become more and more widespread and this new variable, which influences both births and abortions, should also be considered. Bearing this in mind it would be appropriate to refine our terminology and use ‘manifest fertility’ for conceptions and ‘potential fertility’ for all biologically possible pregnancies.

Women in marital unions had more live births than those in consensual unions. The relationship between cumulative fertility and the number of fertile sexual unions is positive for the early childbearing years and negative for the later ones. There is no consistent pattern of relationship between fertility and the sociocultural independent variables for different subgroups according to nuptiality pattern. The relationship between fertility and nuptiality in the Dominican Republic is consistent with that for the Caribbean region.

The maxim of proponents of pseudoscience is to spread ignorance through false perceptions of its scientific status. One of its most attractive — and simultaneously harmful — manifestations is complementary and alternative medicine (CAM). Despite the scientific evidence against them, CAM has taken hold in today’s society as a therapeutic model for a growing segment of the population. We analysed 379 articles on homeopathy, acupuncture, reiki and Bach flower remedies published in mainstream Spanish newspapers (El País, El Mundo, La Vanguardia, El Periódico and ABC) for the period 2011-2016, finding that disinformation is participated in actively by the Spanish press. CAM content was detected in these newspapers, together with a lack of an editorial perspective. In most of the cases, the uncritical articles were found in the interpretive genre and the society section. We also characterized the pseudoscientific discourse aimed at the public, finding that it is irrational and fraudulent in sowing fear and distrust regarding science. On the basis of theories invalidated by the scientific method and on appeals to the emotions, pseudoscience not only threatens scientific knowledge, but directly undermines public health by encouraging the abandonment of conventional medicine. In order to remedy this situation, better scientific training, informative screening and editorial commitment is urgently needed in the Spanish press.

Blood pressure (BP) control is a key intervention to decrease cardiovascular diseases (CVD), the main cause of death in low and middle-income countries (MIC). Scarce data on the determinants of BP control in Latin America are available. Our objective is to explore the role of gender, age, education, and income as social determinants of BP control in Argentina, a MIC with a universal health care system. We evaluated 1184 persons in two hospitals. Blood pressure was measured using automatic oscillometric devices. We selected those patients treated for hypertension. The average BP of less than 140/90 mmHg was considered a controlled BP. We found 638 hypertensive individuals, of whom 477 (75%) were receiving antihypertensive drugs, and of those, 248 (52%) had controlled BP. The prevalence of low education was more frequent in uncontrolled patients (25.3% vs. 16.1%; P < .01). We did not find association between household income, gender, and BP control. Older patients had less BP control (44% of those older than 75 years vs. 60.9% of those younger than 40; test for trend P < .05). Multivariate regression indicates low education (OR 1.71 95% CI [1.05, 2.79]; P = .03) and older age (OR 1.01; 95% IC [1.00, 1.03]) as independent predictors of the lack of BP control. We conclude that rates of BP control are low in Argentina. In a MIC with a universal health care system low education and old age but not household income are independent predictors of the lack of BP control.

International comparisons of the effectiveness of coronavirus disease 2019 (COVID-19) non-pharmaceutical interventions (NPIs) based on national case and mortality data are fraught with underestimated complexity. This article calls for stronger attention to just how extensive is the multifactorial nature of national case and mortality data, and argues that, unless a globally consistent benchmark of measurement can be devised, such comparisons are facile, if not misleading. This can lead to policy decisions and public support for the adoption of potentially harmful NPIs that are ineffective in combating the COVID-19 pandemic and damaging to mental health, social cohesion, human rights and economic development. The unscientific use of international comparisons of case and mortality data in public discourse, media reporting and policymaking on NPI effectiveness should be subject to greater scrutiny.

In the Kingdom of Tonga, migration to overseas developed countries has prevailed. To elucidate the effects of migration on population dynamics, an interview survey was conducted in the migrant-sending community of Kolovai, in the outer region of Tongatapu Island. All births, deaths, marriages and in- and out-migrations that took place between 1983 and 2002 were recorded for all members of the ‘Kolovai population’, consisting of persons who had lived in this community for at least a one-year period during this 20 years. The ‘Kolovai population’ members, numbering 1184 (564 males and 620 females), were divided into three groups based on residence at the end of each year, i.e. Kolovai (called KK), other places in Tonga (KT) or overseas countries (KO). The KK population decreased from 774 in 1982 to 570 in 2002, owing mostly to an increase of 167 persons as the natural balance and a decrease of 324 persons as the balance of international migration. Comparison of total fertility rate (TFR) between KK and KO women revealed that the mean TFR of the former decreased from 3·460 in the earlier 10-year period (1983–1992) to 2·240 in the later 10-year period (1993–2002), while that of the latter was more than 3·5 in both 10-year periods. This difference was largely due to the decrease in the proportion married among KK women. If the current trends of international migration and fertility continue, the population of Kolovai will be reduced and its age composition will become cylinder-shaped in the near future.

Data from a sample interview survey conducted in Accra in 1982 show that there is a very high level of knowledge of contraceptive methods among mothers aged 25–39 years (92%). However, only about 41% of the women had ever used any form of contraception, and only 18% were using modern methods at the time of the study. This is low for a city where an official family planning programme has been in operation for 15 years, even when the confounding effects of availability and accessibility of services have been controlled for. Suggestions are made for increasing the effectiveness of such programmes.
