Exploring the ways in which sexual fantasies may affect sexual experience and satisfaction is of relavence in the clinical setting involving sexual dysfuntion.

To observe how the sexual fantasy scores differ in their relationship with sexual satisfaction, experience and quality between sexual dysfunction cases and normal controls.

Scales included: Wilson’s sex fantasy questionnaire (WSFQ), Arizona Sexual Experience Scale (ASEX), Sexual Quality of life Questionnaire (SQoL), and a subjective sexual satisfaction meter. Differences in resposes of both groups on WSFQ (item-wise and domain-wise) were analysed using T-tests. Two-way ANOVA was applied to see how other scales affected sexual fantasy.

Cases scored significantly higher on ASEX scale, and low on satisfaction, SQoL and WSFQ

Majority of WSFQ responses, both in cases and controls, fell in the intimate and impersonal domains. Sexual fantasy scores and sexual satisfaction had a strong positive and significant correlation in controls but no linear correlation in the case-subjects. sexual fantasy scores contributed to 5.7% of difference in the scores of SQoL between groups. Major variance in scores of satisfaction in our subjects depended on presence or absence of sexual dysfunction(46.5%)but sexual fantasies also contributed to 8.8% of the variance.

The study showed that fantasies contribute to positive sexual outcomes only in absence of sexual dysfunction. ANOVAanalysis revealed that in case-subjects sexual satisfaction briefly increases initially with increase in fantasy scores but starts to decline as fantasies increase.

No significant relationships.

Psychological problems like anxiety in patients who are candidate for coronary artery bypass graft (CABGs) may increase mortality and morbidity. The effect of reassurance and preoperative information in decreasing anxiety is uncertain.

This study was designed to address the effect of preoperative information and reassurance in decreasing anxiety patients who are candidate for CABGs.

With experimental study and random sampling 85 (55male and 30 female) of 238 patients who were candidate for CABGs were included and completed spilberger anxiety test on the day of admission. Preoperative information and reassurance was done and test was completed again a day before the surgery. At last the Statistical analysis was done.

The Mean score of anxiety in patients with mild anxiety disorder (21 patients) was 34± 4.2 before and 39±5.8 after intervention. In patients with moderate anxiety disorder (39 patients) the score decreased from 52.61±3.8 to 50.76±56. Twenty five patients had severe anxiety disorder formerly with score of 63.88±2.8 which decreased to 53.88±7.6. All of differences were significant statistically.

In patients with mild anxiety intervention of preoperative information and reassurance resulted in increasing of anxiety but in patients with moderate or severe anxiety disorder such intervention decreased the level of anxiety significantly.

The participant will get an insight in the challenges of the implementation of new systems in routine clinical practice. He will get an opportunity to learn about the positive impact of operations management in health care. He also will get an idea about the attitude of patients towards new systems.

Families of individuals with schizophrenia experience disempowerment and helplessness because of unique stressors in their everyday life, mainly derived from the positive and negative symptoms of the illness (Bauml, et al., 2006). Another line of research provides evidence that working with families of people with schizophrenia is effective in reducing family burden and distress levels, through structured psychoeducational interventions (Falloon, 2003; Bauml, et al., 2006).

Despite evidence of their efficacy, psychoeducational interventions have not been widely implemented in standard clinical practice. The goal of this ongoing research project is to investigate the efficacy of a group psychoeducational intervention for parents of individuals with schizophrenia in reducing family burden and in improving their communication and problem solving skills, as well as their general family functioning.

The theoretical foundations of family psychoeducation is mainly derived from behavioral family therapy. Four couples (all parents of one or two individuals with schizophrenia) participated in this psychoeducational intervention program. Twenty three group sessions were conducted, organized around three primary goals: a) provision of information about the nature and the treatment of schizophrenia b) management of social and self-stigmatization and c) teaching family members problem-solving and communication skills (Solomon, 2000).

Pre and post treatment measures, as well as direct measures during treatment, were used to quantitatively assess family burden, family rituals, knowledge about the nature of the disorder, coping with stigma, communication and problem solving skills and general family functioning. Findings will be discussed.

The people who seem to have less social support And suffer lack of multiple social supports are more likely at risk to drug abuse.

This research was an experimental with pre-test post-test design with randomly assigned 30 patients referred to drug addiction treatment centers in experimental and control groups. present aim at testing the effect of eight, 2- hour sessions of stress management training based on cognitive behavioral techniques. Subjects were tested at the beginning and at the end of the project with (SSQ).

Study findings showed that the subjects of experimental group that were under the intervention(training)compared with control group showed significantly better scores and test effectiveness.

Interventions based on cognitive – behavioral approaches, is effective treatment that it can alters behavioral and cognitive factors associated with substance abuse and be as an efficient model for the prevention of drug abuse and also after treatment.

Delirium is a common neuropsychiatric syndrome with considerable heterogeneity that includes a variety of clinical (motor) subtypes. Because delirium is typically highly fluctuating, understanding the longitudinal stability of subtypes is crucial to evaluating their relevance to treatment and outcome.

to examine the changes (variability) in motor subtype profile in patients with delirium over serial assessment using the Delirium Motor Subtype Scale, and to investigate predictors of variability.

We studied motor subtype profile of patients with delirium assessed daily over a week in elderly patients undergoing hip fracture surgery. A Generalized Estimating Equations Model examined possible predictors of change in motor subtype status, including baseline variables and delirium course.

We included 118 patients developing DSM-IV delirium after hip-surgery [mean age 87.0±6.5 years; range 65–102; 66% females]. At first assessment, hyperactive subtype was most common (49%), followed by hypoactive (31%) and mixed subtype (14%), with 6% of delirious patients not fulfilling criteria of any DMSS-defined motor subtype. Almost two-thirds (n=69) of these patients underwent at least one more assessment, and for these 45 (57%) remained stable in motor subtype over time, while the rest 34 (43%) underwent change. A range of baseline characteristics were not significant predictors of variability in subtype profile.

Motor subtype profile is typically stable for orthopaedic patients with delirium. Thus evidence from cross-sectional studies of motor subtypes can be applied to many patients with delirium. Further longitudinal studies can clarify the stability of motor subtypes across different clinical populations.

Othello syndrome is a psychotic disorder characterized by delusion of infidelity or jealousy. It predominantly occurs in the context of specific psychiatric or neurological disorders. Othello syndrome is associated with mental changes including excessive aggression, hostility, and irritability. Patients with Othello syndrome misinterpret the behaviour of the spouse or sexual partner to provide evidence for their false perception.

The purpose of this paper is to examine the phenomenon of Othello syndrome as a result of specific neurological diseases.

The study design was a retrospective case series of patients with Othello syndrome. We searched the electronic databases PubMed and Embase for review articles and original research using the search terms ‘Othello syndrome, Morbid Jealousy, Pathological Jealousy, Delusional Jealousy, Delusions and Infidelity, Delusions of Jealousy or Infidelity’.

In the present study of 95 case reports, the relationship between Othello syndrome and a neurological pathology was described. This syndrome was most commonly associated with neurodegenerative diseases (59%), followed by medication induced Othello syndrome (13.7%) and vascular dementia (8.4%). Lesions particularly in the right (dorsolateral) frontal lobes were associated with this syndrome.

This study demonstrates that Othello syndrome occurs most frequently in patients with right frontal lobe dysfunction. It is predominantly related with Lewy Body Disease and Alzheimer's disease. Clinicians should keep an “index of suspicion” regarding dementia when Othello syndrome presents in elderly persons.

Swedish accommodation to EU-policy has weakened state control over the alcohol supply as well as increased the inflow of drugs. Limiting alcohol and drug consumption must thus occur through reducing demand. As the Internet is virtually a universal phenomenon, influencing alcohol and drug behaviors via an Internet-based service is therefore a natural next step. A new Swedish website for self-screening via the AUDIT- and DUDIT-questionnaires is eScreen (www.escreen.se).

Self-screening via eScreen generates user feedback and treatment recommendations. In-depth self-report is possible using the Alcohol-E and DUDIT-E. Site users can follow their progress over time in diagrams showing personal risk level as well as by writing a personal electronic diary.

During the first 18 months, 2492 user-accounts were registered, evenly distributed between genders. With regard to selection bias, approximately 65% of adult site users over 18 report harmful alcohol consumption, compared to 63% of female and 51% of male adolescent users. Screening suggests that 11% of female and 16% of male adult users are dependent on illicit drugs; for adolescents the rates are virtually identical. These results will be compared to rates from an ongoing general Swedish population study.

Internet-screening for alcohol and drug problems appeals to people with harmful alcohol and drug use. Low cost combined with the potential to reach many people could make Internet-based self-help services a cost-efficient self-help tool as well as a complement for patients in treatment for substance abuse.

ADHD in adults is associated with a significant impairment in many life activities increasing the risk of chronic stress in everyday life. Previous studies reported normal cortisol awakening response (CAR) in children with ADHD without comorbidities, nevertheless there is a lack of studies in adults.

The aim of the present research is to examine CAR in adults with ADHD and to assess possible differences between the combine and inattentive subtypes.

Patients were recruited from the Program for adults with ADHD in the Department of Psychiatry of the Hospital Universitari Vall d’Hebron. The clinical sample consisted of 50 adults, age between 18 and 51 years (mean 35.24 ± 9.21) fulfilling current diagnostic criteria for ADHD (DSM-IV criteria). All patients were naïve to stimulant medication. Psychiatric and organic comorbid disorders were excluded. To assess CAR, four salivary cortisol samples were collected at 0, 30, 45 and 60 minutes after awakening.

The mean increase in CAR for the whole group of patients was 10.34±8.79 nmols/l. T-test comparisons showed no significant differences in the mean increase of CAR between the inattentive (mean: 9.47±9.04 nmols/l) and combine (mean: 11.25±8.67 nmols/l) subtypes (t=0.610; z=0.546).

Despite there were no significant differences in salivary CAR between ADHD subtypes in adults, the mean increase of CAR was higher in combine than in the inattentive subtype. Salivary CAR needs to be further explored as an index of vulnerability to stress in these patients.

Recently, Baltic Sea Diet Score (BSDS) was developed based on the Baltic Sea Dietary Pyramid. The association between Baltic Sea Diet Score and prevalence of depressive symptoms has not been studied earlier, though diet including fish and whole grain products may be preventive for depression.

We assessed whether adherence to Baltic Sea Diet is associated with a decreased prevalence of depressive symptoms.

The population-based Kuopio Ischaemic Heart Disease Risk Factor (KIHD) Study included 2682 Finnish men aged 42-60 years. Diet was assessed by a 4-d food record and depressive symptoms with the 18-item Human Population Laboratory Depression Scale. Those who scored five or more were considered to have elevated depressive symptoms. The BSDS was calculated according to the quartiles of consumption of each score component. For healthy considered foods (fruits and berries, vegetables, wholegrain products, ratio of polyunsaturated fatty acids to saturated fatty acids and fish) the quartiles were coded 0-3 points (lowest quartile was coded as 0, etc.). For meat and total fat scores were opposite. Alcohol was coded 1 point (< 20 g), otherwise 0 point.

Altogether 228 (8.5%) men had depressive symptoms. In a multivariable model, after adjustments for age, energy intake, smoking, marital status, socio-economic status, maximal oxygen uptake and body mass index, men in the highest v. lowest BSDS quartile were 40% more likely to be non-depressed (OR 0.60, 95% CI 0.39, 0.93, p=0.02).

Adherence to Baltic Sea diet may be associated with lowered level of depression.

The prevalence rate of dementia in older adults with intellectual disability (ID) is comparable with that of the general population and is significantly higher in people with Down Syndrome (DS). It is recommended that diagnosis should be made following history, physical and mental state examination, cognitive assessment, blood tests and, ideally, neuroimaging. However, experience suggests that dementia may be underdiagnosed in this patient group thereby restricting access to appropriate management.

We aimed to elucidate the barriers to a timely diagnosis of dementia in the ID population attending a specialised ID memory clinic in London, UK.

We conducted a service review of the Clinic, retrospectively reviewing the records of all patients seen in 2011 to assess attendance and diagnosis rate, co-morbid physical and mental illness and the completion of neuroimaging and blood tests.

32 patients were seen at least once during 2011 in the memory clinic, of whom 11 (34%) received a diagnosis of dementia. 22 (69%) of those seen had DS and 18 (56%) were male. 12 (38%) patients have thyroid dysfunction and 8 (25%) have epilepsy. 23 patients had completed a DLD in the past year and 14 (44%) had completed more than one DLD. 12 (38%) patients had a previous brain scan and 27 (84%) patients had had blood tests in the previous year.

Comorbid physical illnesses, consistent attendance at appointments, access to psychological assessment and willingness to undergo neuroimaging and phlebotomy are significant barriers to diagnosis of dementia in patients with ID.
