Have changing demographics, increased life expectancy and findings about gender similarities and differences, altered portrayals of older people in American feature films during the past 65 years? We identified 3,038 films made between 1929 and 1995 in which actors and actresses, nominated at least once during their lifetimes for an Oscar award, appeared when aged 60 years or older. Academy Award nominees were selected because they offered a sample of ‘notable’ performers and an accessible database. We selected an eight per cent random sample for a content analysis of their roles. Throughout this period, men were more likely to be depicted as vigorous, employed and involved in same-gender friendships and adventure (whether as hero or villain). Women remained either peripheral to the action or were portrayed as rich dowagers, wives/mothers, or lonely spinsters. Despite changing gender roles in later life since the 1930s and despite social and economic changes for older Americans (earlier retirement age and better health are but two examples), their film roles have remained remarkably static in age and gender stereotyping. In feature films, the mask of ageing differs by gender. Male masks veil inactivity and physical changes, while female masks reveal ageist and sexist stereotypes.

As populations age, increased focus is given to the importance of enabling older people to age in place. The study reported in this paper explored the extent to which older people considered their homes and neighbourhoods to be ‘supportive’, and sought to increase understanding of the needs and experiences of older people and their expectations of future housing needs. This paper reports qualitative data from the Housing and Independent Living (HAIL) study carried out in Australia. Semi-structured interviews were conducted with 202 community-dwelling people aged 75–79 years. Interviews were transcribed, coded and analysed using computer-assisted qualitative analysis and a narrative approach to identify broad themes. Thematic analysis was used to examine and understand how occupants subjectively viewed their homes, and how they planned to adapt/modify either their activities or homes to accommodate changing needs. Six key themes emerged, namely housing choice, attachment to place, financial issues, changes to the home over time, transport, and anticipating the future. In this study, people who most strongly identified with and felt connected to their neighbours/communities had more positive perceptions of their homes and communities, and may be better able to remain in their home despite increasing disability or frailty. Housing policies and home and urban design should ensure home and neighbourhood environments are safe, accessible, promote positive associations, and are adaptable to facilitate independence and accommodate change as people age.

This article presents an analysis of the factors that were associated with preferences for ‘early retirement’ or ‘late exit’ from paid work in Sweden. It draws upon special questions that were added to the country's Labour Force Survey of 2001 and were put to a sample of more than 1,000 people aged 55–64 years who were (self-) employed. Separate models of the factors influencing women's and men's preferred retirement age were calibrated, using variables that described current working conditions, whether the respondents perceived themselves as being appreciated as employees, and their attitudes to work and to private or home life. Among the findings, a positive attitude towards work motivated both women and men to stay in work beyond the normal retirement age, while positive attitudes towards private life promoted an early exit. Poor working conditions generated negative attitudes towards work (and vice versa). The strengths of various push and pull factors were gender-specific: for women, a trying job tended to push them out of the labour market; for men, a socially-rewarding job tended to keep them in the labour force. The more that men worked mainly for pecuniary reasons, the stronger their wish for an early exit. Men who felt that they were unappreciated at work preferred early retirement: in some of these cases, the ‘push’ factor was related to ageism. As people approach retirement age, many appear to reconceptualise more positively their life outside the work place.

This paper describes where elderly people with moderate to severe (‘advanced’) cognitive impairment are living, what resources they use and the costs of these services to different funding agencies. It operationally defines dementia in terms of cognitive impairment and uses the OPCS disability surveys to estimate the prevalence of moderate to severe cognitive impairment in England. The paper goes on to describe services received by this client group on the basis of the same surveys, and costs these by supplementing the data with information from related studies by the PSSRU. Since accommodation is a major cost, the population of elderly people with cognitive impairment is classified according to their place of residence. Thus we are able to estimate the ‘baseline’ global cost of provision for this client group at current prices. We make cost projections at 1991/92 price levels to the national level, and discuss the implications in terms of burdens to different agencies.

The elderly population in Sri Lanka is growing rapidly. Elders are traditionally cared for in the homes of their adult children, but the shifting socio-economic environment in Sri Lanka challenges this arrangement. This paper describes the dynamics of elder–care-giver relationships in Southern Sri Lanka. Data included four focus group discussions and five in-depth interviews with elderly, and ten in-depth interviews with adult children of the elderly. Discussion guide topics included care-giving arrangements, and roles/responsibilities of elders and care-givers. Using a grounded theory approach, a comprehensive analytic memo was developed and discussed to explore emerging themes on the care-giver dynamic. Both elders and care-givers felt that elders should be taken care of in the home by their children. They pointed to a sense of duty and role modelling of parental care-giving that is passed down through generations. Even as elders desired support from their children, they feared losing their independence, and saw financial autonomy as important for maintaining relationship balance. Care-giving challenges included: households where both the adult child and his/her spouse worked outside the home; households where elders had a disproportionate amount of household work; economically stressed households; and lack of direct communication between elders and care-givers regarding conflicts. Results point to strong values around caring for elderly in the home, but identify challenges to this arrangement in the future.

Earlier research on residential mobility has demonstrated a tendency for the young old of the 55+ population to prefer peripheral locations, whereas older age groups choose central locations. Here, we present survey results indicating that such late-adulthood differences in preferences are supported by age-related shifts corresponding to differences in housing preferences expressed by individuals in peripheral as well as central locations in Sweden. A sample of 2,400 individuals aged 55 years and over was asked to select the seven most important characteristics of a dwelling from a list of 21 alternatives (Survey of Housing Intentions among the ELDerly in Sweden (SHIELD), 2013). The preferences expressed were used as dependent variables in logistic regressions to determine to what extent the housing preferences of older people are linked to age, gender, socio-economic status and type of geographical area. The results demonstrated a close link between neighbourhood characteristics and housing preferences. Owning the dwelling, having a garden and access to nature were stressed as important by individuals living in non-metropolitan middle-class areas and in suburban elite areas. The youngest cohort expressed similar preferences. Older age groups instead stressed the importance of an elevator, single-storey housing and a good design for independent living; preferences that have similarities to those expressed by individuals living in large cities and smaller urban centres where such housing is more readily available.

The age-friendly cities and communities movement has focused on how to better support older adults to age well within urban environments. Central to ‘ageing well’ and ‘active ageing’ agendas is ensuring that older adults can participate in meaningful forms of social participation. The benefits of social participation in old age have been well documented, and research amongst community-dwelling older adults has explored some of the neighbourhood qualities that facilitate or impede such forms of engagement. However, understandings of how older adults construct and negotiate social participation within everyday urban environments have been largely unexplored. To address this gap, we present results from 104 interviews conducted with older adults living in three cities and nine neighbourhoods in the United Kingdom (UK). The findings explore three themes generated from the research: ‘constructing meaningful social participation in old age’, ‘negotiating access to social participation’ and ‘navigating home and community’. Across these themes, the paper describes how experiences of social participation in old age involve a number of inter-connected physical, psychological and social processes experienced by individuals across a range of environmental settings including the home, outdoor spaces and community facilities. The paper concludes by discussing the implications of the findings for practice, specifically in the delivery of age-friendly communities.

It has been claimed, but not empirically supported, that early retirement leads to longer life. The present investigation addressed this question using data from a 1997 Israeli national household survey of adults aged 60 or more years linked to mortality records from the national death registry, for 2004. The study examined the association between early retirement and seven-year all-cause mortality among the population of older Jewish Israelis who were employed prior to or at baseline (N=2,374). Both the timing of retirement and the reasons for exit from the labour force were considered in the analysis. The initial hazard regression models, adjusted by gender and reason for retirement including poor health, showed that early retirees indeed had lower mortality risk ratios than respondents who had retired ‘on time’. When additional variables were controlled in the final analytic model, however, the association between early retirement and mortality was not supported. Older age, male gender, and having been diagnosed with one or more of five major illnesses were all associated with greater risk for mortality. Medium level education and being employed at baseline were associated with lesser mortality risk. Nevertheless, the timing of retirement, viz. early versus normative exit from the workforce, was not related to survival. In sum, the respondents who had prematurely exited the labour force did not benefit from disproportionately longer lives when compared with the respondents who retired ‘on time’.

Quality of life and wellbeing in nursing homes are becoming more important in research and practice. One of the main influences on residents' wellbeing is the interaction with their professional care-givers. The purpose of this study was to explore to what extent care-givers support the residents' needs of relatedness, autonomy and competence, and how this need support is related to wellbeing. Residents and their professional care-givers of four nursing homes in the Netherlands participated in an observational and questionnaire study. Three video-observations of each resident (with different care-givers) were made during morning care. Additional data were collected by means of questionnaires. The results show that the needs of residents were, on average, moderately fulfilled during care interactions. More need support by care-givers was related to higher resident wellbeing. Care-givers provided more need support to residents with stronger functional impairments. More need support was provided by higher-educated care-givers and care-givers in higher job functions. The results show the importance of need support for situational wellbeing, but the contribution to the general subjective wellbeing of residents remains unclear. Further (longitudinal) research is needed to investigate changes in wellbeing over time. Possible differences between subjective ratings and observations of need support and wellbeing should be taken into account.

This article reports on findings from a qualitative study, undertaken in England, which explored the experiences of 21 gay men and lesbian women who care, or cared, for a person with dementia. The aim of the study was to explore this experience through the lens of a person's gay or lesbian sexuality. The paper reports two related themes that emerged from the wider study – respondents' hopes, fears and plans for the future and, specifically, the way in which their caring experiences had coloured their views and expectations of how their own health and social care needs may be met. Respondents' narratives reflect a range of pervasive anxieties about the future. First, about the possibility that they might be diagnosed with a condition such as dementia and, consequently, the myriad ways in which their sexualities and lifestyle choices may be perceived and interpreted as they themselves age and, possibly, require health and/or social care and support. In light of these concerns, respondents reflected upon the need for specialist service provision for older gay and lesbian people – an idea that was, perhaps surprisingly, not universally welcomed.

With increasing life expectancy, changes in family structure and, most recently, the relaxation of the hitherto strict family planning policies, understanding how mid-life individuals support multiple generations, particularly their older parents and younger grandchildren, is of increasing research and policy significance in China. This paper analyses data from the 2011 China Health and Retirement Longitudinal Study (CHARLS) to examine the characteristics of Chinese mid-life individuals aged 45–64 who are potentially being ‘sandwiched’ between providing care to older parents/parents-in-law and/or younger grandchildren (under age 16). Binary logistic and multinomial regression models shed light on the factors associated with providing support to one generation or multiple generations. The results highlight that amongst the Chinese mid-life sandwich generation, 58 per cent only provide care to their young grandchildren, 23 per cent only provide care to their parents/parents-in-law, whilst 15 per cent are simultaneously supporting both generations. Rather than acting as competing demands upon the mid-lifers’ time, the multivariate analysis provides evidence that the provision of intergenerational care is complementary, with caring for grandchildren increasing the probability of also supporting one's parents/parents-in-law, and vice versa. However, an increase in the number of younger grandchildren has a negative impact on the care provided to older parents/parents-in-law, indicating that at higher care intensities there may be competing demands across the generations.

In this study, we identify the dominant storylines that were embedded in the narratives of younger people with dementia and their nominated family members. By implementing a longitudinal, narrative design underpinned by biographical methods we generated detailed family biographies with five families during repeated and planned research contacts (N=126) over a 12–15-month period between 2009 and 2010. The application of narrative analysis within and between each family biography resulted in the emergence of five family storyline types that were identified as: agreeing; colluding; conflicting; fabricating; and protecting. Whilst families were likely to use each of these storylines at different points and at different times in their exposure to young onset dementia, it was found that families that adopted a predominantly ‘agreeing’ storyline were more likely to find ways of positively overcoming challenges in their everyday lives. In contrast, families who adopted predominantly ‘conflicting’ and ‘colluding’ storylines were more likely to require help to understand family positions and promote change. The findings suggest that the identification of the most dominant and frequently occurring storylines used by families may help to further understand family experience in young onset dementia and assist in planning supportive services.

This paper presents a discourse analysis of talk in reminiscence groups. Two main issues are addressed. First, we examine how speakers' identities are accomplished through the way they position themselves in social relationships and social practices of ‘remembered pasts’. Particular analytical attention is given to how people claim entitlements to the significance and consequences of their lived experience. Second, issues of membership are examined through the way people index their engagement in the narrative environment accomplished in reminiscence group talk. Finally, we are concerned with how these narratives contribute to a ‘reconstitution’ of understandings in common about cultural and moral orders of remembered pasts and the historical era in which the reported events, experiences and practices took place. Our analysis aims to demonstrate how reminiscence work affords a context for ‘re-membering’ where older people on their own behalf can work entitlements to voice the consequences of their experiences of life.

This paper presents a structured literature review that focused on comprehensive case management by nurses for adults with long-term conditions living in the community. The emphases of the review are the implementation of case-management approaches, including its roles, core tasks and components, and the coverage and quality of the reported implementation data. Twenty-nine studies were included: the majority were concerned with case management for frail older people, and others focused on people with multiple chronic diseases, high-cost patients, or those at high risk of hospital admissions. All the studies reported that case managers undertook the core tasks of assessment, care planning and the implementation of the care plan, but there was more variation in who carried out case finding, monitoring, review and case closure. Few studies provided adequate implementation information. On the basis of the reviewed evidence, three issues were identified as key to the coherent and sustainable implementation of case management for people with long-term conditions: fidelity to the core elements of case management; size of caseload; and case-management practice, incorporating matters relating to the continuity of care, the intensity and breadth of involvement, and control over resources. It is recommended that future evaluations of case-management interventions include a comprehensive process component or, at the very least, that interventions should be more fully described.

This paper reports a qualitative study that used narrative analysis to explore how social support helps many armed-services veterans cope with traumatic memories. The analysis was carried out on two levels, that of narrative form (level of narrative coherence), argued to be indicative of reconciliation, and narrative content (themes of social support), which allowed exploration of the types of social support experienced by veterans with coherent, reconciled and incoherent narratives. Ten British male Second World War veterans were interviewed regarding their war experiences, presence of traumatic memories, and experiences of social support from comrades, family and society. Different patterns of support were qualitatively related to coherent, reconciled and incoherent narratives. Veterans with coherent narratives were no less likely to have experienced traumatic events than those with reconciled or incoherent narratives, but they reported more positive perceptions of their war experience and of the war's outcomes, more positive experiences of communication with family in later life, and more positive perceptions of societal opinion. The results are discussed in relation to how veterans can be supported by family and friends to reconcile their traumatic memories, thus to lessen the burden in later life when vital support resources may be unavailable.

A desire to ‘return’ or ‘pay back’ past care has been identified as a potential motivator of support provided by adult children to their ageing parents. The purpose of this study is to examine whether, how and in what ways adult children interpret and apply the concept of delayed reciprocity in filial relationships. Twenty-eight men and women supporting one or both ageing parent(s) in a Western Canadian city participated in a qualitative study of filial responsibility. Data were analysed interpretively, using thematic coding, contextualised reflection and guiding questions. Findings suggest delayed reciprocity is limited as an interpretive framework for describing parent support. Overall, comments reflected qualification or rejection of ‘paying back’ in the sense of a filial contract. Delayed reciprocity appears for most participants to symbolise imbalance, expectedness or obligation, and a lack of affection. In response, participants tended to reject delayed reciprocity in favour of interpretations emphasising mutuality, family role duties and reciprocated love. Findings are discussed in relation to interpretive purposes, symbolic meanings of parent support, participant characteristics and cultural contexts.

Various studies have dealt more or less directly with qualitative questions concerning social services provided in the home. While as a result of such studies the general characteristics of home services of importance to elderly people are quite well known, this is not the case regarding the relative importance assigned to such characteristics. Personal interviews using the ‘Multiattribute Utility Technology’ (MAUT) were used. This procedure organises the attributes in a ‘tree’ structure for comparing the relative importance & ranking of different quality attributes. A random sample of 180 receivers of home-help services aged 65 to 95 and living in a rural, small town and city area were interviewed. The interviewed persons were asked to rank-order according to personal importance each grouping of quality attributes. The relative values of the different qualities were determined, using a scale with a range from 0 to 100 points. Continuity, especially Staff Continuity, was the most important quality attribute together with Suitability and Personal Relation. Availability and Influence were the least important quality attributes. The characteristics studied had largely the same relative importance for all older people, irrespective of category.

This paper reviews the past development of the publicly funded long-term care (LTC) system and aims to advance further discussion of LTC in Taiwan. The Ten-year Long-Term Care Plan 2.0 introduced in 2017 calls for a major reform of a publicly funded LTC system in Taiwan. The reform expands on the previous universal tax-based LTC system, allowing for more comprehensive and accessible subsidies on LTC services. This paper provides a brief overview of the political context of the reform and an introduction to the legal basis, financing and delivery mechanisms of the reform plan. To this end, as a preliminary evaluation, this paper identifies major institutional and socio-cultural tensions that could challenge the implementation of the plan. Institutional tensions include the dominant foreign worker caring model, which relies on approximately 220,000 foreign workers to provide LTC services, and the discontinuity between health and social care governance, which leads to a discontinuity between curative and LTC care. Socio-cultural tensions focus on conflicting values in the allocation of responsibility of care and in the understanding of disability between universal social citizenship in the modern state and traditional Confucian ethics. Policy implications of these tensions for the LTC system are then discussed.

For older adults, exercise that challenges the muscular system, commonly referred to as resistance training, has significant physical, psychometrical and functional benefits. While well recognised by the scientific community, the translation of these benefits into practice has received little attention. Particularly neglected is an understanding of the personal experiences, motivation towards and adherence to resistance training recommendations among older adults. This paper investigated the benefits older individuals attribute to resistance training and the motivational tactics they employed to undertake it. Data were drawn from three focus groups where participants (⩾65 years; presently, previously or wanting to become involved in a resistance training intervention) were encouraged to openly discuss resistance training, physical activity and exercise. Findings revealed that participants were aware of the benefits of training on general and functional health, and that these benefits were employed in the motivation to train. In addition, presently or previously trained individuals stress the importance of environment and programme structure as a training motivator. The benefits to mental and social health, effect on ageing and body image were also raised. However, participants discussed these in a broad context. While it could be said that public knowledge reflects current evidence, it is also clear that individuals are still unaware of a number of specific benefits.

Amid rapid population ageing and the high-speed progress of information and communications technology, use of smart electronics can improve older people's quality of life by helping them to stay connected, active and independent, which may, in turn, increase their overall happiness and sense of social belonging. Taking a gender perspective, this paper aims to understand variations in digital exclusion among older people. Using data from the 2016 China Longitudinal Ageing Social Survey, it finds that digital exclusion seems to vary by gender, with rural female older people being most excluded, possibly because they have the lowest capital endowment. The gender difference in the digital divide is even more salient with all other things being equal. Digital exclusion in the information era may further exacerbate gender inequality, particularly for those who are already most vulnerable. Since digital inclusion is somewhat inherent in the concept of intelligent old-age support, it should be considered an urgent necessity to help older people, especially rural women, become familiar with smart electronic products.
