The mandatory clinical radiotherapy department visit undertaken by potential applicants aims to provide understanding of the profession and therefore reduce attrition. Increasing pressure on clinical departments makes visits a logistical challenge. This additional step may also present as an unnecessary barrier to applicants. With no evidence relating to visits, this study aimed to explore the perceptions of both students and clinical educators concerning potential benefits and challenges.

A focus group interview method was used to gather in-depth qualitative data concerning the clinical department visit experiences from first-year undergraduate students and clinical educators.

Three themes emerged from the student focus groups: the perceived purpose of the clinical visit, the visit content and the outcomes and impact arising from the visit. Clinical educator data also followed these themes in addition to ‘logistical impact’ theme.

The clinical visit has value to applicants in affirming their decision to study radiotherapy. There is variation in expectation and content for these visits and they are logistically challenging. Nationally agreed guidelines for visit structure and content could improve visit efficiency and effectiveness. A national clinical visit form may reduce workload for educators and applicants.

There is abundant evidence of the comparative efficacy of single-fraction (SF) radiotherapy and multi-fraction (MF) radiotherapy when treating patients with bone metastases. Despite this, previous surveys have shown SF schedules to be underutilised.

To determine current patterns of practice in patients with bone metastasis and to investigate the factors that influence practice.

An electronic audit was performed amongst 46 physicians, within 7 hospital trusts in the UK. The audit comprised of four hypothetical cases in which consultants and registrars chose which dose and fractionation they would recommend and their reasons for this recommendation.

SF radiotherapy was the most common radiotherapy schedule in hypothetical cases 1, 3 and 4. SF radiotherapy was recommended by 65% of respondents in case 1, 47% in case 2, 89% in case 3 and 46% in case 4. For case 2, 50% proposed MF radiotherapy. For case 4, 22% of respondents recommended Stereotactic Body Radiotherapy (SABR). The following deciding factors were cited as influencing choice of an SF schedule: prognosis, published evidence, performance status and spinal cord compression.

The most common radiotherapy schedule selected was SF. However, there were inter-institution differences regarding the use of SF radiotherapy. Furthermore, the survey had shown that a third of respondents recommended an MF regime, despite evidence supporting the efficacy of an SF schedule.

Radiotherapy clinical trials are at the forefront of modern-day prostate cancer patient management. Patients are reviewed during treatment by clinical oncologists or competent on-treatment review radiographers to minimise treatment toxicities. Clinical Research Radiographers (CRRs) routinely monitor and gather research data from patients participating in clinical trials.

The aim of this article is to evaluate the effectiveness of the CRR undertaking the on-treatment review of clinical trial patients.

An experienced CRR within the Northern Ireland Cancer Trials Network was supervised by a clinical oncologist to undertake the role of the on-treatment review of patients receiving radiotherapy for prostate cancer. The CRR explored published literature and compiled this written evaluation as part of their advanced practice learning.

The supervising clinical oncologist verified, following the planned period of supervised practice and academic study, that the CRR was competent to fulfil the role. Evidence of the beneficial synergistic impact of co-joining the roles was experienced at first hand during the undertaking of supervised practice.

Co-joining the roles and responsibilities of the CRR and the on-treatment review radiographer enhanced the quality of care offered to the patients participating in clinical trials.

This study reported the justification and selection of acceptable γ criteria with respect to low (6 MV) and high (15 MV) photon beams for intensity-modulated radiation therapy quality assurance (IMRT QA) using the Gafchromic external beam therapy 3 (EBT3) film.

Five-field step-and-shoot IMRT was used to treat 16 brain IMRT patients using the dual-energy DHX-S linear accelerator (Varian Medical System, Palo Alto, CA, USA). Dose comparisons between computed values of the treatment planning system (TPS) and Gafchromic EBT3 film were evaluated based on γ analysis using the Film QA Pro software. The dose distribution was analysed with gamma area histograms (GAHs) generated using different γ criteria (3%/2 mm, 3%/3 mm and 5%/3 mm) for the 6 and 15 MV photon beams, to optimise the best distance-to-agreement (DTA) criteria with respect to the beam energy.

From the comparison between the dose distributions acquired from the TPS and EBT3 film, a DTA criterion of 3%/2 mm showed less dose differences (DDs) with passing rates up to 93% for the 6 MV photon beams, while for the 15 MV a relaxed DTA criterion of 5%/3 mm was consistent with the DD acceptability criteria with a 95% passing rate.

Our results suggested that high-energy photon beams required relaxed DTA criteria for the brain IMRT QA, while low-energy photon beams showed better results even with tight DTA criteria.

The purpose of this study was to investigate variations in surface dose, with and without the use of a Klarity® Mask (Orfit Industries America, Wijnegem, Belgium), using intensity-modulated radiotherapy (IMRT) and 3-D conventional radiotherapy (3D-CRT).

Thermoluminescent dosimeters (TLDs) together with a phantom were used to examine acute skin toxicity during nasopharyngeal cancer treatment. These plans were sequentially delivered to the perspex phantom. Dosimeters were placed in five fixed regions over the skin. A Klarity mask for immobilization was used for covering the head, neck, and shoulder. The phantom was irradiated with and without a Klarity Mask, using IMRT and 3D-CRT, respectively.

The Klarity mask increased the skin doses for IMRT and 3D-CRT approximately 18·6% and 8·6%, respectively, from the prescribed maximum skin dose using treatment planning system (TPS). Additionally, the average percentage dose between IMRT and 3D-CRT received on the surface region was 30·9%, 24·9% with and without Klarity mask respectively. The average percentage dose received on surfaces from the total therapeutic dose 70 Gy, without using the mask was 7·7% and 5·7%, for IMRT and 3D-CRT, respectively. The TPS overestimated the skin dose for IMRT planning by 20%, and for 3D-CRT by 16·6%, compared with TLD measurements.

The results of this study revealed that IMRT significantly increases acute skin toxicity, compared with CRT. Although it is recommended to use Klarity mask as a sparing tool of normal tissue, it increases the risk of skin toxicity. In conclusion, skin dose is an important issue of focus during radiotherapy.

The Electronic Portal Imaging Device (EPID), primarily used for patient setup during radiotherapy sessions is also used for dosimetric measurements. In the present study, the feasibility of EPID in both machine and patient-specific quality assurance (QA) are investigated. We have developed a comprehensive software tool for effective utilisation of EPID in our institutional QA protocol.

Portal Vision aS1000, amorphous silicon portal detector attached to Clinac iX—Linear Accelerator (LINAC) was used to measure daily profile and output constancy, various Multi-Leaf Collimator (MLC) checks and patient plan verification. Different QA plans were generated with the help of Eclipse Treatment Planning System (TPS) and MLC shaper software. The indigenously developed MATLAB programs were used for image analysis. Flatness, symmetry, output constancy, Field Width at Half Maximum (FWHM) and fluence comparison were studied from images obtained from TPS and EPID dosimetry.

The 3 years institutional data of profile constancy and patient-specific QA measured using EPID were found within the acceptable limits. The daily output of photon beam correlated with the output obtained through solid phantom measurements. The Pearson correlation coefficients are 0.941 (p = 0.0001), 0.888 (p = 0.0188) and 0.917 (p = 0.0007) for the years of 2014, 2015 and 2016, respectively. The accuracy of MLC for shaping complex treatment fields was studied in terms of FWHM at different portions of various fields, showed good agreement between TPS-generated and EPID-measured MLC positions. The comparison of selected patient plans in EPID with an independent 2D array detector system showed statistically significant correlation between these two systems. Percentage difference between TPS computed and EPID measured fluence maps calculated for number of patients using MATLAB code also exhibited the validity of those plans for treatment.

Radiation therapy (RT) remains a common and effective treatment modality for patients with locally advanced prostate cancer. Technological advancements over the past decade have resulted in the introduction of intensity-modulated radiation therapy (IMRT) planning and delivery techniques that maximise the dose of radiation delivered to the prostate while sparing organs at risk (OAR). A more recent and evolving IMRT technique, called volumetric-modulated arc therapy (VMAT), involves a continuous irradiation at a constant or variable dose rate when the gantry rotates around the prostate using one or more arcs.

This paper reports on a dosimetric evaluation of our implementation of VMAT technique for prostate cancer treatment. A retrospective analysis of VMAT plans was performed for 300 prostate cancer patients treated during the period of January 2013 to December 2014. Two prescription cohorts of patients treated to a dose of 78 Gy in 39 fractions as the primary radiation therapy treatment (XRT) and 66 Gy in 33 fractions as a post-op or salvage XRT were considered.

The mean and maximal doses, dose inhomogeneities and conformity indexes for the planning target volumes were evaluated for each prescription cohort of patients. Similarly, the doses to OAR such as rectum, bladder and femoral heads were also assessed for various dose levels.

This study shows that highly conformal radiation dose distribution for the treatment of prostate cancer is achievable with the VMAT technique. It provides evidence to support the adoption of such conformal technology in many disease sites such as the prostate. We believe that our experience reported here could help form the foundation for individual institutions to evaluate and develop the most suitable planning criteria tailored to their own needs and priority. This endeavour hopefully will provide further improvement in the planning process and, therefore, help achieve an effective and efficient delivery of radiotherapy for prostate cancer.

Late adverse effects following radiation therapy for prostate cancer involve the urinary and lower gastrointestinal tracts, with continuous rectal bleeding being the most serious issue. We focused on late adverse effects, particularly rectal bleeding after volumetric-modulated arc therapy (VMAT), for patients with locally advanced prostate cancer.

Seventy-three patients with localized prostate cancer were treated with radiation therapy using VMAT with an image-guided radiation therapy system. Patient age at the start of irradiation ranged from 54 to 81 years (median, 71 years). The follow-up period ranged from 23 to 87 months (median, 57 months). The prescribed total irradiation dose was 76 Gy in 38 fractions.

Late rectal bleeding was observed in 14 (19%) patients, with nine (12.3%), four (5.5%), and one (1.4%) being classified as grades 1, 2, and 3, respectively. One grade 3 patient with rectal bleeding had severe diabetes and was administered intravenous warfarin for cardiomyopathy.

VMAT may provide better accuracy and involve fewer time constraints for patients compared with other intensity-modulated radiation therapy (IMRT) methods. The incidence of late rectal bleeding in VMAT is almost equivalent to that of other IMRT methods.

The aim of this study is to evaluate the influence of flattened and flattening filter-free (FFF) beam 6 MV photon beam for liver stereotactic body radiation therapy by using volumetric modulated arc therapy (VMAT) technique in deep inspiration breath hold (DIBH) and free breathing condition.

Eight liver metastasis patients (one to three metastasis lesions) were simulated in breath hold and free breathing condition. VMAT-based treatment plans were created for a prescription dose of 50 Gy in 10 fractions, using a 230° coplaner arc and 60° non-coplanar arc for both DIBH and free breathing study set. Treatment plans were evaluated for planning target volume (PTV) dose coverage, conformity and hot spots. Parallel and serial organs at risk were compared for average and maximum dose, respectively. Dose spillages were evaluated for different isodose volumes from 5 to 80%.

Mean D98% (dose received by 98% target volume) for FFF in DIBH, flattened beam in DIBH, FFF in free breathing and flatten beam in free breathing dataset were 48·9, 47·81, 48·5 and 48·3 Gy, respectively. D98% was not statistically different between FFF and flatten beam (p = 0·34 and 0·69 for DIBH and free breathing condition). PTV V105% (volume receiving 105% dose) for the same set were 3·76, 0·25, 1·2 and 0·4%, respectively. Mean heterogeneity index for all study sets and beam models varies between 1·05 and 1·07. Paddik conformity index using unflattened and flattened beam in DIBH at 98% prescription dose were 0·91 and 0·79, respectively. Maximum variation of isodose volume was observed for I-5%, which was ranging between 2288·8 and 2427·2 cm3. Increase in isodose value shows a diminishing difference in isodose volumes between different techniques. DIBH yields a significant reduction in the chest wall dose compared with free breathing condition. Average monitor units for FFF beam in DIBH, flattened beam in DIBH, FFF beam in free breathing CT dataset and flattened beam in free breathing CT dataset were 1318·6 ± 265·1, 1940·3 ± 287·6, 1343·3 ± 238·1 and 2192·5 ± 252·6 MU.

DIBH and FFF is a good combination to reduce the treatment time and to achieve better tumour conformity. No other dosimetric gain was observed for FFF in either DIBH or free breathing condition.

Stereotactic body radiotherapy (SBRT) is widely used for the treatment of stage-I non-small cell lung cancer (NSCLC). Patient-specific motion correlated with 4DCT could be essential for hypofractionated SBRT. All patients undergoing SBRT do not require motion management during the dose delivery. The objective of this study was to evaluate which patient may benefit from Gated SBRT.

Treatment planning of 20 patients of stage-I NSCLC was analysed. Conventional and 4DCT scans were taken. Internal target volume as well as planning target volume (ITV and PTV) were determined in the CT data sets. PTVall phases created using 4DCT data sets and PTV15mm created using conventional CT data were compared. Also, ITVall phases were compared with ITV created from maximum intensity projections (ITVMIP). Suitability of patients for motion management-based treatment delivery was also evaluated.

The average ITVMIP to ITVall phases ratio is 1·06 indicating good agreement between them. Based on the ratio of intensity projections, 9 out of 17 patients were found suitable for our existing gated treatment.

4D CT is the main requirement in SBRT to identify the patients who can benefit from motion management during the dose delivery.

This exploration is intended to analyse the dosimetric characteristics of proton beams of multiple energies using different snout sizes.

A synchrotron was used for the extraction of eight proton beam energies (100–250 MeV). Dosimetric measurements were taken in a water phantom that was irradiated with a proton beam emanating from the gantry system at angles 0, 90, 180 and 270 degree using a large and a medium snout. The range of beam energies in the phantom, their corresponding centre modulation depth (CMD) and the width of spread out Bragg peak (SOBP) were measured by Markus chamber. Double scattering technique was employed for the creation of SOBPs.

The range of proton beams varied from 4·3 cm for 100 MeV beam to 28·5 cm for 250 MeV beam with the medium snout and from 4·3 cm for 100 MeV to 25 cm for 250 MeV beam with large snout in the water phantom. SOBP width showed a variation from 4 to 10 cm with medium and large snout. While determining the output with medium snout, the discrepancy of 1·1% was observed between the maximum and minimum mean values of output for all the given set of energies and angles. There occurred a difference of 0·9% between the maximum and minimum mean values of output with the large snout. Beam output at SOBP centre was 12% higher with large snout as compared to that with medium snout for all the given beam energies. Flatness and symmetry were found within ±2·5% tolerance limits with medium and large snouts.

Flatness and symmetry were found within explicit limits with both medium and large snouts. Large snout produced higher beam output than that of medium snout at the centre of SOBP. This exploration can be extended to the determination of beam output, flatness and symmetry with a small snout.

The focus of this study is to find the optimal clinical tumour volume (CTV) to planning tumour volume (PTV) margins for precise radiotherapy treatment of prostate cancer. The geometrical shape of the target volume posses challenges in accurately identifying the CTV to PTV margins, especially when the organ affected by cancer demonstrates anatomical variations and the surrounding organs have high radio-sensitivity, in comparison to the organ of origin of the cancer.

The geometrical margins of CTV to PTV are investigated using portal imaging, in three directions. This study is carried out on 20 patients treated by the external photon beam radiotherapy of prostate cancer using standard accelerator without stereotaxic and without prostate markers.

Based on previous studies and the findings of our work, we propose CTV to PTV margin of 5·84 mm in the lateral direction, of 5·1 mm in the cranio-spinal direction and of 7·3 mm in the antero-posterior direction for external photon beam radiotherapy of prostate cancer.

The proposed CTV to PTV margins ensure high radiotherapy treatment precision of prostate cancer.

The purpose of this study was to determine the dose enhancement factor (DEF) of gold nanoparticles in a dosimeter gel and construct percentage depth dose curves, using the Optical CT system and the Monte Carlo simulation model, to determine the effect of increasing the dose caused by increasing the concentration of gold nanoparticles at depths in the gel.

The Magic-f Gel was made based on the relevant protocol in the physics lab. To determine the amount of the increase in the absorbed dose, the gold nanoparticles were added to the gel and irradiated. An increase in the dose after adding nanoparticles to the gel vials was estimated both with the Optical CT system and by the Monte Carlo simulation method.

Dose enhancement curves for doses of 2, 4 and 6 Gy were prepared for gel vials without adding nanoparticles, and nanoparticle gels at concentrations 0·17, 3 and 6 mM. Also, the DEF was estimated. For the 0·17 mM molar gel, the DEF for 2, 4 and 6 Gy was 0·7, 0·743 and 0·801, respectively. For the 3 mM gel, it was 1·98, 2·5 and 2·2, and for the 6 mM gel, it was 37·4, 4·24 and 4·71, respectively.

The enhancement of the dose after adding gold nanoparticles was confirmed both by experimental data and by simulation data.

The success of radiation therapy for cancer patients is dependent on the ability to deliver a total tumouricidal radiation dose capable of eradicating all cancer cells within the clinical target volume, however, the radiation dose tolerance of the surrounding healthy tissues becomes the main dose-limiting factor. The normal tissue adverse effects following radiotherapy are common and significantly impact the quality of life of patients. The likelihood of developing these adverse effects following radiotherapy cannot be predicted based only on the radiation treatment parameters. However, there is evidence to suggest that some common genetic variants are associated with radiotherapy response and the risk of developing adverse effects. Radiation genomics is a field that has evolved in recent years investigating the association between patient genomic data and the response to radiation therapy. This field aims to identify genetic markers that are linked to individual radiosensitivity with the potential to predict the risk of developing adverse effects due to radiotherapy using patient genomic information. It also aims to determine the relative radioresponse of patients using their genetic information for the potential prediction of patient radiation treatment response.

This paper reports on a review of recent studies in the field of radiation genomics investigating the association between genomic data and patients response to radiation therapy, including the investigation of the role of genetic variants on an individual’s predisposition to enhanced radiotherapy radiosensitivity or radioresponse.

The potential for early prediction of treatment response and patient outcome is critical in cancer patients to make decisions regarding continuation, escalation, discontinuation, and/or change in treatment options to maximise patient survival while minimising adverse effects and maintaining patients’ quality of life.

In advanced radiotherapy techniques such as intensity-modulated radiation therapy (IMRT), the quality assurance (QA) process is essential. The aim of the study was to assure the treatment planning dose delivered during delivery of complex treatment plans. The QA standard is to perform patient-specific comparisons between planned doses and doses measured in a phantom.

The Delta 4 phantom (Scandidos, Uppsala, Sweden) has been used in this study. This device consists of diode matrices in two orthogonal planes inserted in a cylindrical acrylic phantom. Each diode is sampled per beam pulse so that the dose distribution can be evaluated on segment-by-segment, beam-by-beam, or as a composite plan from a single set of measurements. Ninety-five simple and complex radiotherapy treatment plans for different pathologies, planned using a treatment planning system (TPS) were delivered to the QA device. The planned and measured dose distributions were then compared and analysed. The gamma index was determined for different pathologies.

The evaluation was performed in terms of dose deviation, distance to agreement and gamma index passing rate. The measurements were in excellent agreement between with the calculated dose of the TPS and the QA device. Overall, good agreement was observed between measured and calculated doses in most cases with gamma values above 1 in >95% of measured points. Plan results for each test met the recommended dose goals.

The delivery of IMRT and volumetric-modulated arc therapy (VMAT) plans was verified to correspond well with calculated dose distributions for different pathologies. We found the Delta 4 device is accurate and reproducible. Although Delta4 appears to be a straightforward device for measuring dose and allows measure in real-time dosimetry QA, it is a complex device and careful quality control is required before its use.
