We assessed the rate of recovery of fluoroquinolone-resistant and fluoroquinolone-susceptible Escherichia coli isolates from culture of frozen perirectal swab samples compared with the results for culture of the same specimen before freezing. Recovery rates for these 2 classes of E. coli were 91% and 83%, respectively. The majority of distinct strains recovered from the initial sample were also recovered from the frozen sample. The strains that were not recovered were typically present only in low numbers in the initial sample. These findings emphasize the utility of frozen surveillance samples.

We analyzed the Spanish hospital discharges registered from 1997 to 2005 with patient diagnoses of “intestinal infection due to Clostridium difficile.” The mean annual incidence rate was 41.2 diagnoses per 100,000 discharges. A significant increase was found from 1997 to 2005 (slope of the regression line [b], 5.12; P < .001). Overall rates were 2.5 times as high in the group of patients aged at least 65 years as in the group aged 45-64 years.

We report on 2,688 nosocomial infection episodes involving 3,238 pathogens in Taiwan's only children's hospital. The mean annual incidence density was 4.06 cases per 1,000 patient-days. The relative risk was highest in intensive care units, oncology wards, and neonatal observation wards. Very young, low-birth-weight, critically ill, and oncology patients were most at risk.

A quasi-experimental before-after study was conducted to determine the impact on prescribing habits and economic outcomes of an intervention that required use of an order form for surgical antimicrobial prophylaxis. Implementation of this intervention improved compliance with guidelines for prevention of surgical site infections, particularly with respect to choice of antibiotic, duration of use, and dosage, and resulted in decreased cost of surgical prophylaxis.

Cellulose acetate coatings containing the light-activated antimicrobial agents toluidine blue O and rose bengal have previously been shown to be successful in killing a range of microorganisms. Here, we report on the ability of these coatings to achieve reductions in the microbial load on surfaces in a clinical environment.

A total of 5,628 neurosurgical patients were observed in France to assess the occurrence of surgical site infection (SSI). Their risk of SSI was defined by calculating both the US National Nosocomial Infection Surveillance and the Brest National Nosocomial Infection Surveillance risk indexes. This study compares SSI rates stratified according to either the US or Brest (France) National Nosocomial Infection Surveillance risk index. The SSI rates were correlated with National Nosocomial Infection Surveillance data involving only local operation durations.

A matched case-control study was performed to identify predictors of mortality among patients (n = 46) with community-onset infections due to extended-spectrum β-lactamase-producing Escherichia coli in Thailand. The crude mortality rate was 30%. By multivariable analysis, community-onset bloodstream infection due to extended-spectrum β-lactamase-producing E. coli was the sole predictor of mortality (adjusted odds ratio, 41.3 [95% confidence interval, 4.3-69.4]; P = .001).

Data from an electronic medical record were used to demonstrate a large variation in the proportion of patients treated with vancomycin in 56 newborn intensive care units, which ranged from 18% to 70% . Use of oxacillin or nafcillin instead of vancomycin was rare during the first few years of the study period but was routine in 13% of the newborn intensive care units during the last fewyears of the study period. The use of electronic medical record data for studies of antibiotic use is discussed here.

Determining sensitivity and specificity of a postoperative infection surveillance process is a difficult undertaking. Because postoperative infections are rare, vast numbers of negative results exist, and it is often not reasonable to assess them all. This study gives a methodological framework for estimating sensitivity and specificity by taking only a small sample of the number of patients who test negative and comparing their findings to the reference or “gold standard” rather than comparing the findings of all patients to the gold standard. It provides a formula for deriving confidence intervals for these estimates and a guide to minimum requirements for sampling results.

This 5-year study of 25,378 hospitalizations measured the utility of infection control documentation of prior methicillin-resistant Staphylococcus aureus (MRSA) colonization or infection for the optimization of empirical antibiotic therapy. Documented prior MRSA colonization or infection was predictive of subsequent MRSA infections (odds ratio, 4.05). Physicians appear to use this documentation when prescribing empirical therapy for suspected bacteremia.

Air samples taken in a hospital undergoing construction and analyzed with a quantitative polymerase chain reaction (qPCR) assay for the Aspergillus genus did not show elevated concentrations of Aspergillus or particulate matter with a diameter of 5 μm or less in patient areas. Air samples from the construction zone indicated the containment system, which used polyethylene film barrier and negative pressure, was effective.

A prospective study of incident hepatitis C in 515 gastroenterology patients was conducted by follow-up sampling at 3-6 months after admission to the gastroenterology unit to test for antibodies to hepatitis C virus and for hepatitis C virus RNA. Universal precautions were implemented, and the use of multidose vials had been banned in this unit. Despite 5,964 exposure–days for several risk factors associated with nosocomial hepatitis C virus transmission, no incident case of hepatitis C occurred.

We compared the rates of exposure to blood in the operating room among third-year medical students during 2005-2006 with the rates reported in a study completed at the same institution during 1990-1991. The number of medical students exposed to blood decreased from 66 (68%) of 97 students during 1990-1991 to 8 (11%) of 75 students during 2005-2006 (P < .001)

In a cohort study of 36 patients with community-onset extended-spectrum β-lactamase (ESBL)–producing Escherichia coli or Klebsiella pneumoniae bloodstream infections, we found that predictors of mortality were community-onset infection with ESBL-producing K. pneumoniae pathogens (P = .02) and failure to receive an initial empirical regimen that included either β-lactam and β-lactamase–inhibitors or a carbapenem (P = .04).

In a survey of 150 physicians from an area where avian influenza virus (H5N1) infection is endemic, practice location (adjusted odds ratio [aOR], 4.45 [95% confidence interval {CI}, 1.02–26.4]) and the belief that rapid tests reliably predicted H5N1 infection (aOR, 5.6 [95% CI, 1.14–33.6]) were associated with not prescribing antiviral therapy; the belief that antiviral therapy reduced mortality was associated with prescribing an antiviral agent (aOR, 0.56 [95% CI, 0.14–0.95]).00

We prospectively evaluated 46 possible risk factors for methicillin-resistant Staphylococcus aureus (MRSA) surgical site infection (SSI) among patients with MRSA carriage in a large intervention study. Of 6,130 study patients, 68 (1.1%) developed MRSA SSI, which occurred a median of 14 days after surgery. Risk factors associated with MRSA SSI were receipt of emergency surgery, presence of comorbid condition, receipt of immunosuppressive therapy, receipt of contaminated surgery, and a surgical duration longer than the 75th percentile. MRSA carriage on admission did not predict MRSA SSI.

We analyzed the epidemiologic characteristics and risk factors for surgical site infection (SSI) in kidney transplant recipients. From among 1,939 kidney transplant recipients, 120 with corresponding control subjects were evaluated in this study (1:1 ratio). Reoperation, chronic glomerulonephritis, acute graft rejection, delayed graft function, diabetes, and high body mass index were identified in the analysis as risk factors for SSI.

The incidence and factors associated with inappropriate use of antifungal medications were studied in a Thai tertiary care center. The incidence of inappropriate antifungal use was 74% (in 42 of 57 patients). Isolation of Candida species from urine (P = .004) was a risk factor, whereas receipt of an infectious diseases consultation (P = .004) was protective.

Coadministration of oral divalent or trivalent cation-containing compounds with oral fluoroquinolones may impair fluoroquinolone absorption. Among 3,134 patients who received a course of oral levofloxacin, coadministration was significantly associated with subsequent identification of a levofloxacin-resistant isolate. Strategies to curb the emergence of fluoroquinolone resistance should include avoiding the coadministration of divalent or trivalent cation-containing compounds and fluoroquinolone.
