The mean isolation rates of salmonellas from the mesenteric lymph nodes and faeces of healthy mink were 16·7% of 618 animals from three farms and 3·9% of 772 animals from four farms respectively. Salmonella senftenberg was the most commonly isolated serotype. S. typhimurium, S. dublin, S. livingstone, S. menston, S. enteritidis, S. bredeney and S. infantis were also seen.

The prevalence of salmonellosis in 316 dead mink from 12 farms was 0·6%.

The epidemiological aspects are discussed.

A number of workers have shown that antibodies to the influenza A viruses which are demonstrable in the sera of normal adults undergo periodic fluctuations in titre related to epidemic occurrence of the disease. The rise in antibodies accompanying infection and demonstrable by neutralization, agglutination-inhibition and complement-fixation tests is used as a method of diagnosis which is admitted to be of greater sensitivity than actual recovery of virus from the throat. Studies of the population during an epidemic have also revealed the existence of subclinical infection with rise in antibodies comparable to that occurring in those suffering clinical illnesses. But surveys of sera from large samples of the population show no general upward shift in antibody levels unless an actual epidemic occurs (Martin, 1940). The studies made by Francis, Magill, Rickard & Beck (1937), Hoyle & Fairbrother (1937), Rickard, Lennette & Horsfall (1940) and Martin (1940) also indicate the relative impermanence of the enhanced antibody levels consequent upon an epidemic. A relatively rapid decrease in antibody is shown both by the neutralization and complement-fixation tests during the 3–6 months after an epidemic and then a slower fall occurs until the next epidemic again causes a rise in titres. There have, however, been relatively few studies on large samples of populations situated in different geographical areas before and during an outbreak.

1. The tuberculin reactions of 259 Hong Kong Chinese, mostly adults, were determined by the Mantoux method. Each individual was tested with various dilutions of both old tuberculin and tuberculo-protein.

2. Of 215 subjects above the age of twenty, 93·1 per cent, yielded positive reactions to old tuberculin in dilutions of 1/1000 or higher.

3. Tuberculo-protein compared with old tuberculin of similar dilutions did not yield such a high percentage of positive reactions, seldom produced such vigorous reactions and never caused vesiculation. (This may have been due to deterioration.)

1. An M-producing strain of Streptococcus pyogenes type-12 was shown to be carried on the surface of apparently healthy tonsils for at least 3 years.

2. Loss of M substance in a strain of type-3 in a tonsillar carrier was shown to be by gradual replacement of matt forms by glossy variants. This pair of naturally occurring variants showed very different growth rates as mixtures in liquid media.

3. Under suitable conditions, glossy variants regularly appeared and replaced the M-producing strain from which they were derived in these and all other strains of type-3 and type-12 tested from a wide variety of sources.

4. It is suggested that the selection of variants by the differential nutrient value of the micro-environment is the deciding factor in carrier strains showing loss of M substance.

Nosocomial infection in intensive care unit (ICU) practice is a common problem and is associated with abnormal carriage of Gram-negative aerobic bacilli in the gastrointestinal tract, resulting in endogenous infections. Selective decontamination of the digestive tract (SDD) is a regimen aimed at preventing or eradicating this abnormal carriage.

A large number of trials examining SDD in ICU practice have been published, the vast majority showing a significant reduction in the incidence of nosocomial, Gram-negative infection. However, the impact on morbidity and mortality is much less certain. A recent meta-analysis has suggested a 10–20% reduction in mortality (3–6% absolute difference) with SDD. A discussion of these results is presented together with potential criticisms of SDD.

Several attempts have been made at various times to obtain a mathematical expression of the relation between dosage, body weight and death time. The subject has recently gained importance through the interesting publications of Dreyer and Walker, who have put forward such a formula, and have illustrated its application to toxicity determinations for a number of substances, and, in particular, for different samples of diphtheria toxin—a matter of considerable practical importance. I propose to examine the theoretical basis of their formula, and its relation to formulae which other workers have suggested, and to criticise the evidence with which they have illustrated its application to experimental material.

In France, surveillance of anti-tuberculosis drug resistance is performed by the Azay-Mycobacteria network, representing 30% of all culture-positive cases. We sought to validate administrative and clinical data gathered by the network in 2004 and to produce corrected resistance rates accounting for the observed misclassification. We reviewed a 10% sample of patients' records diagnosed in 2004 and measured the agreement between controlled data and data collected by the network by using the kappa (κ) statistic. A re-sampling bootstrap-based method was used to investigate the impact of bias found on resistance rates. Most of data collected by the network, such as demographic data, and country of birth had an excellent agreement (κ>0·8) with controlled data. The concordance was good (κ>0·6) for HIV status and tuberculosis site. The only variable slightly discordant with controlled data was prior history of treatment (κ=0·52). However, after correcting crude resistance rates for the observed misclassification, all estimated rates were within confidence intervals based on reported rates. This validation study is in favour of a good quality of data produced by the network, even though corrected rates are slightly different from observed rates. Therefore, data collected through the network may be used for policy making and tuberculosis programme evaluation. However, improvement in data collection regarding prior history of treatment should be considered.

Two general practitioners (GPs) with SARS-CoV-2 infection provided in-person patient care to patients of their joint medical practice before and after symptom onset, up until SARS-CoV-2 laboratory confirmation. Through active contact tracing, the local public health authorities recruited the cohort of patients that had contact with either GP in their putative infectious period. In this cohort of patient contacts, we assess the frequency and determinants of SARS-CoV-2-transmission from GPs to patients. We calculated incidence rate ratios (IRR) to explore the type of contact as an explanatory variable for COVID-19 cases. Among the cohort of 83 patient contacts, we identified 22 (27%) COVID-19 cases including 17 (21%) possible, three (4%) probable and two (2%) confirmed cases. All 22 cases had contact with a GP when the GP did not wear a mask, and/or when contact was ≥10 min. Importantly, patients who had contact <10 min with a GP wearing a facemask were at reduced risk (IRR 0.21; 95% CI 0.01–0.99) of COVID-19. This outbreak investigation adds to the body of evidence in supporting current guidelines on measures at preventing the transmission of SARS-CoV-2 in an outpatient setting.

We aimed to investigate the prevalence and associated factors of HSV-2 discordance and concordance in HIV-1-discordant couples. This study used the baseline data from a cohort study of HIV-1-discordant couples in Dehong prefecture of Yunnan province, China. Of 954 participating couples, 42·4% were affected by HSV-2, of which 20·4% were HSV-2-concordant positive, 7·6% were HSV-2-discordant where the male was HSV-2 positive, and 14·4% were HSV-2 discordant where the female was HSV-2 positive. Compared to HSV-2-negative concordance, HSV-2 discordance with an HSV-2-positive male spouse was significantly associated with characteristics of the male spouse, including Han ethnicity and being in a second marriage. HSV-2 discordance with an HSV-2-positive female spouse was significantly associated with characteristics of the female spouse, including Han ethnicity, having engaged in commercial sex, having a sexual relationship of <3 years and being HIV-1 infected. Compared to HSV-2 discordance, HSV-2-positive concordance was significantly associated with an education level of middle school or higher for both spouses, a sexual relationship of ⩾3 years, more frequent sex and having an HIV-1-infected male spouse. The findings highlight the need for HSV-2 prevention and treatment efforts to reduce HSV-2 transmission in this population, and emphasize the importance of implementing prevention interventions early in couples' relationships.

The purpose of this paper was to determine how contact behaviour change influences the indoor transmission of influenza A(H1N1)pdm09 among school children. We incorporated transmission rate matrices constructed from questionnaire responses into an epidemiological model to simulate contact behaviour change during an influenza epidemic. We constructed a dose–response model describing the relationships between contact rate, viral load, and respiratory symptom scores using published experimental human infection data for A(H1N1)pdm09. Findings showed that that mean numbers of contacts were 5·66 ± 6·23 and 1·96 ± 2·76 d−1 in the 13–19 and 40–59 years age groups, respectively. We found that the basic reproduction number (R0) was <1 during weekends in pandemic periods, implying that school closures or class suspensions are probably an effective social distancing policy to control pandemic influenza transmission. We conclude that human contact behaviour change is a potentially influential factor on influenza infection rates. For substantiation of this effect, we recommend a future study with more comprehensive control measures.

This study compares the presence and level of measles haemagglutination inhibiting antibody in the sera of primary school children in selected rural and urban areas of Kaduna State, Nigeria following a vaccination campaign. The results, analysed by Mann-Whitney statistical test at α=0·05, showed significantly higher levels of haemagglutination inhibiting antibody in all the age groups in urban areas when compared with rural areas. The implications of these findings on measles vaccination campaigns are discussed.

Immunoelectrophoresis of ultrasonically disrupted Haemophilus equigenitalis (contagious equine metritis organism) cells against rabbit and equine antisera disclosed at least 11 precipitating antigens. Two of these, a polysaccharide and a lipopolysaccharide–protein complex, were of high molecular weight and located on the cell surface. The remaining antigens were intracellular and were small- to medium-sized proteins.

The surface antigens were the most significant in relation to the serological response in infected horses. They also reacted with sera from apparently healthy cattle, but the reason for this was not determined. No serological cross-reaction between H. equigenitalis and species of Achromobacter and Moraxella was detected.

We have recently had the opportunity of studying a plague epidemic that broke out on Terceira Island, Azores, in 1908. Terceira Island contains a population of 45,000 souls, the epidemic spread over 27 parishes. About 260 persons were attacked, with a mortality of about 48%.

The antigenic response of guinea-pigs to live poliomyelitis viruses of all three types has been studied and the loss of antigenicity involved in inactivation by formaldehyde and by ultra-violet light have been determined.

The bronchial constriction occurring in death from acute anaphylaxis has been demonstrated radiologically by the use of lipiodol.

In a recent number of this Journal1, I have pointed out that the well-known antagonistic action of Ca to Na, was probably at the bottom of the peculiar “defense rupture” action of Ca, as described by Bullock and Cramer2, in relation to infection with the organisms of gas gangrene.

By means of the graphic records given on Plates II–VI and VIII the following facts have been illustrated.

Immunity to Erythrocytes.

Normal rabbit's serum is relatively innocuous for bullock's erythrocytes. The serum of an immunised rabbit acquires the power to dissolve bullock's erythrocytes.

Besides acquiring the power to dissolve bullock's erythrocytes, an immune serum may also acquire power to clump them, and it has been shown that the phenomena of haemolysis and of agglutination are independent.

The powers acquired by the immune serum can be artificially modified. The serum may be deprived of its powers by heat. Serum cautiously so deprived of its haemolytic power can have it restored by the addition of normal serum. The haemolytic power of the un-heated serum is augmented if normal serum be superadded.

It has been shown that an immune serum only differs from a normal serum by its containing antitoxic bodies which are endowed with powers of specific reaction with the bullock's erythrocytes.

The mechanism by which erythrocytes are laked by an immune serum has been analysed, and it has been shown that the solution of the erythrocytes is effected through the intervention of an anti-erythrocytic body called forth by immunisation. The erythrocytes which have been subjected to the action of this product of immunity give indication of their reaction with it if they are subsequently or concomitantly placed under the influence of normal serum. The erythrocytes and normal serum together, therefore, form a combined indicator of the presence of the anti-eiythrocytic body. The part played by normal serum has nothing to do with the acquisition of immunity.

The only conclusion drawn from the above observations is that in the production of immunity to erythrocytes the serum of the immunised animal acquires certain powers which are concomitant with, but are not necessarily the cause of the immunity. This special case of immunity to erythrocytes is therefore probably parallel to induced immunity to those bacterial toxines for which antitoxines are known to exist.

The course and progressive augmentation of artificial immunity to erythrocytes has also been illustrated, and it has been shown that erythrocytes saturated with anti-erythrocytic body retain the power to augment the immunity of an already immune animal.

The serum of an animal actively immunised has power to confer passive immunity upon other animals, and the course of this passive immunity differs in the two cases when it is induced in the same species and in a species alien to that providing the immune serum.

The experiments with bullock's erythrocytes have been repeated in parallel observations with ricin in order to permit of the observations on haemolysis being utilised in drawing conclusions on the behaviour of bacterial toxines.

By adjusting the conditions of experiment in such a way that the minimal lethal dose for an animal was also the minimal agglutinating dose in test-tube experiments, it has been possible to give graphic records showing the parallelism between the processes when erythrocytes or living animals are used as indicators of the presence of free ricin. In this way it has been possible to illustrate the determination of the minimal lethal and minimal agglutinating doses of ricin and that quantity of antitoxine (antiricin) which is necessary to abolish the corresponding actions in the animal and in the test-tube, and to show that the mixture of toxine and antitoxine which is physiologically neutral in vitro is also physiologically neutral in vivo within the limitations imposed by the preliminary determinations.

The consequences of conferring passive immunity upon the guinea-pig by means of active immune serum of the rabbit have also been illustrated, and it has been shown that the alien antiricin serum leads to the production of agencies directed against itself.

Ricin neutralised by antiricin retains its power to produce immunity when injected into the species of animal which has yielded the antiricin.

In connection with the conference of immunity to erythrocytes and to ricin, the nature of the difference between normal and immune sera has been studied. Attention has been directed to the possession by normal sera of properties which simulate those possessed in more marked degree by the immune sera. In the case of haemolysis, it has not been possible to clearly demonstrate that the actions manifested by the normal and immune sera are distinct, although the weight of evidence is in favour of this view. In the case of ricin, however, it has been possible to demonstrate that the immune serum possesses properties which are quite distinct from those possessed by normal serum, and that the latter does not interfere with the action of ricin because of the natural presence of a trace of antiricin. In the case of immunity to ricin, the antitoxine is certainly something which has been super-added to the serum in consequence of the process of immunisation.

The facts ascertained in regard to artificial immunity to erythrocytes and to ricin completely agree. Only in oue point is it impossible to be quite sure that the phenomena are identical, viz., in the simulation by normal serum of the powers characteristic of the immune serum; for the demonstration that the two are distinct has been possible for ricin, but open to doubt in the case of erythrocytes. My investigations have been extended to diphtheria and tetanus toxines and to cobra venom, kindly placed at my disposal by Sir Thomas R. Fraser. They have however been interrupted, but so far as they go they support fully the observations made on ricin and erythrocytes.
