Individuals aged over 80 years represent the fastest growing segment of the population. It is becoming increasingly important to investigate the effect of age on cognitive functions such as language, in order to document “normal” and “abnormal” functioning. A task commonly used to test naming ability in clinical practice is the Boston Naming Test (BNT). Although norms exist for this age group, they may have limited applications because of small sample sizes on which they were derived. In addition, this test uses stimulus items that have been shown to be culturally specific. This study presents normative data for the BNT for two levels of education and two age bands based on a randomly selected Australian sample of older adults between the age of 81 and 94 years. Frequencies of the most common error types made in this group of nondemented individuals are also reported.

This study examined subjective and objective cognitive functioning in 26 female breast cancer survivors (BCS) who received chemotherapy treatment that finished .5 to 5 years prior to testing and compared their results to 25 demographically matched women with no history of cancer. Participants were assessed on prospective memory (PM) tasks; neuropsychological tests of processing speed, attentional flexibility with greater cognitive load, executive function, and verbal memory; self-report measures of cognitive dysfunction and PM failures; and distress. The BCS group showed significantly slower speed of processing and reduced attentional flexibility, and reported significantly more cognitive complaints and PM failures than the control group on five of six self-report measures. The groups did not differ on other PM or neuropsychological measures or on a measure of distress. Subjective cognition correlated with some neuropsychological tests and with a virtual reality PM task. Objective cognitive impairments were associated with reduced quality of life in the BCS group. The results provide some evidence of both self-reported impairment and objective cognitive dysfunction following chemotherapy treatment.

Background: To retrain upper limb function after stroke, a high dose of activity-related therapy is recommended. However, observational studies indicate that the dose undertaken is minimal. While it is speculated that those with severe disability will perform less therapy, this remains to be explored.

Objective: Quantify the dose and content of upper limb therapy performed by stroke survivors with severe upper limb disability during routine inpatient rehabilitation.

Methods: Therapy provided by physiotherapists and occupational therapists to 32 stroke survivors receiving inpatient rehabilitation over 20 weekdays was recorded. Dose of individual and group therapy was analysed by discipline and severity of upper and lower limb disability. Dose and content of individual therapy was also analysed by functional domain.

Results: On average, 46 minutes of individual and 11 minutes of group upper limb therapy were provided per participant, per day. Occupational therapists provided a higher dose of both individual and group therapy compared to physiotherapists (p < 0.0005). Findings suggest that greater residual upper and lower limb movement can lead to provision of a higher dose of activity-related therapy. Within individual therapy, a higher dose (29 versus 17 minutes, p < 0.002) and greater number (1218 versus 549) of impairment- than activity-related interventions were administered.

Conclusions: The dose of activity-related upper limb therapy provided to stroke survivors with severe disability was limited. There is a need to identify interventions and models of service delivery that can increase the intensity and appropriateness of therapy that stroke survivors with severe disability undertake during inpatient rehabilitation.

There are many challenges in incorporating an evidence-based approach into clinical practice and a large literature exists on various barriers and facilitators. Within this context, we have developed a framework to advocate an empirical approach to therapeutic interventions, using our Model to Assess Treatment Effect (MATE). The MATE is designed to characterise how an intervention is delivered to an individual patient/client. It is organised hierarchically into seven levels, from low levels of capacity to demonstrate treatment effect (e.g., Level 1: treatment commenced without pre-intervention assessment) to high levels where cause–effect relationships can be established between the intervention and the behaviour being treated (e.g., Level 6: implementation of a well-designed single-case experimental design). Consequently, the MATE captures all levels of clinical intervention and can be applied readily to an individual patient, as well as an entire clinical setting. In this paper, we present a decision tree in the form of a detailed flow-chart which assists the clinician to classify treatment delivery on the MATE. We believe that the MATE has the capacity not only to document current clinical practice, but also to provide a model whereby future treatments can be planned and implemented with greater scientific rigour and hence accountability in relation to treatment outcomes.

Apathy is a decrease in behavioural, cognitive and emotional components of goal-directed behaviour. Clinically, it is characterised by diminished initiation, reduced concern, and decreased activity. Apathy is a common occurrence following traumatic brain injury (TBI), occurring in around 60% of people. Consequences are widespread, negatively impacting independence, social integration, rehabilitation outcome, vocational outcome, coping and caregiver burden. The current knowledge base on apathy following TBI is presented, with implications for clinical practice. This includes a review of clinical presentations, neuroanatomical and neurochemical substrates associated with apathy, and differential diagnoses. Instruments to measure apathy are presented, highlighting those with demonstrated reliability and validity for the TBI population. Current evidence for pharmacological and non-pharmacological methods of treatment is described, with a model for non-pharmacological interventions provided and discussion of challenges faced by clinicians when treating the patient with apathy. In the TBI arena, greater understanding of apathy and methods of treatment is pivotal given the frequency of occurrence and widespread negative consequences.

Various studies have shown the efficacy of errorless learning (EL) in teaching patients with dementia a wide variety of skills and everyday tasks, with some studies showing beneficial effects and others reporting limited or no advantage. However, EL procedures vary greatly and, to date, no clinical guidelines or manuals are available. Here, we present a nationwide survey exploring the interest in, and feasibility of, EL in dementia care in The Netherlands. Based on the survey results and available evidence in the literature, we subsequently drafted an EL manual and had this concept manual evaluated in a Delphi round using the AGREE instrument. Forty-five health professionals associated with 22 dementia care facilities in The Netherlands, including those survey respondents who had piloted an EL intervention in accordance with the concept manual and an eight-strong expert panel representing various disciplines, deemed EL to be meaningful and feasible for use in dementia care and their residential facilities. Although our manual was favourably received, future studies are required to examine how EL can best be implemented in clinical practice and to determine the optimal outcome measures and quality indicators to reliably evaluate intervention outcomes and to consider the cost-effectiveness of the approach.

Background: Traumatic brain injury (TBI) is a leading cause of disability in Australia. Evidence shows that multidisciplinary rehabilitation and support in the six months following TBI is important for successful independent living and social re-integration. Despite this, access to services and supports during this period is often limited by environmental, socio-economic, geographic and cultural factors. Australian studies on outcomes after brain injury have reported primarily on non-Indigenous people. This study will investigate key sentinel events during the transition from hospital to home after a TBI in the first longitudinal study with Indigenous Australians.

Method: Indigenous Australians admitted to one of three major trauma hospitals in northern Australia with a TBI, and their care givers, will be recruited. Clinical and brain injury risk factor information, along with measures of cognitive function, transition events, mental health and community re-integration will be collected at three time points prior to hospital discharge, and at three and six months post-discharge. Qualitative interviews will also be conducted. Data will be analysed using regression methods for the quantitative component, and situational analysis for the qualitative component. Annual rates of brain injury will be calculated for patients admitted to tertiary hospital facilities in the study region with a diagnosis of TBI.

Discussion: Understanding the experience and events which shape the transition period is critical to determining the services and supports that may enhance transition outcomes, and ensure that such services are culturally appropriate and endorsed by Indigenous families and communities.

Previous studies have indicated an increased likelihood of intellectual and memory impairments in children with epilepsy. It has been argued that early surgical intervention in children with intractable seizures might be advantageous to their development. We conducted a retrospective review of pre- and postoperative IQ and verbal memory scores in 16 paediatric patients who underwent temporal (N = 10) or extra-temporal (N = 6) focal surgery for epilepsy. Patients with an extratemporal seizure focus performed significantly worse than patients with a temporal lobe seizure focus on Full Scale IQ and Verbal IQ before surgery. Postoperatively a statistically significant decline in verbal memory, but not in any of the IQ indices was noticed in the extratemporal group. Individual outcome analyses revealed a significant decline in verbal memory in 22% of temporal lobectomy and 60% of extratemporal cases. Our review suggests that verbal memory but not IQ is vulnerable to postoperative decline in children who undergo focal extratemporal surgery for intractable epilepsy. The findings raise the possibility that extratemporal regions play a role in anterograde memory in children. Given the small number of subjects in this study, further investigations into the cognitive effects of paediatric surgical intervention are recommended.

Multiple sclerosis (MS) is an inflammatory disease of the central nervous system leading to physical and cognitive disability. The impact of the disease on social cognition has only come to light quite recently. The aim of this study was to evaluate the social cognition abilities of MS patients and their links with characteristics of the disease, such as physical disability, cognitive impairment and disease duration.

The performances of a group of 64 MS patients were compared with that of 30 matched healthy individuals in facial emotion recognition and Faux Pas tasks as well as on a battery of standardized neuropsychological tests.

The MS patients performed worse than the control group in the recognition of the expressions of fear and anger and in the interpretation of faux pas. The impairment in social cognition increased with overall disease course. Executive impairment did not correlate with the performance in the social cognition tests.

Our results show that emotional impairment is observed at early stages of the disease in the absence of cognitive dysfunction, even if social cognition abilities worsen with the progression of MS. These data highlight the need to attempt to identify these impairments in clinical practice.

Over the past two decades there has been an increasing number of patients surviving severe traumatic brain injury and entering rehabilitation facilities at an earlier phase in their recovery. These patients often present in a minimally conscious state and are difficult to objectively assess. As a result, a number of batteries have been developed targeting this population. The Western Neuro Sensory Stimulation Profile (WNSSP) was used in the present study to identify patterns in recovery of cognitive-sensory modalities. The WNSSP records of 25 patients in a minimally conscious state were examined and profiles of recovery over four test occasions of a subset of 12 patients were examined in detail. Results demonstrated that within this highly heterogeneous group, Object Manipulation was an early emerging skill, and trends to significance were also observed in Auditory Response, Visual Tracking and Olfactory Response. A case study is presented to illustrate how the WNSSP results can be utilised to direct individualised treatment planning.

Introduction: Cognitive training in Alzheimer's disease (AD) has recently started to demonstrate its efficacy. We used our ‘puzzle-like’ task (GEO) as training for a large group of early-stage AD patients, to detect its effects over time.

Method: AD patients (N = 40) and healthy controls (N = 40) were involved. Participants were administered the Geographical Exercises for cognitive Optimization (GEO) task. Participants underwent individual sessions with GEO three times a week for 2 months, and then their performance was recorded again. Lastly, at the 12-month follow-up the GEO task was administered for the last time.

Results: Patients’ scores were significantly worse than controls’ scores only on a few neuropsychological tests. We ran a repeated measures GLM by considering groups’ performance on the GEO task at the assessment points. Results showed a significant main effect of group, and a significant effect of the interaction between group and time: patients’ performances both at the end of the training and at the follow-up were virtually identical to controls’ performances.

Conclusions: Patients effectively acquired new procedural abilities, and their achievements were stable at follow-up. This study suggests the GEO is a useful strategy for cognitive training in AD, and should prompt further investigations about the degree of generalisability of patients’ acquired skills.

Objectives: The Griffith Empathy Measure (GEM) is a self-report measure of empathy. The current study aimed to explore the reliability and validity of the Chinese version of the GEM. It also aimed to compare individuals with and without schizotypal personality disorder (SPD) features on various components of empathy.

Methods: 420 college students from Beijing and 526 college students from Guangzhou completed a set of questionnaires that measured empathy and schizotypal personality disorder traits. Cronbach's coefficient alpha was used to evaluate the internal consistency of the GEM. Construct validity was evaluated using exploratory and confirmatory factor analyses (EFA and CFA). Construct validity was also examined by looking at the relationship between the GEM and the Interpersonal Reactivity Index (IRI). Finally, the sensitivity of the GEM was evaluated by comparing the total and factor GEM scores between individuals with and without SPD traits.

Results: The Chinese GEM showed good internal consistency (Cronbach's alpha = 0.83). Results of an exploratory factor analysis suggested a three-factor model consisting of cognitive, affective and behavioural empathy components. Results of a confirmatory factor analysis showed that the three-factor model, as well as the two-factor model found in the English version, were both acceptable. Construct validity (specifically convergent validity) was also corroborated by significant correlations between the IRI subscales and GEM (personal distress: r = .09, p > .05; perspective taking: r = .34, p < .01; fantasy: r = .44, p < .01; empathic concern: r = .51, p < .01). Individuals with SPD traits showed lower scores on the GEM than individuals without SPD traits (t(1,250) = −1.99, p = .05), thus confirming discriminative validity.

Conclusions: These preliminary findings suggest that the Chinese version of the GEM demonstrated satisfactory internal consistency and construct validity. In addition, the findings suggest that the GEM is culturally appropriate and researchers can use it to study empathy in healthy and clinical Chinese participants.

Culturally-appropriate neuropsychological assessment is important if we wish to gather valid information about the abilities of individuals from minority cultural groups. This study compares 20 Maori and 20 white New Zealanders (16–30 years) from a low socioeconomic group on a range of neuropsychological tests, some adapted in an attempt to increase their appropriateness for Maori. Results generally support the hypotheses that Maori score more poorly on tests that rely heavily on formal western education and concepts, and score as well as or better than the white New Zealanders on tests that rely on concepts valued by Maori (e.g., visuospatial abilities), or have some Maori content. The test adaptations in this study might provide ideas for test adaptation for other cultural groups.

Objectives: To consider the strategies currently used by family care givers to manage challenging behaviour in the community following ABI, and to identify what supports are available that support family care givers in addressing this issue.

Research design: Delphi study.

Methods: Service providers (n = 8) and family care givers (n = 3) completed a three-round Delphi process with the aim of reaching consensus (75% agreement) regarding research objectives. Round 1 consisted of an open-ended questionnaire in which items were identified; these items were then rated in Round 2 and 3 using a Likert scale. Statistical aggregation then allowed for quantitative analysis.

Results: The findings suggest a number of behaviour management approaches are being used by family care givers, including basic antecedent strategies, agreeing with the person's demands, and medication. Consensus was reached that only two services in SA provide specific support to family care givers regarding behaviour management in the community: Families4Families Inc. (a peer support network) and Private Specialists (e.g., [neuro]psychologists and [neuro]psychiatrists).

Conclusions: Findings emphasise the need for more specialised services specific to educating and supporting family care givers in the management of challenging behaviours following ABI. These services should be accessible, centralised, and easy for family care givers to navigate.

With visible disabilities, observers tend to overgeneralise from the disability. In contrast, with invisible disabilities such as traumatic brain injury and stroke, observers often fail to allow for challenges resulting from the disability. Persons who have suffered a stroke claim that people misunderstand their symptoms and stigmatise them as a result of these symptoms. This misunderstanding, which happens particularly with young survivors of stroke, may reflect people's causal attributions for symptoms that follow a stroke. Using a scenario design, this cross-sectional study examined whether people attribute ambiguous symptoms that may result from stroke to other causes (the stroke survivor's personality and age) and whether these attributions reflect the age of the stroke survivor. Participants (N = 120) read scenarios describing a male who was aged either 22, 72, or whose age was unstated and who showed four symptom changes: fatigue, depression, irritability and reduced friendships. For each symptom change, participants rated three causal attributions: the person's age, his personality and stroke. The age of the person in the scenario affected attributions; when the person in the scenario was 22, participants attributed his symptoms significantly more to his personality than to his age or stroke, whereas when he was 72, participants attributed his symptoms more to his age than to his personality or stroke and when his age was unstated, they attributed his symptoms equally to age, stroke and personality. Because misattributions for stroke symptoms hinder rehabilitation, therapy can target people's misattributions to enhance rehabilitation for survivors of stroke.

Objective: To explore the availability and perceived adequacy of service support for younger people with ABI and their families in Queensland during the transition phase from the perspective of service providers. Research Design: A qualitative research design utilising survey-based open-ended questioning. Methods and Procedures: Participants were service provider organisations in Queensland for younger people with aquired brain injury (ABI). A two-part survey was developed for the purpose of the study. The first part of the survey obtained general information regarding client base and service characteristics for each organisation, while the second part was designed to identify gaps in, and barriers and facilitators to service provision. Snowballing recruitment was used with the initial seed point an existing Brain Injury Network. Results: A response rate of 20% was obtained with a total of 12 surveys returned representing seven service providers. Services currently available included rehabilitation, consultancy, education and training, and practical and emotional supports. Existing gaps in, and barriers to service provision were clustered under three key themes: service infrastructure, transition support, and transition context. Some of the facilitators to service provision identified included interagency linkages and financial compensation for injury. Conclusions: Gaps in, and barriers to service provision during the transition phase were identified that could be utilised to develop models of service delivery to improve short- and long-term outcomes for younger people with ABI.
