Behavioural psychotherapy has been demonstrated as the treatment of choice in most forms of serious mental disorders. In some instances it interacts synergistically with drug therapies. The Buckingham Project is a field trial of a comprehensive mental health service that employs behavioural analysis and therapy in the management of all cases of adult disorders occurring in an epidemiologically defined population. A multi-disciplinary team provide in vivo treatment. Results suggest that the incidence and prevalence of major episodes of mental illness have been substantially reduced and that the need for traditional hospital-based care has been minimised. The significance of this project for planning health services and for the future of behavioural psychotherapy is discussed.

This study assessed the relative merits of sociodemographic variables and psychological variables in understanding women's fear of rape. A comprehensive understanding of the factors involved in women's fear of rape may allow for more effective interventions with women. Four hundred and eleven women, aged 18 years or older participated in the study, which surveyed their fear of rape and experience of rape. In agreement with previous findings, sociodemographic variables contributed significantly but relatively little (13%) to the variance in women's fear of rape. In contrast, psychological factors (perceived likelihood of being raped and perceived severity of consequences) predicted an additional 29% of the variance to this fear. Greater perceived likelihood of being raped functioned as a partial mediating variable between sociodemographic factors of age and relationship status and women's fear of rape. These results are encouraging as psychological models provide more avenues for restructuring such fear and modifying concordant, dysfunctional behaviours. Consequently, these results provide direction to intervention and education programs aimed at reducing women's fear of rape.

Research has demonstrated that life transitions lead to heightened experiences of loneliness, in part because they engender isolation. We tested whether the degree of isolation during a transition influences loneliness, and if this is due to their effects on social identity processes. Employing an experimental paradigm, Study 1 (N = 213) found that when the transition involved isolation from new networks, in this case studying at university online as opposed to in person, it led to more loneliness, and this was mediated through reduced social identification and continuity of group memberships. Study 2 (N = 215) replicated these effects using a different experimental paradigm in which the transition involved isolation from old group memberships, namely moving to a new neighbourhood from interstate. Study 3 (N = 2346) employed a quasi-experimental repeated-measures design to assess the impact of a highly isolating life transition on loneliness–COVID lockdown. Australians in prolonged lockdown experienced increases in loneliness and this was mediated through (lack of) continuity of group memberships. Overall, these results suggest that isolation needs to be considered when assessing the impact of life transitions on loneliness. Moreover, the mediation results indicate that isolating transitions may be responsible for loneliness because these make it difficult to maintain crucial group memberships and form a sense of identification with relevant new groups.

The 13-item Self-Efficacy Questionnaire for Specific Phobias (SEQ-SP) was developed as a measure of perceived ability to cope with phobic symptoms when approaching feared stimuli. This study examined the psychometric properties of the SEQ-SP in a sample of 43 Australian children and adolescents aged 7 through to 17 years. Participants met the DSM-IV diagnostic criteria for a specific phobia. Exploratory factor analysis yielded two reliable subscales labelled Cognitive/Physiological Efficacy and Behavioural Efficacy. A confirmatory cluster analysis revealed the possibility of three ad hoc clusters comprising of cognitive, physiological and behavioural classifications. Results furthermore provided preliminary evidence for the reliability and validity of the SEQ-SP. Psychometric evaluation revealed excellent internal consistency and test-retest reliability. Additionally, results indicated that higher SEQ-SP scores were significantly correlated with lower scores on the Child Anxiety Sensitivity Index and higher scores on the Behavioural Avoidance Test, displaying evidence of moderate to excellent construct validity. This new self-report measure has a potentially useful role in clinical work and research with children and adolescents presenting with a specific phobia. Subsequent research examining the clinical utility of the SEQ-SP to predict treatment outcome is discussed.

Twenty-eight non-clinical subjects, scoring either high or low on a variety of anxiety measures, were asked to match their perceived heart rate to a series of tones produced by a metronomal device, both at rest and during a period of mild exercise. Actual heart rate was measured concurrently with an electrocardiogram. No differences were found between groups in the accuracy with which they were able to monitor their heart rates and, in fact, all subjects markedly underestimated their heart rate. Also, no significant correlations were found between heart rate awareness and any of the measures of anxiety, except for a negative relationship between awareness and scores on the Anxiety Sensitivity Index. Theories of anxiety, and methodological considerations, are discussed.

In his address to the Annual Conference of the Australian Behaviour Modification Association in 1986, Dr. Robin Winkler chose the topic “The social history of behaviour modification in Australia” (Winkler & Krasner, 1987). Dr. Winkler was concerned to recognise the contributions of a number of individuals who were prominent in the new movement in the 50s, 60s and 70s. My aim is rather different. I want to try to capture what the early workers were trying to achieve, what they saw as the problems, and how they viewed the early developments. I will then look at more recent developments in Australian behaviour therapy, and try to characterise its current status. Finally, I'll discuss what seem to me the major current problems, and suggest some possible solutions. Where I feel able to do so, and it seems to me appropriate, I'll make some comparisons with the situation in the USA. Many of the more general points, of course, will be relevant to behaviour therapy in any country.

This article discusses the problem of jealousy in couple relationships. The clinical presentations are summarised, and the question of what is morbid jealousy is addressed. Issues in the assessment of jealousy in couples are discussed, with suggestions for the areas to be covered. A framework for the formulation of the problem is proposed. Finally, the clinical treatment of morbid jealousy is reviewed. A treatment approach is described, which includes relationship enhancement work and a range of specific techniques.

Routine outcome measurement and progress monitoring is well established to enhance quality assurance in clinical psychology service delivery but is not widely used in routine care. A major barrier to more widespread implementation is the lack of public domain, brief, psychometrically sound outcome measures that easily integrate into clinical information systems. The current study assessed a brief 10-item version of the widely used Depression Anxiety Stress (DASS)-42 scale, which we called the Depression Anxiety Stress-10 (DASS-10) scale. In two clinical samples of adults (n = 1036, 445 men, 591 women; and n = 1084, 493 men, 591 women), the DASS-10 had a replicable two-level factor structure, which at the lower level had two factors assessing stress-anxiety and depression, which each loaded onto a superordinate psychological distress scale. The items in the distress score discriminated between a clinical sample (n = 376) and a community sample (n = 379) and were sensitive to clinical change. The measure has the potential to make routine outcome measurement and progress monitoring more cost-effective to implement than existing measures, particularly when integrated with practice management software to make administration, scoring, and use easy.

Loneliness and isolation worsen health and wellbeing have been exacerbated by COVID-19, and represent a significant concern for supporting older adults. Music listening has effects that could be particularly supportive during periods of isolation. The aim of this study is to examine older adults’ music listening behaviour during the COVID-19 pandemic and explore music's social and emotional impact in this context. Semi-structured interviews enhanced with music-elicitation were carried out remotely between May and June 2021. Participants were self-selected, community-dwelling older adults residing in Northern Ireland (N = 14; 6 males; 60–83 years). Most were living with their spouse or family, all were of White ethnicity and had varying levels of education. Data were analysed using reflexive thematic analysis. Two main themes were identified: (1) Music as an emotional resource and (2) Music as a social surrogate. Older adults had a preference for using music to induce positive feelings, and used music for negative affect regulation and consolation. Music acted as a social surrogate providing company, and reminders of social relationships and experiences. Music listening was a valued behaviour during COVID-19. Findings have implications for how music listening might be used as an accessible, low-resource tool for supporting isolated older adults.

Self-instructional materials are widely used to help smokers develop cognitive-behavioural techniques which may assist them to stop smoking. The effectiveness of these materials may be augmented by a minimal therapist support component to encourage and supervise their use. Forty-five smokers were recruited to a smoking-cessation program at a community health centre, and were randomly assigned to either a self-instructional manual plus regular telephone contact condition, or to a manual-only condition. A higher proportion of participants in the telephone contact condition reported being abstinent at 3- and 6-month follow-ups, but not at a 12-month follow-up. There were no differences between the two conditions for the smoking rates of the remaining smokers at each of the follow-ups. Levels of adherence to the techniques presented in the manual were generally low, with a preference for the more-simple behavioural techniques, and those which were related to initial behaviour change rather than to maintenance. There was no relationship between levels of reported adherence to the techniques and smoking status at follow up. There is the need for further research on the cost-effective use of social support strategies to augment the impact of self-instructional behaviour-change materials.

The aim of this study was to determine the degree to which fearful and catastrophic cognitions, and self-efficacy for managing panic predicted various panic attack characteristics in panic disorder. The cognitive variables consisted of anxiety sensitivity, the frequency of fearful agoraphobic cognitions and measures of catastrophic misinterpretation of symptoms. The panic parameters were number and severity of panic symptoms, distress associated with panic attacks, worry about future panics, duration of panic disorder, and life interference due to panic disorder. These variables were measured in 40 people with panic disorder, 31 of whom also had significant agoraphobia. The frequency of fearful agoraphobic cognitions was the strongest predictor of panic attack symptomatology, predicting number of symptoms, symptom severity and degree of anticipatory fear of panic. Catastrophic misinterpretation of symptoms and anxiety sensitivity did not independently predict any panic parameters. Only self-efficacy for managing the rapid build-up of panic symptoms was specifically related to panic severity. The results therefore suggest that cognitive behaviour therapy for panic symptoms in panic disorder should reduce fearful cognitions rather than focus on panic coping strategies. The results offer little support for the contribution of the expectancy or catastrophic misinterpretation theories to the maintenance of panic disorder.

Individuals with intellectual disabilities commonly engage in a variety of problematic behaviours such as aggression and self-injury. This paper discusses the value of using a communicative model to study these behaviour problems. First, the reciprocal nature of behaviour problems is outlined. Next, research on the use of functional communication training as a treatment for behaviour problems is reviewed in terms of its initial effectiveness, generalisation, and maintenance, as well as the mechanisms responsible for behaviour change. Finally, recommendations are made for future research and training efforts aimed at reducing severe behaviour problems.

Social problem-solving training has been successful in improving maladaptive behaviour and problem-solving skills for individuals with intellectual disability. However, in contrast to individuals without intellectual disability there has been only one study that has examined whether social problem-solving training can improve psychological distress in individuals with intellectual disability and a psychiatric diagnosis. Three participants from a vocational community centre with mild intellectual disability, comorbid mental illness, and challenging behaviour, participated in 15 individually delivered sessions of social problem-solving training. Social problem-solving skill, behaviour, and psychological distress measures were used to assess outcome. All three participants showed improvement in social problem-solving skills, and two participants showed improvement in depression. Improvement was maintained at 4-week follow-up. The results provide preliminary evidence that social problem-solving training could be an effective intervention tool for the treatment of psychological distress in individuals with mild intellectual disability.

The aim of this study was to assess the effects of a partially parent-implemented self-management intervention incorporating video-modelling for discrimination training on improving social skills in a child with autism spectrum disorder (ASD). The participant was a 9-year-old girl with ASD. A multiple baseline across behaviour design (no interruption, asking for opinions, and appropriately greeting unfamiliar adults) was used to assess the effects of the intervention. Results showed: (a) the intervention was associated with improvements in all target behaviours in the training setting with a strong overall treatment effect; (b) the behavioural gains were generalised to non-training settings and maintained in both fading and follow-up phases; and (c) social validity measured by the Behavior Intervention Rating Scale — Adapted version (pre- and postintervention) was high. The intervention was effective in improving social skills with this participant, with good generalisation and maintenance effects and high social validity.

The following review paper describes the outcomes of data-based behavioural intervention strategies for the defining characteristics of autistic disorder as defined by the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV; American Psychiatric Association, 1994). The results of relevant evaluations of behaviour analytic procedures are reviewed for each of the four potential areas of difficulty under each of the main criteria for autistic disorder, which are: (a) impairment in social interaction, (b) impairment in communication, and (c) restrictive, repetitive, and stereotyped patterns of behaviour, interests, and activities (DSM-IV). The current areas of research activity as well as those domains that have received comparatively little attention by behavioural researchers are identified, and suggestions for future research are discussed.

Clinical reasoning is one of the central components of psychological assessment. The identification of a client's psychological difficulties and the subsequent depiction of their onset, development, and interrelationships enables clinicians to plan treatment in a systematic and effective manner. In an article (Ward, Vertue, & Haig, 1999), we outlined the abductive theory of method (ATOM) and argued that it offered a useful framework for highlighting and integrating the major phases of psychological assessment. These phases involve detecting clinical phenomena, postulating psychological mechanisms, developing a case formulation, and evaluating a case formulation. In this article we present a revised version of the adaptation of ATOM and elaborate on the related clinical dimensions of assessment.

Perfectionism has been argued to have both positive and negative aspects. Negative perfectionism has a robust positive correlation with psychopathology. This study explored the personality pattern of a group of clinical participants and a group of athletes in relation to positive and negative perfectionism. The results indicated negative perfectionism is related to neuroticism and agreeableness in both clinical and non-clinical groups. Negative perfectionism was most strongly associated with low agreeableness but had no significant relationship with conscientiousness or extraversion in the clinical sample. In the athlete sample, higher negative perfectionism was most strongly related to higher neuroticism but was also associated with lower extraversion and conscientiousness. In order to more fully understand these relationships and their clinical implications, more studies using validated measures of positive and negative perfectionism with larger samples are required. It would be useful to determine if personality factors of agreeableness and competence could be increased in order to ameliorate the distress associated with negative perfectionism.

This study investigates whether hyperaccessibility occurs for supraliminally or subliminally presented sweets-related stimuli after prior suppression of thoughts about sweets. Thirty-three students (all female; 18—25 years old) participated in the experiment. In the first phase, half of the experimental group was instructed to suppress all sweets-related thoughts. The other participants were given control instructions. In the second phase, as part of a modified Stroop task, participants were asked to state the colour of a stimulus as quickly as possible. This stimulus could be presented either subliminally or supraliminally. In both conditions, neutral control words as well as sweets words were used. It was found that the participants in the suppression group, compared to those in the control group, showed attentional bias for the sweet-related suppressed thoughts, but that this effect was determined by the reaction times of subliminally presented sweets words. No differences were found for the control words. In addition, the study explored whether there was a relationship between thought suppression and dietary restraint attitudes. The link to dietary behaviour, however, remains unclear.

Psychological sense of community (PSOC) was studied within two types of virtual community and one type of community of place. Fifty-six members of these community groups completed the Sense of Community Index (SCI). An analysis of variance found partial support for the initial hypotheses with the intentional-community-of-place group scoring significantly higher on the SCI than the location-oriented virtual community. The results were discussed in the context of 3 PSOC correlates of depth of disclosure, reciprocity and shared interests/experience. It was concluded that virtual communities may contribute to overall PSOC.
