Mobility research tends to focus on physical movement and experiences in later life; however, changes in older adult mobility over time remains underexplored. Furthermore, older adults typically experience many life events, some of which result from cognitive and physical decline, and many of which impact mobility. This article aims to explore how life events affect the mobility of older adults over time. We conducted in-depth interviews with 22 older adults aged 55 years and over from Lancashire, United Kingdom. Of these participants, eight lived with memory problems. The findings show that both anticipated and unexpected life events play a profound role in the participants' mobility over time. Retirement, long-term illness and age-related illness were examples of anticipated life events, while the death of a loved one and developing memory problems were examples of unexpected life events. In both cases, participants' made external adaptations, such as moving home, or internal adaptations, such as self-awareness. The findings also emphasise the layered nature of life events and adaptations playing a role in the participants' mobility. Additionally, life events such as developing memory problems showed a domino effect, triggering further life events and adaptations which impacted the participants' mobility. This article emphasises how transition periods can occur before or after a life event, showing that adaptations can be pre-emptive to a life event. Our article contributes to calls for internal adaptations to be fully incorporated into age-related policy and also for age-related policy to be more inclusive for older adults who experience memory problems and dementia.

There is a special feature of the health and welfare problems of the elderly population that sets them off from the other domestic problems of the developed countries of the world. They combine two difficult elements, complex enough when taken alone but much more so when combined. There is, on the one hand, the economic problem of finding ways to make adequate provision for the needs of the old, barely met with full adequacy in any country and full of greater threats in the future. There is also, on the other hand, the extraordinary delicacy needed to conduct serious public debate on the issues. At a time when many countries still struggle against a pervasive ageism, trying to open up new possibilities for elderly people and to foster new attitudes, it is exceedingly painful to contemplate further economic constraints, even rationing, to meet the impending economic problems.

The information reported here comes from a study of a random sample of adult deaths registered in 1987. Interviews were sought with the person who could tell us most about the last twelve months in the life of the person who died; 639 people took part in the study. Just under one in ten of the people who died had attended a day centre in their final year. Attenders were more likely to be aged 75 or over at death, to be women and to have been living alone. They were also more likely to have suffered from a number of long-term symptoms and restrictions. According to respondents it would have been helpful if a further 11% of those who died had attended a day centre in the last year of their lives. The most often stated reason for this was that it would have given the carer a break from the demands of caring. But the most often given reason why the person who died had not attended a day centre was that they had not wanted to – an indication of some conflict between family and friends providing care and those they cared for

Older people are increasing in number and proportion of the population in Latin America and, as they do, their health and social needs are becoming more apparent. This paper describes new initiatives to promote health and well-being in Costa Rica, a Latin American country which is economically poor by international standards, but which is known for its emphasis on health and human services. It draws on research to provide perspectives on health and ageing, discuss initiatives from neighbourhood to nation, and analyse prospects for the future. It emphasises health and ageing in Costa Rica, but identifies issues in terms of their wider significance.

Three weeks after the outbreak of the Scud Missile crisis residents of Tel Aviv and the outlying regions, aged 50–91 participated in a study the focus of which was the relationship between personal (cognitive) control of the aversive environmental stimuli and (1) distress in areas of mood and affect, and (2) distress in interaction with the social environment. Personal control was measured along two dimensions: perceived control of the situation and attribution of meaning to events and their outcomes. Findings revealed no significant differences between age categories in levels of cognitive control or in levels of distress. Situation control emerged as the most significant variable in explaining variation in distress scores. Attribution of meaning, though significantly related to situation control, was only a moderately significant predictor of interaction distress. The findings support the thesis that the existence of a causal link between life events and psychological equilibrium makes theoretical sense only when the cognitive structure of such events for individuals is considered.

This paper reports on the study of a subsidy programme that was established in Quebec for alternate housing models (AHMs), which allows private and community organisations to offer housing services within the framework of a partnership with public health-care services. The research objectives were: (a) to compare how facility characteristics and services provided by AHMs and nursing homes (NHs) differ; (b) to examine the personal characteristics of residents living in AHMs; and (c) to compare residents with similar characteristics within AHMs and NHs in terms of unmet needs, quality of care, satisfaction with care and services, and psycho-social adaptation to the residence. A cross-sectional study was undertaken with individually matched groups to assess whether AHMs meet the needs of elders in a way similar to NHs. Overall, residents in both groups had moderate to severe levels of disability and about 60 per cent had mild to severe cognitive problems. While their general features were heterogeneous, the AHMs were more comfortable and homelike than the NHs. The quality of and satisfaction with care was appropriate in both settings, although AHMs performed better. Only one-quarter of residents in both settings, however, evidenced a good level of psycho-social adaptation to their residence. This partnership approach is a good strategy to provide a useful range of housing types in communities that can respond to the needs of elders with moderate to severe disabilities.
