The neurologic complications of endocarditis fall into three categories: nonspecific, cerebrovascular, and infectious. The nonspecific symptoms are largely those of systemic infection. They include confusion, personality change, and myalgias. The vascular manifestations are embolic cerebral infarction, bland or hemorrhagic, and mycotic cerebral aneurysm. The infectious complications typically occur with aggressive organisms such as staphylococcus. Brain abscesses, usually numerous small ones, and epidural spinal abscesses are not rare. A neurologic manifestation can be the chief complaint leading to the diagnosis of endocarditis.
