With the publication of this inaugural issue we launch The Cognitive Behaviour Therapist (tCBT). This new journal will offer high-quality, professional development-focused articles that advance the practice of the behavioural and cognitive psychotherapies. tCBT will be published quarterly, featuring articles on clinical and professional issues, which contribute to the theory, practice and evolution of the cognitive and behavioural therapies.

It is widely recognized that psychological factors play a central role in the adjustment process and subsequent management of chronic pain. The role of anxiety, and specifically pain-related fear, has received particular attention. Paralleling developments in the anxiety disorders literature, psychological models of pain-related fear now highlight the importance of cognitive processes in its maintenance and treatment. However, theoretical and treatment advances in the anxiety disorders literature have not been widely applied to the pain field. In particular, certain cognitive processes, specifically safety-seeking behaviours and imagery, which appear to be involved in the maintenance of pain-related fear. This paper explores how these concepts may apply to pain-related fear and demonstrates how they may aid conceptualization and be used to guide a more cognitively orientated and efficacious treatment.

Empirical investigations of mood variability during actual and ‘real time’ shopping episodes are absent from the compulsive-buying (CB) literature. A field experiment was therefore conducted with a participant meeting desired clinical and research requirements for identifying CB and a control subject for comparison purposes. Each participant went on four shopping trips and rated various parameters of mood and self-perception prior to, during and after each shopping trip. Each phase (i.e. pre-shop, shop and post-shop) lasted for 1.5 h, with ratings of mood and self-evaluation taken every 10 min. Results indicate that, although shopping was a guilty pleasure for the CB participant, there were more similarities than differences apparent in terms of mood and self-evaluation between the CB and control participant. The CB participant experienced significant levels of self-dislike during the shopping episodes. In terms of the comparisons between the pre-, during and post-shopping phases, the first 40 minutes of actual shopping seemed to be particularly arousing/exciting for the CB participant. The study is discussed in terms of key methodological requirements for increased validity and reliability in studying CB phenomena and the range of indicated possible cognitive and behavioural interventions.

Recent policy, service and financial drivers that are aimed at improving access to psychological services with a particular focus on cognitive behavioural approaches have resulted in a number local and national service planning initiatives. The extent to which these developments ought to be informed by theory and research regarding the transfer of classroom-based learning to the work setting is made clear. The existing evidence base has implications for how education providers develop collaborative curricula with NHS employers in order to prepare students for the practice setting, and for how service providers support the students' knowledge, skills development, skills transfer and consolidation of these within the practice setting. The dangers of assuming that the dissemination of the clinical evidence base is straightforward within complex organizations and the structure of the NHS are also critically discussed.

Data from a patient survey on self-help and coping strategies during waiting time for CBT are reported. Individuals on an outpatient treatment centre waiting list received a short questionnaire assessing problem-related activities carried out in the interval before therapy (mean waiting time 69 days). A total of 306 patients with a wide range of reliably diagnosed mental disorders were assessed. Results showed that the vast majority of patients (~95%) instigated at least one form of coping or self-help activity related to their mental problem. Resource-orientated strategies were most often reported (88%), but ‘seeking information about the problem’ was also highly prevalent. About one third of persons reported additional health-care utilization. Higher rates of symptom distress and depression were associated with additional health-care utilization and with more passive coping strategies such as distraction. Active self-help strategies contributed to poorer depression outcome and were, thus, no indicator of good prognosis. Other coping strategies were not linked to outcome variables. It can be concluded that problem-related processes do start before treatment. A more systematic integration of self-help activities into the case-formulation and more systematic research on the related (motivational) processes are recommended.

The identification of personality disorder among mental health service users is problematic but important because it is associated with high levels of comorbidity and possibly ineffective service delivery. This study assessed the prevalence of personality disorder using the Millon Clinical Multiaxial Inventory – 3rd edition (MCMI-III) compared with prevalence using frequency of assignment of diagnosis in people referred to an in-patient CBT clinical psychology service. Prevalence rates differed from 85% (MCMI-III) to 16% (diagnosis) using these different measures. Reasons for this difference and implications for CBT therapists' practice are discussed.
