JoEllen Patterson is Professor in the Marital and Family Therapy Program at the University of San Diego. She is also an Associate Clinical Professor of Family Medicine and Psychiatry at the University of California, San Diego, directing a medical family therapy clinic embedded in a Family Medicine residency (equivalent to General Practice in New Zealand). She also works in Pediatrics and Reproductive Medicine at UCSD. At present her work involves training family therapy students and family medicine residents to do interdisciplinary work using a biopsychosocial model. Research suggests that our bodies are influenced by our mental health; our immune systems and cardiovascular systems seem especially susceptible to mind-body interplay. JoEllen is particularly interested in how family functioning influences the mind-body system. In 2003, JoEllen Patterson received a Fulbright Senior Scholars award to visit the Wellington School of Medicine and Health Sciences, and the Child, Adolescent and Family Service in Lower Hutt, New Zealand. She spent two weeks between the clinical service and the academic department, teaching, consulting and advising. I was lucky enough to arrange this posting and to spend time with her during it.

The extent to which students feel accepted, valued, respected and included in the school has recently surfaced as one of the most important predictors of adolescent mental health (particularly depressive symptoms). The school environment is an established predictor of school connectedness, but we set out to examine whether parental attachment predicts both adolescents' perception of the school environment and school connectedness. A study of 171 high school students from years 8 to 12 showed that parent attachment strongly predicted both. We also confirmed that the relationship between parent attachment and school connectedness is not a direct one but that parent attachment influences individual differences in the way adolescents perceive the school environment, which in turn influences school connectedness. This finding shows how multiple systems might be interlinked in influencing wellbeing in adolescents, and confirms the importance of intervening at the double level of both the family and the school system.

Following the ‘discursive’ turn in family therapy, the attention of practitioners shifted towards understanding how culture and language shape meaning-making in therapy. In this article, we demonstrate how conversation analysis (CA) can be used to examine the processes and outcomes of systemic/constructionist practice. We used CA to study collaborative interactions of a renowned constructionist therapist Karl Tomm and one client–family. Viewing collaboration as a pivotal aspect of the therapeutic alliance, we demonstrate how the ‘split’ within-system alliances were developed and sustained in the course of therapy and how they were discursively transformed into ‘intact’ alliances. The therapist's efforts to align with perspectives of family members (and subsystems) seemed pivotal in this process.

Jeff Young talks about having been trained to be formal with clients, and his gradual realisation that he could return to the more direct ways of speech he learned as a child, and combine that with warmth. He found that clients and fellow professionals welcomed a therapist who did not ‘beat around the bush’. Together, Jeff and Ron talk about when directness is appropriate, and how directness needs to be modified for particular clients.

A disclosure of sexual abuse, which is then retracted, places the family therapist in a difficult position both ethically and in terms of whom the therapist's primary alliance is to. This case study will examine some of these difficulties and will describe the process of working with a family when child protection issues are suspected but not substantiated.

Catherine Sanders is a clinically trained psychologist and Director of Bower Place Psychology, Family Therapy, Psychotherapy & Management in Adelaide, South Australia. She has 28 years of clinical and teaching psychology and psychotherapy practice as an individual, couple, and marital and family therapist. Catherine is a Senior Clinical Associate at the School of Psychology, Flinders University, Clinical Supervisor, School of Psychology, University of Adelaide and Visiting Lecturer, University of South Australia. She was the president of the board of the Australian & New Zealand Journal of Family Therapy (2004–2005), board member and associate editor from 1984 to 1998, and member of the Australian Association of Family Therapy Working Group from 2004 to 2007. Catherine has published 13 papers and presented in excess of 300 presentations and conference papers.
