Clostridioides difficile infection (CDI) is a common and often nosocomial infection associated with increased mortality and morbidity. Antibiotic use is the most important modifiable risk factor, but many patients require empiric antibiotics. We estimated the increased risk of hospital-onset CDI with one daily dose-equivalent (DDE) of various empiric antibiotics compared to management without that daily dose-equivalent.

Using a multicenter retrospective cohort of adults admitted between March 2, 2020 and February 11, 2021 for the treatment of SARS-CoV-2, we used a series of three-level logistic regression models to estimate the probability of receiving each of several antibiotics of interest. For each antibiotic, we then limited our data set to patient-days at intermediate probability of receipt and used augmented inverse-probability weighted models to estimate the average treatment effect of one daily dose-equivalent, compared to management without that daily dose-equivalent, on the probability of hospital-onset CDI.

In 24,406 patient-days at intermediate probability of receipt, parenteral vancomycin increased risk of hospital-onset CDI, with an average treatment effect of 0.0096 cases per daily dose-equivalent (95% CI: 0.0053—0.0138). In 38,003 patient-days at intermediate probability of receipt, cefepime also increased subsequent CDI risk, with an estimated effect of 0.0074 more cases per daily dose-equivalent (95% CI: 0.0022—0.0126).

Among common empiric antibiotics, parenteral vancomycin and cefepime appeared to increase risk of hospital-onset CDI. Causal inference observational study designs can be used to estimate patient-level harms of interventions such as empiric antimicrobials.

Develop a Staphylococcus aureus wound antibiogram among patients who use fentanyl (PWUF) presenting with acute S. aureus skin and soft tissue infections (SSTIs) in Philadelphia, Pennsylvania.

Retrospective, multisite cohort study.

Individuals presenting to emergency departments or admitted to inpatient units of four Penn Medicine hospitals with an acute S. aureus SSTI and illicit fentanyl use within the previous year.

We described susceptibilities of S. aureus isolated from wound cultures among the PWUF cohort and compared these to the health system’s wound antibiogram. We compared frequency of in-hospital medication treatment for opioid use disorder among patients who left the hospital prior to vs after the availability of S. aureus susceptibilities.

Among 131 S. aureus isolates from 131 PWUF, 35/131 (26.7%) were susceptible to oxacillin, 73/121 (60.3%) were susceptible to clindamycin, 77/122 (63.1%) were susceptible to tetracycline, and 119/126 (94.4%) were susceptible to trimethoprim-sulfamethoxazole. PWUF displayed significantly reduced susceptibility to oxacillin and tetracycline compared to the health system’s outpatient wound S. aureus antibiogram. Compared to patients discharged prior to susceptibility availability, more patients discharged after the reporting of susceptibilities were administered buprenorphine or methadone in the hospital (82.0% vs 51.4%, P < 0.001).

High nonsusceptibility to clindamycin and tetracycline suggests these agents should not be prescribed as empiric therapy for acute S. aureus SSTI in PWUF in Philadelphia. PWUF would benefit from joint management by infectious diseases and addiction medicine experts to ensure prescription of active therapy. Additional study is needed of PWUF in other regions.

To evaluate the impact of implementing a clinical care guideline for uncomplicated gram-negative bloodstream infections (GN-BSI) within a health system.

Retrospective, quasi-experimental study.

A large academic safety-net institution.

Adults (≥18 years) with GN-BSI, defined by at least one positive blood culture for specific gram-negative organisms. Patients with polymicrobial cultures or contaminants were excluded.

Implementation of a GN-BSI clinical care guideline based on a 2021 consensus statement, emphasizing 7-day antibiotic courses, use of highly bioavailable oral antibiotics, and minimizing repeat blood cultures.

The study included 147 patients pre-intervention and 169 post-intervention. Interrupted time series analysis showed a reduction in the median duration of therapy (–2.3 days, P = .0016), with a sustained decline (slope change –0.2103, P = .005) post-intervention. More patients received 7 days of therapy (12.9%–58%, P < .01), oral antibiotic transitions increased (57.8% vs 72.2%, P < .05), and guideline-concordant oral antibiotic selection was high. Repeat blood cultures decreased (50.3% vs 30.2%, P < .01) without an increase in recurrent bacteremia. No significant differences were observed in 90-day length of stay, rehospitalization, recurrence, or mortality.

Guideline implementation was associated with shorter antibiotic therapy durations, increased use of guideline-concordant oral antibiotics, and fewer repeat blood cultures without compromising patient outcomes. These findings support the effectiveness of institutional guidelines in standardizing care, optimizing resource utilization, and promoting evidence-based practices in infectious disease management.

Evaluate the effects of handshake stewardship on adult general surgical units.

Retrospective quasi-experimental pre- and post-intervention study.

A total of 1,278 bed academic medical center with a level 1 trauma center in St. Louis, Missouri.

Adults admitted to general surgery units.

Once weekly handshake antimicrobial stewardship rounds were initiated in January 2022 on adult general surgery units. The handshake stewardship team consisted of an infectious diseases (ID) physician and pharmacist who reviewed charts of patients receiving systemic antimicrobials without a formal ID consult. Antimicrobial recommendations were delivered in person to general surgery teams including trauma, geriatric trauma, and emergency/general surgery.

A total of 1,241 charts were reviewed during the post-implementation period with 391 interventions. Seventy-two percent of those interventions were accepted and the acceptance rate improved over the 18-month post-implementation period. Total antimicrobial usage significantly decreased between the pre- and post-implementation period (608 vs 542 d of therapy/1,000 d present, P = 0.004). An interrupted time series found that there was an immediate (P < 0.001) and sustained (P < 0.001) decrease in antibiotic spectrum index during the post-implementation period. No difference was found for in-hospital mortality between the pre- and post-implementation periods [28 (1%) vs 29 (1%), P = 0.791].

A once-weekly handshake antimicrobial stewardship program was successfully implemented in general surgery units. Antimicrobial use significantly decreased without negatively impacting hospital mortality.

Emerging data surrounding the rise in antimicrobial resistance have prompted a shift towards shorter antibiotic durations. Studies show similar clinical outcomes comparing shorter antibiotic courses to longer ones for uncomplicated Gram-negative bloodstream infections (BSI). However, there is a lack of data to inform durations of therapy for Streptococcal BSI.

This was a retrospective cohort study of patients admitted to University of Utah Health with uncomplicated Streptococcal BSI. Inverse probability of treatment weighting (IPTW) was used to estimate the average treatment effects (ATE) of antibiotics administered for 10 days or fewer (short duration) versus more than 10 days (long duration). The primary outcome was a composite of recurrent BSI, all-cause mortality, and readmissions at 30 days from end of therapy.

Five hundred patients were screened and 196 were included in the final analysis. The most common sources were skin and soft tissue infections. The median duration in the short and long groups were 8 (IQR, 7–10) and 15 days (IQR, 14–17), respectively. The ATE of short versus long duration of antibiotics was not significant for the composite primary outcome (18% vs 18%; OR = 1.42 [95% CI: 0.57 to 3.53]).

We found no appreciable difference in outcomes between patients treated with short versus long antibiotic durations for uncomplicated Streptococcal BSI. Given low absolute rates of mortality and recurrent BSI, along with the lack of evidence indicating a significant difference related to treatment duration, it is reasonable to consider shorter durations. Future research is needed to confirm our findings.

To compare the clinical outcomes of patients with lower limb osteomyelitis (LLOM) and negative methicillin-resistant Staphylococcus aureus (MRSA) cultures treated with anti-MRSA therapy (AMT) versus those treated with no-anti-MRSA therapy (NAMT).

Retrospective cohort study.

Hospitalized adult (≥18 yr of age) patients admitted to multiple tertiary referral centers in a single healthcare system between April 1, 2017 and April 1, 2023, with LLOM and planned intravenous antibiotics for at least four weeks.

Electronic medical records were queried for demographic information, admission dates, treatment strategies, imaging and culture results, and discharge diagnoses. Descriptive statistics measured baseline characteristics, imaging, and culture results.

Out of 473 patients, 64 met the inclusion criteria and 409 were excluded. Of the 64 patients, 26 (40%) had AMT and 38 (59%) had NAMT. A larger but statistically insignificant portion of patients in the NAMT cohort failed therapy (23% AMT vs 32% NAMT, P = 0.325). However, hospital readmission for LLOM within 180 days of antibiotic completion (46.2% vs 47%, P = 0.92), hospital length of stay (median (IQR): 6 (5–9) d vs 7 (5–12.5) d, P = 0.285), incidence of new renal replacement therapy initiation (0% vs 2.6%, P = 0.594), creatinine kinase levels (0 vs 2.6%, P = 0.594), and drug-induced immune thrombocytopenia (0% vs 5.3% P = 0.349) were comparable between the two cohorts.

Treatment failure rates and adverse events did not differ significantly among patients with LLOM treated with AMT or NAMT. Further investigation of determinants of clinical failures in LLOM may help optimize overall treatment.
