This article examines how care encounters at the elders’ homes are forged, and how the way these encounters are forged avoids or evokes the social imaginary of the fourth age. Data were gathered in Portugal from elders receiving home care (16 cases), their care workers (eight cases) and family carers (six cases), through participant observation and informal conversations (conducted at the elders’ homes), as well as focus groups. The collected data were analysed according to the procedures of Framework Analysis. This study found five forms of care encounters – marked by conflict, infantilisation, burden, harmony and indifference – the harmony form being the only one found to maintain the fourth age at a distance. It concludes that home care has a Janus-like nature in relation to the fourth age, and that the way home care encounters are forged depends on the conditions of the care settings and the actions of all participants in care encounters. It also concludes that it is difficult to maintain the social imaginary of the fourth age at a distance when the elders exhibit high levels of infirmity. Finally, it concludes that family carers play a crucial role in the way care encounters unfold. Implications for practice and policy include vocational training regarding the relational component of care, and information and educational programmes for family carers.

The changing retirement landscape calls on employers to develop practices that respond to individuals’ retirement needs. Line managers are a key stakeholder in managing retirement and this study focuses on how they respond to employee retirement scenarios. This empirical work examines manager openness to involvement in retirement, focusing on three sets of explanatory variables: manager psycho-social characteristics (experience of managing older workers, intention to work past 65), their decision-making environment (influence, discretion and decision-making support) and older employee situational factors (performance, ease of replacement, retirement affect and attitude to work). Data were collected from 129 managers in the United Kingdom's university sector using survey items and a factorial vignette design. The multi-level analysis found support for each category of variables in predicting manager openness to involvement in employee retirement. Managers with more experience of managing older workers were more likely to be open to involvement although managers’ own retirement intentions were not significant as a predictor. Decision-making environment variables were significant predictors of manager openness to involvement. The only older employee situational factor that was associated with manager openness to involvement was employee performance. Practically, organisations need to recognise the potential influence that managers have on employee retirement decisions and this study's findings show that managers may need training to help them understand their own role in supporting older employee retirement.

Older people now currently drink alcohol more frequently than previous generations, indicating a need to understand how this influences health and wellbeing in older adults. However, knowledge and awareness of the changing role alcohol plays in the lives of older people is not necessarily widely understood by allied health professionals in acute hospital contexts. In turn, conversations about drinking alcohol in later life may not be routinely addressed as part of practice, limiting an older person's choice to make informed decisions about their drinking. This paper qualitatively examines when occupational therapists (N = 17) in an acute hospital setting will initiate a conversation with older people (65+ years) about their drinking, guided by a theoretical lens that encompasses both person-centredness and collective occupation. Adopting a qualitative methodology, this study illustrates a typology of reasoning describing how, and in what circumstances, therapists ask older people about their alcohol use. Three themes were generated that provide further insight into the typology, these being ‘hesitancy in practice’, ‘failure to link life transitions to alcohol use’ and ‘challenges of focusing on healthfulness’. These findings provide a potentially useful tool for therapists, services and organisations to self-assess their approach to asking older people about alcohol use; a necessary element of professional health-care practice as social trends in alcohol use continue to increase.

Male ageing is in focus nowadays. The aim of this study was to investigate conceptions of the male menopause (also known as andropause) in the educational and professional literature of Finnish physicians from 1982 to 2002, and the main point of interest was how the presentation of the male menopause and its treatments have changed. Published items for analysis were retrieved from the two main Finnish medical journals and from introductory gynaecology and urology textbooks using keywords for male ageing and hormones. It was found that disagreements about the male menopause have been marked. Some authors described it as a consequence of the decline in gonad functioning that comes with increased age, and some argued that we are making a disease out of normal ageing, but its association with sexual problems has risen in prominence: libido and potency disorders have recently been identified as symptoms. The treatment provided for male menopause was androgens, about which opinions diverged, especially the effect of androgen therapy on cardiovascular diseases and osteoporosis. New forms of testosterone treatment have been eagerly adopted, but opinions varied on the appropriate duration of the therapies. By the 2000s, the male menopause was increasingly likened to the female menopause, with emphasis upon the similar symptoms. While gerontological thinking largely sees the male menopause as an aspect of ageing and a normal condition, the andrological approach regards it as a treatable disease and its rapid adoption can be seen as a reflection of both private and public concerns about increased longevity.

In mainland China, as the population ages, Alzheimer's disease and related dementia (ADRD) is estimated to increase among Chinese older adults. Chinese older adults tend to hold stigmatising beliefs about ADRD that in turn affect their help-seeking behaviour and receipt of prevention and treatment. The Framework Integrating Normative Influences on Stigma provides a rationale for Chinese older adult's stigma about ADRD. Questionnaires were administered in person to 754 older adults (42% male, mean age = 69.54 years) from two urban communities in mainland China. We examined ADRD stigma and the associations with real-life exposure, knowledge of ADRD, health conditions and social networks. This study found that Chinese older adults who had good family quality, lower depression (as measured by the Center for Epidemiological Studies Depression Scale) and better cognitive health (as measured by the Montreal Cognitive Assessment) were more likely to have lower perceived stigma. Conversely, those individuals who experienced neglect and had more ADRD knowledge exhibited higher levels of perceived stigma. Social networks moderated the associations between cognitive scores and perceived stigma. This research suggested that the quality of one's social networks is essential to reduce perceived stigma among Chinese older adults. Future research should continue to explore ADRD stigma among Chinese older adults to help guide relevant interventions, services and supports for this population.

Therapies to save or sustain the lives of elderly patients are sometimes used when they can bring little benefit, and sometimes denied because of age to patients who could benefit. Selection for treatment should depend on balancing probable benefits and burdens for the patient and on the patient's preferences. Burdens include those of the treatment itself, the risk of complications and of extending life of poor quality. Factors to consider when deciding are illustrated by reference to intensive care, surgery, dialysis, cardiopulmonary resuscitation and tube feeding.

Based on an ethnographic study of an old age home in Israel, this essay presents an analysis of how religious activity is used as part of a survival strategy by elderly residents in an institution for the well-aged. The study describes the symbolic manipulation of religious symbols and the social meaning of religion in an old age home in terms of four behavioural levels: (a) the synagogue members and their study groups; (b) the rabbi's lack of identification with his congregants; (c) the relationship to the non-religious management of the home; and (d) the differentiation of categories of residents in terms of ‘proper functioning’. Religion in this context is thus seen as an important social resource used in interactions with the social structure of the institution.

The position of the older person in society as well as in the workforce is receiving an increasing amount of attention at the present time. However there is very little historical knowledge as to the forces which shaped the withdrawal of older persons from gainful economic activity earlier this century. The research outlined below explores some of the precedents which shaped contemporary attitudes towards older workers in general, and the relationship between age and unemployment in particular — during a period of low labour demand.

This is a retrospective cohort study based on data from five nursing homes which aims to appraise how physical and cognitive characteristics of nursing home residents were associated with the use of restraints, and to provide information on their prevalence in Spain. The goal was to assess, in a visual way, the possible interactions between the nursing homes residents’ characteristics and their association with the use of restraints. Motivation, risk factors, characteristics of the residents analysed by validated rating systems that assess mobility, level of dependence, cognitive condition and nutritional status, and their association with the use of restraints, were described by means of linear and non-linear multivariate approaches in the form of self-organised maps. Findings showed that the prevalence of restraints was high when compared to other developed countries. The visual analysis reinforced the knowledge that a greater impairment was associated with the use of restraints and vice versa. However, the residents’ characteristics were not always associated with the use of restraints. Subjective factors seem to play a relevant role in decision-making, so it is important to assess risk factors continuously and determine the actual need for the use of restraints from an individual perspective by basing the criteria on specific objectives, and on consistent, reproducible and reliable methods. Initiatives to minimise these subjective factors should be promoted. Likewise, a clear definition of physical restraints should be offered at each centre. In addition, effective legislation that clearly states the need, alternatives and motivation for the use of restraints is needed.

The need to ensure the economic wellbeing and quality of life of those who reach the official retirement age is a matter of concern in the world of social services and in social policy making. Since some working older adults may be forced to retire when they reach the official retirement age while others retire voluntarily, the study is based on a dedicated survey among 508 persons who retired both willingly and unwillingly for good after reaching the official retirement age and 437 persons who continued working uninterruptedly. The findings show that the odds of staying on the job after retirement age are contingent on the socio-demographic and health-related characteristics of the older adult. Furthermore, the economic predictors of remaining in the labour force after retirement age depend on how strongly the older adult wishes to retire. Those who continue working after retirement age and those who retire willingly are undifferentiated in the level of financial support that they give others. Comparing older adults who continue working uninterruptedly with those who retire unwillingly, the chances of being among the former are higher among those who are better off before reaching retirement age. These results emphasise the need to extend welfare and financial-support policies to older adults who are forced to retire, in order to minimise the economic blow that this path to retirement causes.

Since the publication of her first book in 1988, Margaret Gullette has been a vigorous exponent of the view that the imaginative novels we read about ageing are an important cultural resource for making sense of the biological processes of growing older. Through the emphasis they choose to give to positive or to negative aspects of ageing and old age, authors can encourage and support the ageist tendencies in western culture, or alternatively can celebrate creativity and renewal in later life and, in so doing, make a potentially age-liberating contribution to the diminishment of prejudice against older people.

Widowhood is often described through stereotypes and images of passive, weak, lonely and dependent women. This study presents additional aspects of widows who have chosen to continue their lives without a new spouse. It thus joins the recently growing body of knowledge which presents widowhood in a less one-dimensional way while referring to the complexity and different layers of widows' lives. A qualitative study was conducted with 30 women aged 63–87 who had been widows for 1–34 years. The findings showed that the dominant experience shared by all women is one of liberation and freedom – a feeling of independence that is expressed socially, personally and economically. Thus, alongside feelings of loneliness and adaptation difficulties, they also experience strength and empowerment. The term ‘gender contract’ serves as a theoretical explanation of the independence narrative that characterises the widows; the termination of the couple's gender contract allows them to redefine their priorities, identify themselves as active instead of passive figures and create for themselves a place of their own. By bringing widowhood to the centre of the discussion, the study gives legitimacy to a discourse on feelings less spoken about, such as relief, freedom and independence. In this way, the study contributes to the ongoing debate on widows by shattering the accepted myth of widows as vulnerable, weak and dependent but without underestimating the difficulties or ignoring the women's diversity.

Critical and feminist ageing scholarship has drawn attention to how dominant discourses of ageing negatively impact older women's identities and social lives. While research intersecting migration and ageing has broadly focused on older immigrants' settlement experiences, very little is known about the discursive influences over older immigrant women's sense of agency and social relations. To address this knowledge gap, this study explores the subjectivity-formations of older South Asian Muslim women engaged in transnational migration. Narrative and discourse analysis principles were used to conduct and analyse life story-based interviews with 15 South Asian Muslim older women who recently migrated to Toronto, Canada. Findings indicate that participant subjectivity-formations are shaped by: (a) discourses of ageing; (b) socio-cultural–religious discourses; and (c) discourses of transnational migration. In response, participants engaged in dynamic strategies including conforming, negotiating, creating alternative narratives and/or resisting these discourses to organise their lives. These findings reinforce the continued need to trace the governmentality of structural conditions over ageing migrant women's lives and their responsive strategies to manage these impacts.

Despite a large body of research on the effects of widowhood on health, little is known about whether spousal loss is related to functional impairment in widowed persons. This study examines the trajectories of functional impairment (sensory and masticatory functions) before and after spousal loss. This study also investigates whether the temporal changes in functional impairment of widowed people are gendered. Using data from the Korean Longitudinal Study of Ageing over seven waves (42,967 person-observations), this study estimated fixed effects regression models to account for unobserved individual-level heterogeneity. Gender-stratified fixed effects regression models were used to determine whether changes in functional impairment associated with spousal loss differ by gender. The results of this study indicated that the vision of widowed people began to decrease within the first year following spousal loss and persisted through the fourth and subsequent years. By contrast, mastication deterioration occurred only among widowers. Masticatory impairment began during the first year of spousal loss and lasted the entire survey period. No statistically significant reduction in hearing loss was found for both widowers and widows. The results of this study suggest that spousal loss has a long-term effect on functional impairment, particularly in vision and masticatory functions. This study also documents gender heterogeneity in the trajectories of functional impairment before and after spousal loss. Vision impairment was found to be universal among widowers and widows, whereas masticatory impairment was significant only among widows. To address the physical and psychological vulnerability of widowed people, policies should be developed early in the process of adjusting to widowhood.

For older persons, age discrimination can undermine equal participation in modern service and consumer societies. One way to tackle age discrimination is to implement anti-discrimination policy and legislation. However, age discrimination is an ambiguous concept that can be interpreted in different ways and differences in the interpretation can impact the scope and application of related policy and legislation. Hence, to understand how policy, legislation and their central constructs operate within society, it is necessary to look beyond the letter of the law or policy text and explore the interpretations of age discrimination employed by actors in the field. We interviewed experts from Austria and Ireland, who by virtue of their professional or representative position co-structure the conditions for people to claim they have experienced age discrimination. Based on 12 expert interviews and two focus groups, each involving six representatives of the major national advocacy and interest organisations on ageing, we reconstructed four interpretation patterns of age discrimination: age discrimination as the ‘denial of dignity’, as the ‘denial of recognition of contribution’, as the ‘denial of participation’ and as the ‘lack of consideration of need’. The findings are discussed with reference to the concepts of recognition and representation, and considerations are provided on possible legal and political implications of this research.
