Summary

This chapter focuses on the initial management of patients with ischemic stroke. It presents a case study of a 54-year-old female with a past medical history significant for hypertension and bilateral carotid stenosis. A head computed tomography (CT) scan was significant for evidence of a large right middle cerebral artery (MCA) infarct. The patient was transferred to an institution capable of performing intra-arterial thrombolysis. Emergency evaluation of a patient with possible ischemic stroke should initially focus on rapid assessment and stabilization of the airway, breathing, and circulation. Early diagnosis of acute ischemic stroke is facilitated with the wide spread use of neuroimaging. Intravenous thrombolytic therapy with recombinant tissue plasminogen activator (rtPA) has been shown to improve outcomes following ischemic stroke. As new data on management of ischemic stroke continue to emerge, guidelines will be updated with the latest evidence-based recommendations.
