Hyponatrémie induite par la carbamazépine et affection rénale: à propos d’un cas

Résumé

L‘hyponatrémie constitue un effet secondaire rare de la carbamazépine, qui possède une action antidiurétique dont le mécanisme reste controversé. L‘existence avant le traitement d’un taux de sodium à la limite inférieure de la normale et l’association aux diurétiques semblent constituer des facteurs de risque prédisposant à l’hyponatrémie. Nous présentons le cas d’une patiente bipolaire, porteuse d’un rein unique, qui a développé une hyponatrémie après 5 jours de traitement par carbamazépine. Nous suggérons le rôle favorisant joué par la pathologie rénale dans le déclenchement de l’hyponatrémie.

Summary

Hyponatremia represents a rare side-effect of carbamazepine, which exhibits an antidiuretic activity of controversial pathophysiology. The presence before treatment of a sodium level in the tower range and the association with diuretics seem to represent factors increasing the risk for hyponatremia. We presented the case of a bipolar female patient with a single kidney who developed hyponatremia after 5 days of treatment by carbamazepine. We suggest a predisposing role of the renal pathology in the appearance of hyponatremia.
