Variations between a Day Hospital's Team Assessments and Needs Perceived by Referral Source

Abstract

Following 14 months of operation, all 142 patients assessed by the Day Hospital program at Seven Oaks General Hospital in Winnipeg were reviewed. Needs identified through referrals from physicians were compared with those assessed by the Day Hospital team. Findings showed that medical, nursing and learning needs were most frequently not identified and more than half the referrals identifying a single need were assessed as having multiple needs. However, the ranking of needs by referrers and assessors were identical.

The review confirmed that this Day Hospital met the objective of assessing the multiple factors affecting the well-being of the elderly.

Les 142 malades examinés dans le cadre du programme administré par le Seven Oaks General Hospital de Winnipeg ont été passés en revue au bout de 14 mois. Les besoins identifiés par les médecins qui les avaient adressés ont été comparés à ceux qui avaient été reconnus par l'équipe hospitalière. Les résultats montrent que les besoins de soins médicaux, infirmiers ainsi que de réadaptation d'avaient, le plus souvent, pas été reconnus, et que, dans plus de la moitié des cas, un seul besoin avait été signalé alors que le malade en présentait plusieurs. Toutefois, l'ordre de présentation des besoin s'est avéré identique chez les médecins traitants et les membres de l'équipe.

Cette revue confirme le fait que l'hôpital de jour atteint les objectifs fixé au diagnostic des multiples facteurs affectant la santé des personnes âgées.
