Blastocyst transfer is now the norm for the majority of women undergoing a fresh IVF/ICSI cycle as there is a good amount of evidence to show that this enhanced embryo selection increases the chance of pregnancy per embryo transfer. The culture environment is critical for embryo development, and recent studies using single step media combined with time-lapse incubators have demonstrated an increase in clinical pregnancy rates. This may in part be due to the use of morphokinetic data to enhance selection of the embryo for transfer. However an increase in cumulative pregnancy rates suggests that undisturbed culture may contribute to this uplift. As the cleavage grade of the embryo is not always predictive of blastocyst development, it is not therefore beneficial to remove the embryos from their stable culture environment.
