Monkeypox (MPX) is a rare zoonotic illness, like smallpox, caused by the monkeypox virus, which is a member of the Orthopoxvirus genus of the Chordopoxvirinae subfamily that falls under the classification of the Poxviridae family. MPX is clinically characterized by a wide variety of symptoms and signs, including fever, sore throat, headache, myalgia, lymphadenopathy, and rashes. As the world is undergoing progressive industrialization over time, there is a corresponding increase in environmental pollutants and deforestation. Previous studies have found a correlation between exposure to environmental contaminants and the incidence of MPX. Additionally, it has been hypothesized that deforestation may also have played a role in the disease’s resurgence or in its ability to spread. Habitat loss and ecological instability brought on by environmental contaminants and deforestation may increase human-infected animal interaction and hasten the spread. The likely connection should be known by health authorities and doctors, as well as government officials, to help fund further investigations and craft strategies to combat the risk of an increasing prevalence of MPX in the world, especially in densely populated underdeveloped regions of Asia and Africa, where containment of MPX poses greater challenges. In this article, we have provided an important real-world perspective and suggested future recommendations to halt the further spread of MPX to new places.

Before the described monkeypox virus (MPXV) case in this article, limited cases of human MPXV had occurred within the United States. Lessons learned from prior outbreaks contributed to a successful approach to decontamination and containment of an adult case of MPXV in Dallas, Texas. This case report provides an overview of the characteristics of the monkeypox virus, its effects on the human body, and a primary focus on a successful protocol for household decontamination. Additional information is provided on methods related to public relations and information management that were utilized during this incident.

We analysed the transmission of the human mpox virus in Spain by estimating the effective reproduction number of the disease from official surveillance data. Our computations show that this decreased steadily after an initial burst phase, dropping below 1 on July 12, and thus the outbreak was expected to reduce in the following weeks. Differences in trends were found across geographical regions of the country and across MSM and heterosexual populations.

Human monkeypox (mpox) virus is a viral zoonosis that belongs to the Orthopoxvirus genus of the Poxviridae family, which presents with similar symptoms as those seen in human smallpox patients. Mpox is an increasing concern globally, with over 80,000 cases in non-endemic countries as of December 2022. In this review, we provide a brief history and ecology of mpox, its basic virology, and the key differences in mpox viral fitness traits before and after 2022. We summarize and critique current knowledge from epidemiological mathematical models, within-host models, and between-host transmission models using the One Health approach, where we distinguish between models that focus on immunity from vaccination, geography, climatic variables, as well as animal models. We report various epidemiological parameters, such as the reproduction number, R0, in a condensed format to facilitate comparison between studies. We focus on how mathematical modelling studies have led to novel mechanistic insight into mpox transmission and pathogenesis. As mpox is predicted to lead to further infection peaks in many historically non-endemic countries, mathematical modelling studies of mpox can provide rapid actionable insights into viral dynamics to guide public health measures and mitigation strategies.

Monkeypox 2022 exhibits unprecedented human-to-human transmission and presents with different clinical features than those observed in prior outbreaks. Previously endemic only to West and Central Africa, the monkeypox virus spread rapidly world-wide following confirmation of a case in the United Kingdom on May 7, 2022 of an individual that had traveled to Nigeria. Detection of cases with no travel history confirms on-going community spread. Emergency Medical Services (EMS) professionals will likely encounter patients suspected or confirmed to have monkeypox, previously a rare disease and therefore unfamiliar to most clinicians. Consequently, it is critical for EMS medical directors to immediately implement policies and procedures for EMS teams – including emergency medical dispatchers – to identify potential monkeypox cases. These must include direction on actions EMS professionals should take to protect themselves and others from virus transmission. Monkeypox 2022 may manifest more subtly than it has historically. Presentations include a subclinical prodrome and less dramatic skin lesions – potentially limited to genital or anal body regions – which can be easily confused with dermatologic manifestations of common sexually transmitted infections (STIs). While most readily spread by close contact with infectious skin lesions on a patient, it is also transmissible from fomites, such as bed sheets. Additionally, droplet transmission can occur, and the virus can be spread by aerosolization under certain conditions. The long incubation period could have profound negative consequences on EMS staffing if clinicians are exposed to monkeypox. This report summarizes crucial information needed for EMS professionals to understand and manage the monkeypox 2022 outbreak. It presents an innovative Identify-Isolate-Inform (3I) Tool for use by EMS policymakers, educators, and clinicians on the frontlines who may encounter monkeypox patients. Patients are identified as potentially exposed or infected after an initial assessment of risk factors with associated signs and symptoms. Prehospital workers must immediately don personal protective equipment (PPE) and isolate infectious patients. Also, EMS professionals must report exposures to their agency infection control officer and alert health authorities for non-transported patients. Prehospital professionals play a crucial role in emerging and re-emerging infectious disease mitigation. The monkeypox 2022 3I Tool includes knowledge essential for all clinicians, plus specific information to guide critical actions in the prehospital environment.
